Utilizing 18 germinomas, the characteristics of isoenzymes of
alkaline phosphatase
(
ALP
) in
germinoma
cells were examined by light and electron microscopic enzymohistochemical and immunohistochemical techniques and biochemical methods including inhibition tests, electrophoresis and electrosyneresis.
ALP
in
germinoma
cells was generally located on the cell surface. In a few
germinoma
cells, however,
ALP
was seen not only on the cell membrane but also in the endoplasmic reticulum. Enzymohistochemically as well as biochemically,
ALP
in
germinoma
cells was little sensitive to L-phenylalanine, moderately sensitive to L-homoarginine and somewhat resistant to heat in its inhibition tests. Immunohistochemical examinations and electrosyneresis showed the presence of the placental type of
ALP
in
germinoma
cells and biochemical analyses revealed that the heat-stable component of
ALP
in
germinoma
cells was consistent with the D-variant of the placental type of
ALP
. Therefore,
germinoma
cells possessed mixed isoenzymes of
ALP
, consisting mainly of the liver/bone type and a small amount of D-variant of the placental type on the cell surface. The prominent expression of
ALP
in
germinoma
cells may be due to the enhanced expression of gonadal genes active in the
germinoma
genome.

Germinal neoplasms originating in the thalamus and basal ganglia were histologically verified by stereotactic biopsies in five cases and by other methods in three cases. Immunoperoxidase staining was performed on the tumors using antibodies against human chorionic gonadotropin and placental alkaline phosphatase. The presence of human chorionic gonadotropin was demonstrated in one
germinoma
and two mixed tumors, but not in three germinomas. Placental
alkaline phosphatase
was demonstrated to be present in four germinomas and one mixed tumor. Stereotactic biopsy specimens can be studied immunohistochemically, and the placental isoenzyme of

appears to be a new tumor marker for

Eighteen cases of intracranial
germinoma
(16 male and 2 female, age 2.5-30 years, average 16.7) were studied. Morphologically, the pathological changes in 18 cases were similar to those of testis seminoma or ovarian dysgerminoma. Among them, two were accompanied with choriocarcinoma and one with embryonal cancer. No cytoplasmic processes was detected by silver stain, but strongly positive to PAS-stain. All of the 18 cases in this series as well as another 8 cases of testis seminoma and ovarian dysgerminoma used as controls gave a positive reaction in
alkaline phosphatase
stain. Among the 18 cases of
germinoma
, except 3 out of 18 were CEA positive, otherwise, all were CEA, beta-HCG, SP1, AFP and GFAP negative. The syncytiotrophoblasts in the choriocarcinoma and embryonal cancer mentioned before were beta-HCG and SP1 positive, and additionally, the embryonal cancer also showed AFP positive. All these results support the hypothesis that both intracranial (extragonadal) and gonadal
germinoma
have a common cell origin.

Extragonadal germ cell tumors rarely arise from the brain stem. In this report, the authors present a rare case of
germinoma
developing in the medulla oblongata. A 31-year-old woman complained of intractable hiccups, hoarseness, and swallowing disturbance. Magnetic resonance imaging showed a tumor located on the dorsal part of the medulla oblongata without any abnormalities in the pineal body and suprasellar regions. Histological diagnosis of surgically resected tumor was
germinoma
. Tumor cells were positive for
alkaline phosphatase
, c-kit, octamer-binding transcription factor 3/4, cytokeratin, and epithelial membrane antigen in immunohistochemistry. The patient received adjuvant chemotherapy and irradiation to the residual tumor and cerebral ventricles. The patient is alive without recurrence 6 months after the surgery.
