Preoperative LDH, CPK, alkaline phosphatase and liver scan results were compared with the presence or absence of hepatic metastases at operation in 124 laparotomies in 123 patients with colorectal carcinomas during a three-year period. The overall accurcy rates for the four tests ranged from 74 to 84 per cent. The false-negative rates were acceptably low (9-13 per cent) for all but the CPK determination. The false-positive rates were unacceptably high for all four tests (38 to 60 per cent). A 'metastatic score' for reducing the high false-positive rate is described. In view of the high false-positive rates of preoperative screening tests, laparotomy is presently the most accurate method of diagnosing hepatic metastases.

Ten subjects were exposed to high-G on the human centrifuge using seatback angles of 13 degrees, 30 degrees, 45 degrees, 60 degrees, and 75 degrees from the vertical, and body configurations of the lower portion of the body with pelvis and legs elevated, pelvis, elevated, and pelvis elevated with knees on chest (fetal position). Tolerance was measured by peripheral light loss. Mental status, respirations, core and ambient temperatures, and ECG were monitored. Daily physio-chemical data included: creatinine, bilirubin, phosphorus, alkaline phosphatase, uric acid, cholesterol, total protein, albumin, BUN, glucose, LDH cardiac isoenzyme No. 5, SGOT, SGPT, CPK, CBC, and urinalysis. Tiredness, pressure on the chest, and general discomfort in the fetal position were reported. Physical examination demonstrated petechiae. Heart rate, respiratory rate, and temperature increased post-session. There was a significant rise in values for albumin, chloride ion, creatinine, calcium, LDH, BUN, and immature white cells; and a decrease in values for phosphorus, SGOT, SGPT, protein, uric acid CO2, globulin, hematocrit, monocytes, and eosinophils.

ASSAY CURVES, USING A DISK DIFFUSION METHOD FOR THE ANTIBIOTICS GENTAMICIN AND CEFAZOLIN, WERE PREPARED WITH: saline, saline plus 10% serum, and ascitic, synovial, cerebrospinal, and pleural fluids. The curves were compared with a standard curve prepared with pooled human serum. The pH, total protein, glucose, blood urea nitrogen, sodium, potassium, calcium, phosphorus, chloride, CO(2) content, uric acid, cholesterol, bilirubin, serum glutamic oxalacetic transaminase, CPK, LDH, and alkaline phosphatase were determined and compared for all fluids. Measurements for cefazolin levels were falsely elevated in those fluids with low protein content when serum was used as a reference standard. There was a linear inverse relationship between the protein content of the fluids and the cefazolin level with serum as the standard for the assay of this highly protein-bound antibiotic. No discrepancies were observed in the assay curves for gentamicin, an antibiotic known not to be bound by serum proteins.

The activities of aspartate aminotransferase (GOT), alanine aminotransferase (GPT), alkaline phosphatase (alkP), creatine kinase (CPK), and ornithine carbamoyltransferase (OCT) were determined in liver, heart, skeletal muscle, brain, kidney, lung, spleen, adrenals, pancreas, thyroid, thymus, and red cells of 56 bovine fetuses varying in gestational age from 115 to 255 days. The tissue aminotransferase activities were the most variable with gestational age. The GPT activity of liver, kidney, spleen, and red cells and the GOT activity of red cells decreased with fetal age. The GPT activity of heart, brain, and skeletal muscle and the GOT activity of adrenal, brain, and skeletal muscle increased with fetal age. Increasing activities were also described for adrenal and brain alkP and for brain and skeletal muscle CPK. In contrast, the OCT activities were fairly constant for each tissue as a function of gestational age.

Biochemical components usually evaluated in seminal plasma are lower than those in blood serum. In this study the concentration of different constituents in seminal plasma has been analyzed: creatinine, urea, glucose, uric acid, sodium, potassium, triglycerides, cholesterol, bilirubin, alkaline phosphatase, glutamic oxalacetic transaminase (SGOT), glutamic pyruvate transaminase (SGPT), cholinesterase, creatin phospho chinase (CPK), gamma glutamyl transpeptidase, lactic dehydrogenase (LDH), proteins, in comparison with the concentrations of the same constituents in blood. With the exception of uric acid, all the biochemical components in the seminal plasma were either significantly higher or lower than in blood serum, an index of the complexity of the mechanism regulating the presence and distribution of the single components in seminal plasma compared with blood serum. Isoelectro-focussing for proteins showed, in seminal plasma, a higher quantity of fragments and a different distribution of this in comparison with blood serum.

