To ascertain modifications in the activation products derived from oxygen free radicals in patients with chronic pancreatic and extra-pancreatic diseases, lipid peroxide activity was measured in the sera of 40 control subjects, 28 patients with pancreatic cancer, 49 with chronic pancreatitis, and 53 with extra-pancreatic diseases. In 142 of the subjects, elastase 1, amylase, and pancreatic isoamylase activities were also determined. Increased lipid peroxide activities were found in some patients with both chronic pancreatic and extra-pancreatic diseases. Patients with chronic pancreatitis studied during relapse had higher activities of lipid peroxides than those without active disease. No difference was found between the values in patients with pancreatic cancer with liver metastases and those without. Correlations were found between lipid peroxides and both amylase and pancreatic isoamylase activities; no correlation was detected between lipid peroxides and elastase 1. In benign biliary tract disease a correlation was detected between lipid peroxides and alanine aminotransferase and alkaline phosphatase activities. In all patients, however, a correlation was found between alkaline phosphatase and lipid peroxide activities. It is concluded that activation of oxygen derived free radicals occurs in chronic pancreatic as well as in extra-pancreatic disease; it seems to reflect the degree of inflammation.

The production, identification, and utilisation of monoclonal antibodies to enzymes are reviewed. Such antibodies may be produced in vitro by the mouse-hybridoma technique, may occur naturally in vivo as enzyme-binding immunoglobulins and may be produced in the laboratory from the lymphocytes of patients whose sera contains such immunoglobulins. The diagnostic application of monoclonal antibodies to enzymes is considered, with special reference to their use in the measurement of the MB-isoenzyme of creatine kinase, pancreatic isoamylase, prostatic acid phosphatase, and the isoenzymes of alkaline phosphatase.

Serum activities of pancreatic and salivary isoamylases were evaluated in 44 patients with liver disease. In 20 patients with acute hepatitis no significant changes of both isoamylases were observed whereas liver cirrhosis and chronic active hepatitis (n = 24) were associated with significantly increased pancreatic isoamylase activities as compared to the control group. The significantly positive correlation between pancreatic isoamylase and the intestinal isoenzyme of alkaline phosphatase in serum may indicate a similarity in the metabolic clearances of both enzymes. We suggest that decreased clearance of pancreatic isoamylase may contribute to hyperamylasemia found in 42% of our patients with chronic liver disease.

Determination of serum pancreatic enzymes remains the gold standard for the diagnosis of acute pancreatitis. Clinical symptoms and signs are of major importance in suspecting the disease, but they are not accurate enough to confirm the diagnosis. Among pancreatic enzymes, total amylase, pancreatic isoamylase and lipase are preferred, since simple, rapid and unexpensive enzymatic methods are commercially available. More expensive and cumbersome methods (e. g. ELISA for pancreatic elastase) are required if a significant delay to hospital admission occurs. In that case, other serum enzymes are usually normal or only lightly increased. To early define the etiology of acute pancreatic serum pancreatic enzymes lack of value. With this purpose, determination of AST, bilirubin and alkaline phosphatase may allow to distinguish between biliary and non-biliary origin of the disease.
