In vitro trials with washed spermatozoa incubated in medium containing 1 mg/ml of the methyl xanthine phosphodiesterase inhibitor PF showed improved counts of total motile and total progressively motile spermatozoa in cases of oligospermia/asthenospermia. Application of this agent in a PROST program for a series of nine couples presenting for treatment with histories of failed fertilization in vitro resulted in five pregnancies (four singleton, one triplet) and the subsequent delivery of normal infants. The results warrant further evaluation of this sperm treatment for cases of severe male factor infertility.

Treatment of spermatozoa with 10 mM caffeine increases the rotational mobility of thiol-containing proteins in a defined micellar micro-environment. This is associated with inhibition of superoxide dismutase activity and augmented superoxide anion radical generation. Increased sperm competence in presence of caffeine in human oligospermia is explained by these observations. Inhibition of cyclic phosphodiesterase by caffeine is suggested to be part of the observed lattice-rearrangement.

Low superoxide anion radical levels, low membrane fluidity and high superoxide dismutase activity are some of the defects existing in oligospermia. The phosphodiesterase inhibitor pentoxifylline has been used as a motility stimulant in the treatment of oligospermia in vitro. The mechanism of its action is unclear. Our results show significant inhibition of superoxide dismutase, increase in lipid peroxidation and membrane fluidity after pentoxifylline treatment. A biophysical modulation of the sperm membrane by an oxyradical mechanism in oligospermia is discussed.