In 21 patients with chronic renal failure spirometric and electromyographic examinations (including m. pectoralis, m. rectus abdomini, and m. obligus abdomini) were performed before and after hemodialysis. In 10 patients treated by peritoneal dialysis and, in 35 patients treated by hemodialysis serum PTH, thyroid hormones (T4, T3, rT3), CPK, aldolaze, pyruric acid, lactic acid, alkaline phosphatase were determined. In both groups before and after dialysis serum sodium, potassium, calcium, magnesium were determined. We observed negative correlation between PTH and respiratory muscles weakness indices. This fact may confirm the contribution of PTH in uremic myopathy evaluation.

141 children, aged 1 month to 3 years, admitted to an intensive care unit in a critical condition caused by severe acute intestinal infections revealed high levels of GOT, GPT, alkaline phosphatase, LDG, and CPK activity, the degree of increment and the form of combinations depending on the type of the condition. On the basis of enzymological investigations a diagnostic table has been compiled characterizing 6 forms of critical conditions most frequently observed in children with acute intestinal infections. This facilitates early diagnosis of critical conditions and makes it possible to perform timely and adequate intensive therapy.

A herbal tea (called an abafado in Brazil) prepared from the dried leaves of lemongrass was administered to healthy volunteers. Following a single dose or 2 weeks of daily oral administration, the abafado produced no changes in serum glucose, urea, creatinine, cholesterol, triglycerides, lipids, total bilirubin, indirect bilirubin, GOT, GPT, alkaline phosphatase, total protein, albumin, LDH and CPK. Urine analysis (proteins, glucose, ketones, bilirubins, occult blood and urobilinogen) as well as EEG and EKG showed no abnormalities. There were slight elevations of direct bilirubin and of amylase in some of the volunteers, but without any clinical manifestation. These results taken together indicate that lemongrass as used in Brazilian folk medicine is not toxic for humans. The eventual hypnotic effect of lemongrass was investigated in 50 volunteers who ingested samples of lemongrass and a placebo under double-blind conditions. The parameters (i.e. sleep induction, sleep quality, dream recall and rewakening) did not show any effect of lemongrass as compared to the placebo. Eighteen subjects with high scores of trait-anxiety were submitted to an anxiety-inducing test following taking lemongrass or placebo under double-blind conditions. Their anxiety levels were similar, indicating that the abafado of the plant does not have anxiolytic properties. It is concluded that lemongrass, one of the most popular Brazilian herbal medicines, used for its alleged CNS-depressant effects, is atoxic but lacks hypnotic or anxiolytic properties.

A 6-year-old girl with cerebral palsy developed conscious disturbance and generalized convulsion after one-hour hot herb drug bath. Physical examination on admission revealed rectal temperature 41 degrees C, hot skin, respiration 46/min, regular heart beat 98/min, BP 130/60 mmHg, Glascow coma scale 4 (E2M1V1), soft and flat abdomen, no hepatosplenomegaly, no skin rash, no focal neurological sign, increased generalized muscle ton. Laboratory data showed CBC: WBC 20400 cumm (Neutrophils 31%, Lymphocytes 69%), Hb 11.6gm%, ESR 11 mm/hr, arterial blood gas: PH 7.077, PO2 43mmHg, PCO2 57.1mmHg, HCO3- 16 mEq/L, BE-11.5mEq/L, serum sodium 143 mEq./L, potassium 5.2 mEq/L, chloride 101 mEq/L, free calcium ion 3.8mg%, GOT 63IU/L, GPT 263 IU/L, amylase 193 IU/L, alkaline phosphatase 388 IU/L, LDH 1245 IU/L, CPK 677 IU/L, total bilirubin 0.8 mg/dl, direct type 0.1 mg/dl, BUN 18 mg/dl, Glucose 35 mg/dl. Urinalysis revealed proteinuria( ) trace hematuria and pyuria, but no cast. Lumbar puncture is within normal limits. Bacteriology including blood and CSF are normal. Multiple organ failure was noted at that time. Intensive cooling methods were performed including central and peripheral cooling. We used luminal and valium to control the seizure. Condition didn't improve. Afterwards cardiopulmonary arrest developed. Patient expired 8 hours after admission despite of resuscitation. Heat stroke in infancy and childhood is different from that in adulthood. The predisposing factors are high ambient temperature, dehydration, very young baby, sweat gland dysfunction, or ectodermal dysplasia. Definition of heat stroke includes 1) rectal temperature above 41 degrees C, 2) behavioral change, 3) warm skin, wet or dry.(ABSTRACT TRUNCATED AT 250 WORDS)
