A study of the three-dimensional structure of the upper jejunal mucosa in diabetics has been carried out. The structural findings were related to 14C-L-phenylalanine uptake in vitro, sucrase activity in mucosal homogenates, and the enzyme content of the absorptive cells as measured cytophotometrically. A low grade mucosal transformation of the
sprue
-type was found, which was associated with decreased sucrase activity, and with no reduction in phenylalanine accumulation. On the other hand the specific activities of
alkaline phosphatase
, non-specific esterase, and succinic dehydrogenase in the surface cells remained unchanged.

In a retrospective study, jejunal mucosal disaccharidase and
alkaline phosphatase
activities have been investigated in 40 controls and patients with proven celiac sprue (n = 26), lactase deficiency (n = 26), osteoporosis or osteomalacia (n = 16), chronic pancreatitis (n = 12), giardiasis (n = 7), or Crohn's disease (n = 7). Apart from a nonselective reduction of mucosal enzyme activities in the
sprue
syndrome and a selective reduction of lactase activity in the patients with primary lactase deficiency, assays of mucosal disaccharidases revealed only inconstant or slight deviations from the control group and were not of diagnostic significance for any of the above-mentioned disorders. Isolated forms of enzyme deficiencies other than lactase deficiency, such as sucrase-isomaltase or trehalase deficiency were not present among 168 investigations carried out from 1972-1982. It is concluded that assay of small intestinal disaccharidase or
alkaline phosphatase
activities does not expand the diagnostic impact of morphological examination of small bowel biopsy specimens and modern noninvasive methods for the detection of carbohydrate malabsorption. Thus, the method does not appear a necessary or relevant investigation in routine clinical practice.

In 10 southern Indian patients with tropical sprue, in vivo dialysis showed a defect of absorption of water and sodium from the rectum, when compared with 11 healthy volunteers. Sodium-potassium-ATPase activity, measured in homogenates of rectal biopsies, was significantly diminished in patients with

. Magnesium-ATPase and
alkaline phosphatase
were normal in biopsy homogenates. Decreased activity of colonic sodium-potassium-ATPase may contribute to diarrhoea in some patients with tropical sprue.

The virus of transmissible gastroenteritis produced
sprue
-like lesions in the small intestines of young pigs. These lesions were characterized by villous shortening, fusing and blunting in the jejunum and ileum. There was decreased height of the brush border and morphologic alteration of the villous epithelial cells from simple columnar to a variable cuboidal type. Accompanying these microscopic lesions were histochemical changes characterized by decreased staining intensity of acid phosphatase,
alkaline phosphatase
, adenosine triphosphatase, leucine aminopeptidase, succinic dehydrogenase and malic dehydrogenase in the affected intestinal mucosa. The clinical nature of transmissible gastroenteritis in the pig together with the histopathologic and histochemical changes may provide a useful experimental model for obtaining additional basic information on enteric disturbances.
