Internationally, breast cancer mortality is correlated with intestinal
lactase
sufficiency and dairy product consumption beyond childhood. Within the United States, age-adjusted breast cancer mortality is positively associated with consumption of milk, butter, and total milk fat in regional analyses, and it is associated with milk demand in state-based analyses. Breast cancer mortality is also positively associated with demand for total calories, protein, fat, beef, and table fats (butter and margarine), and it is negatively associated with egg demand. Only the associations with milk and egg demand, however, survive when the Southern states are eliminated from the analyses or when either age of first marriage or income is controlled. The associations with milk and egg demand persist despite multiple controls for other dietary and demographic variables, although the association with milk demand loses statistical significance in some second- and third-order partial correlations. The inverse correlation with egg demand is strong but in the opposite direction from what might have been expected from previous studies. The correlation between milk demand and breast cancer mortality, although weaker, is consistent with results from previous studies, and it suggests a possible special role for dairy products in the etiology of breast cancer.

The brush border of normal small-intestine epithelial cells is rich in enzymes that are involved in the digestive process. Such molecules can be used as markers to analyze cell lineages and differentiation properties of colorectal cancers. Monoclonal antibodies detecting dipeptidyl peptidase-IV, aminopeptidase N, endopeptidase F, sucrase-isomaltase, alkaline phosphatase, maltase-glucoamylase and
lactase
have been used to analyze the phenotype of colorectal cancers, adjacent mucosa and histologically normal distant mucosa. The avidin-biotin peroxidase complex method was used. Expression of dipeptidyl peptidase-IV, aminopeptidase N, sucrase-isomaltase and alkaline phosphatase was common in non-neoplastic mucosa adjacent to, and distant from, the tumor; in contrast, endopeptidase F, maltase-glucoamylase and
lactase
were rarely expressed in normal distant mucosa and more frequently expressed in mucosa adjacent to the tumor. Dipeptidyl peptidase-IV, aminopeptidase N, endopeptidase F, sucrase-isomaltase and alkaline phosphatase were frequently expressed in colorectal cancers, whereas maltase-glucoamylase and
lactase
were rarely expressed. Two general patterns of antibody reactivity were observed: diffuse cytoplasmic and apical; apical reactivity was generally associated with more differentiated tumors. A logistic predictive regression model indicated that enzyme expression in colorectal cancers followed a coordinate pattern, but was unrelated to the location of the tumor, Dukes stage or differentiation grade. In conclusion, expression of brush-border-associated enzymes occurs frequently in colorectal cancers and is regulated in a co-ordinated manner. These markers can be used for the phenotypic sub-classification of colorectal cancers.

Expression and cellular localization of brush-border enzymes (aminopeptidase N, dipeptidylpeptidase IV,
lactase
, maltase) in normal human colon, colonic polyps and malignant intestinal tumors were investigated with a panel of monoclonal antibodies reacting with either native or denatured proteins. The enzymes were detected on cryostat sections by indirect immunofluorescence staining, or affinity-purified and analyzed by gel electrophoresis and immunoblotting. Dipeptidylpeptidase IV,
lactase
and maltase were absent from all samples examined, while aminopeptidase N (APN) was detected at the basal membrane of the epithelial cells in most specimens of colon obtained from individuals free of intestinal tumors. In contrast, APN was frequently localized at the luminal membrane of the surface epithelium in large-intestinal mucosa distal to tumors, adenomas and hyperplastic polyps, and from members of hereditary colon cancer syndrome families. APN was also expressed in colonic tumors, where it was present in an apical cell membrane location in 3/23 adenomas and 14/35 adenocarcinomas examined. No correlation was found between tumor-cell invasiveness (classified by "Dukes" stage) and expression or cellular location of aminopeptidase N. Histologically, all positive tumors were moderately or well differentiated. These results suggest that aminopeptidase N is normally expressed in adult human colon, but epithelial cells in the large and small intestine differ in their ways of sorting this enzyme intracellularly and eventually inserting it into different aspects of their surface membrane, a process which may be altered at an early stage of carcinogenesis.

