Fifty-eight patients with bone metastasis from gastric cancer, of whom 33 patients were resected cases and 25 were autopsies, were investigated. The incidence of bone metastasis of gastric cancer was 13.4% (33/246) among autopsies and was higher than that (1.5%, 33/2242) among resected cases. We attempted to clarify the factors related to bone metastasis by clinicopathological study. Bone metastasis had a tendency to occur in invasive cancer such as Borrmann types three or four. Histological examination revealed that 86% (50/58) of bone metastasis was poorly differentiated adenocarcinoma and that the stroma was scirrhous type in almost all cases. We measured serum alkaline phosphatase in patients with bone metastasis of recurrent gastric cancer, and found that the levels were relatively related to the condition of bone metastasis. However, the prognosis of patients with bone metastasis was not good and the mean survival time was about 5 months after the appearance of symptoms. In our department, we employ total body hyperthermia in such cases. This method has proved relatively effective for bone metastasis, as reflected by bone scintigram and changes in serum alkaline phosphatase values. As there is no reliable method of treatment for bone metastasis of gastric cancer, the present approach may be worth trying.

The aim of this study was to evaluate the effect of 5-fluorouracil (5-FU) and folinic acid on liver function and bile production in patients with recurrent gastric cancer and jaundice. Thirteen patients were studied for liver function and hyperbilirubinaemia, and six patients were studied for bile production retrospectively, who were treated with 5-FU [700 mg m(-2)] and folinic acid [20 mg m(-2)] for 4 days. Serum total bilirubin, aspartate aminotransferase (AST), gamma-glutamyl transferase (gamma-GT) and alkaline phosphatase (ALP) concentrations all improved with treatment (P<0.0001), and bile production increased significantly (P<0.0001) following treatment. 5-FU and folinic acid can significantly improve jaundice and liver function, and promote bile production. It is possible that 5-FU and folinic acid may become a new method for the relief of jaundice in patients with gastrointestinal tract malignancies.

The patient was a 76-year-old man who was treated with nivolumab due to recurrent gastric cancer. A blood examination revealed grade 3 alkaline phosphatase (ALP) elevation. A histopathological examination revealed marked portal infiltration, including eosinophils and CD4+ and CD8+ T lymphocytes, suggesting nivolumab-related cholangitis accompanied by the features of both an immune-related adverse event (irAE) and drug-induced liver injury (DILI) with allergic reaction. The patient's ALP level immediately decreased after the administration of prednisolone. Although nivolumab-related cholangitis, a rare irAE, has been reported to be refractory to steroid therapy, patients with features of irAE and allergic DILI might immediately respond to prednisolone.
