Nineteen gastric carcinomas with lymphoid stroma were selected from 554 surgical cases and examined pathologically and immunohistochemically using formaldehyde-fixed, paraffin embedded materials. Most showed ulcerative lesion and 15 cases located in fundic and cardiac gland regions. They were subdivided histologically into three groups, early (group I), localized (group II) and infiltrative tumors (group III), the number of cases being 2, 10 and 7, respectively. Lymph node metastases occurred in 3 cases in group II and 6 in group III, the latter showing a significantly higher incidence. The number of carcinoembryonic antigen and CA19-9 immunoreactive tumor cells was apparently smaller in gastric carcinomas with lymphoid stroma than in ordinary gastric carcinomas. Frequent presence of alpha 1-antichymotrypsin immunoreactivity characterized the tumor cells of gastric carcinoma with lymphoid cells. Stroma cells consisted of lymphocytes, plasma cells, granulocytes and histiocytes. Of these, the greatest number examined immunohistochemically was B cells and IgG cells, followed in descending order by T cells, IgA cells and IgM cells in the order given. A variable number of
lysozyme
immunoreactive histiocytes were also detected in all the cases.
Gastric carcinoma
with lymphoid stroma might be subclassified as a separate entity, although short term follow-up study did not demonstrate a favorable prognosis for this type of gastric cancer.

Eighteen argyrophil cell carcinomas in 101 early gastric carcinomas were explained histologically, ultrastructurally, and immunohistochemically for polypeptides, carcinoembryonic antigen (CEA),
lysozyme
, and human chorionic gonadotrophin (hCG). Seven of these 18 tumors had gastrin, and two of seven tumors also contained somatostatin. In all of these 18 tumors CEA were demonstrated. Seven had

simultaneously. Argentaffin cells were found in seven of 18 tumors. Of the above seven tumors containing gastrin, three had argentaffin cells. Ultrastructurally, several types of secretory granules were noted and tumor cells resembling D1- or P cells were present in nine of the 18 tumors. Macroscopically, many of the tumors showed IIc or IIc + III type. Histologically, the 18 tumors consisted of six well differentiated adenocarcinomas and 12 poorly differentiated adenocarcinomas including signet-ring cell carcinoma. These 12 tumors frequently developed in the stomach of young females. Two cases with primary gastric Ki-1 positive anaplastic large cell lymphoma are presented. Morphologic features of both cases involved pleomorphism of the neoplastic cells, fibrosis and lymphatic infiltration. The neoplastic cells in both cases were positive for BerH2 (CD30), LCA(CD45),
lysozyme
and alpha-1-antitrypsin (alpha 1-AT). In additional case, the neoplastic cells were additionally positive for MAC387 and alpha 1-antichymotrypsin (alpha 1-ACT). The neoplastic cells in these cases were negative for L26(CD20), UCHL-1(CD45RO), DAKO CD3 and epithelial membrane antigen (EMA). According to the results of the phenotypic studies, the authors consider that the neoplastic cells have some of the features of histiocytes. Both patients at 2 and 8 years after surgery without chemotherapy are disease free. This lymphoma is well known to be frequently misdiagnosed as undifferentiated carcinoma. Although rare in occurrence, recognition of this primary lymphoma in the stomach has a significant clinical implication, as the authors consider that its prognosis might be better than undifferentiated

Gastric carcinoma
is the fourth most common cause of cancer death worldwide but its molecular biology is poorly understood. We catalogued the genes expressed in two gastric adenocarcinomas and normal stomach, using serial analysis of gene expression (SAGE), and compared the profiles on-line with other glandular epithelia. Candidates were validated by Northern blotting and immunohistochemistry. A total of 29 480 transcripts, derived from 10 866 genes, were identified. In all, 1% of the genes were differentially expressed (>/=fivefold difference plus P-value </=0.01) between cancers and normal stomach. The most abundant transcripts included ribosomal and mitochondrial proteins, of which most were upregulated in the tumours, as were other widely expressed genes including transcription factors, signalling molecules (serine/threonine protein kinases), thymosin beta 10 and collagenase I. Transcripts abundant in normal stomach were functionally important, including gastrin, immunoglobulin alpha,

, MUC5, pS2 and pepsinogens, which were among 55 gastric-specific genes. Many transcripts were minimally characterized or new, some cancer-associated genes reflected their intestinal morphology, and some normal gastric genes had previously been considered as pancreatic carcinoma markers. The gastric carcinoma profiles resembled other tumours', supporting the existence of common cancer-associated targets. These data provide a catalogue from which to develop markers for better diagnosis and therapy of gastric carcinoma.