Researchers do not understand what causes ovarian cancer. Some studies find galactose to be toxic to oocytes which renders ovaries susceptible to
cancer
. 1 hypothesis is that high levels of lactose consumption and retained ability to digest lactose (
lactase
persistence) lead to high levels of galactose exposure which increases the ovarian cancer risk. Denmark, Sweden, and Switzerland have the highest risk of ovarian cancer in the world. They also have the highest levels of
lactase
persistence and among the highest levels of milk supply for the population. Conversely, Hong Kong, Japan, Shanghai, and Singapore have low ovarian cancer risk, low consumption of milk, and low levels of
lactase
persistence. yet other researchers using the same data did not find per capita availability of milk to be significant independent of nondairy animal fat. Lactase persistence did have an independent association, however, but data was not standardized across populations. In a case control study, researchers took the dietary history of 11 dairy products to measure lactose consumption. A significant increase in ovarian cancer risk only existed with frequent intake of yogurt and cottage cheese. Researchers hypothesized that the prehydrolyzed lactose in both foods was the main contributor. Using the case control data, others found an increased risk for total lactose intake in women who never used oral contraceptives (OCs). The strongest predictor to date is the lactose/transferase ratio, yet those who found this association did not define this index based on an earlier study or examination. 1 problem with these studies is that dietary fat confounds the association. 2 studies showed that low fat milk was associated with decreased risk, even though both milks have the same lactose content. Another problem is that the subsets of exposure or subgroups of the population are unpredictable. Further research in nutritional epidemiology of ovarian cancer is needed.

Twenty human colon carcinoma cell lines were studied for their ability to develop some of the characteristics of the normal intestinal epithelium, e.g., epithelial polarity, presence of the actin-binding protein villin, or the occurrence of an enterocytic differentiation either when cultured under standard conditions, as for Caco-2 cells, or when grown in a glucose-free medium, as for HT-29 cells. Except for the regular presence of villin, which can be considered a marker of the colonic origin of the cells, the cell lines of this study could be classified into four types with regard to their differentiation characteristics. In type 1 (only one cell line, i.e., Caco-2) the cells undergo spontaneously an enterocytic differentiation characterized by a polarization of the cell layer with the formation of domes and the presence of an apical brush border the membrane of which is endowed with hydrolases such as sucrase-isomaltase,
lactase
, amino-peptidase N, dipeptidylpeptidase IV and alkaline phosphatase. In type 2 (three cell lines: HT-29, HCT-EB, and HCT-GEO) the cells are undifferentiated when grown in the presence of glucose but undergo an enterocytic differentiation when grown in the absence of glucose. In type 3 (eight cell lines: HCT-GLY, HCT-FET, HCT-FRI, HCT-CBS, HCT-ALA, Co-115, HRT-18, and SW-1116) the cells are organized into a polarized monolayer with the formation of domes but without any enterocytic differentiation characteristics, whatever the culture conditions. In type 4 (eight cell lines: HCT-116a, HCT-R, HCT-RCA, HCT-Moser, HCT-8R, SW-480, LS-174T, and Vaco-9P) the cells are organized into a multilayer without any feature of epithelial polarity or enterocytic differentiation, whatever the culture conditions.

patients and from uninvolved mucosa of all main segments of the large bowel were examined histologically and assayed for a series of organelle marker enzymes. Six enzymes--

, sucrase, alkaline phosphatase, 5'-nucleotidase, acid phosphatase, and N-acetyl-beta-D-glucosaminidase--showed less activity in adenomas than in adjacent uninvolved mucosa and in specimens from controls.

The serum
lactase
dehydrogenase (LDH) of 56 patients with Ewing's sarcoma was examined to determine the use of LDH as a tumor marker. The initial LDH level was used to determine its ability to predict survival. The follow-up LDH level also was examined, as an indicator of recurrent disease. The initial LDH level was found to have no prognostic value. Both patients with elevated and normal LDH levels had the same survival rate. The LDH level at recurrence was significantly higher, however, than the follow-up LDH level of those without recurrence (P less than 0.001). The LDH level was most sensitive as a tumor marker for recurrent disease in those patients with multiple sites of tumor involvement at the time of recurrence.

The clinical and nutritional significance of radiation enteritis was assessed in eight patients with chronic diarrhea which followed curative doses of radiotherapy for pelvic
malignancies
. Steatorrhea, found in seven malnourished patients, was ascribed to ileal disease or previous surgery, or to bacterial contamination of the small intestine. Lactose intolerance, assessed by breath hydrogen excretion after oral lactose and by jejunal
lactase
levels, was found in six patients. In a subgroup of five patients, the administration of two different defined formula liquid diets by nasoduodenal infusion decreased fecal fluid and energy losses by about one-half. Compared to Vivonex-HN, the infusion of Criticare-HN was associated with greater likelihood of intestinal gas production but a three-fold greater utilization of protein. Intestinal malabsorption and malnutrition in radiation enteritis has diverse etiologies. Whereas nutritional support by liquid diet limits fecal fluid and energy losses, these diets differ significantly in clinical tolerance and biologic value.
