We detected the virus genome in ocular samples from a 65-year-old woman with clinically diagnosed acute retinal necrosis using DNA amplification. She exhibited occlusive retinal vasculitis, confluent necrotizing retinitis, mainly peripheral, and iridocyclitis. For DNA amplification, we used recently published primers specific for varicella-zoster virus (VZV) and herpes simplex virus. Using VZV primers, we detected the VZV genome in the aqueous humor, but not in the vitreous, by amplifying a DNA fragment 642 base pairs in length. HSV DNA was not detected. After detecting the VZV genome, PstI restriction endonuclease was used because an epidemiological study found that about 25% of the VZV strains in Japan carry a mutation lacking a PstI recognition site. The VZV genome from the patient had a PstI cleavage pattern, while the positive control had a VZV genome that carried a PstI-site-less mutation. We considered our patient with acute retinal necrosis to be infected with VZV having a PstI site.

To determine if cytomegalovirus (CMV) retinitis occurs more frequently in patients infected with certain strains CMV isolates from the blood of 44 patients with advanced human immunodeficiency virus disease were grouped by the DNA sequence or the restriction endonuclease digest pattern of a portion of the glycoprotein B (gB) gene. Forty-two patients (95%) were followed clinically until the development of CMV retinitis or death. Fourteen (78%; 95% confidence interval, 7%-39%) of 26 with isolates belonging to other gB groups developed CMV retinitis (P = .002). Viremia caused by gB group 2 CMV strains is associated with higher risk of CMV retinitis than viremia due to other CMV gB groups. The association of CMV gB gene with retinitis suggests this gene, or one linked to it, is a virulence factor for CMV strains causing infection in AIDS patients.

A couple requested preimplantation genetic analysis for a dominant form of retinitis pigmentosum caused by a C-->A transversion in the rhodopsin gene. Since this point mutation does not alter a restriction endonuclease site we designed two separate analytical systems, one involving site-specific mutagenesis and the other involving allele-dependent length polymorphism. After establishing the accuracy and robustness of these assay systems we utilized both systems simultaneously in a heminested polymerase chain reaction (PCR) system. This allowed accurate preimplantation diagnosis to be performed. One embryo was transferred but a pregnancy did not occur.
