In a series of 51 patients with prostate cancer and obstructive
uropathy
, unilateral or bilateral obstruction was identified in 22 (43%) and 29 (57%) respectively. This included a non-functioning kidney in 12 patients. In 86% of patients the T category was advanced. Bone metastases were present in 36 cases (71%); 19 patients (37%) had chronic retention. All patients with metastatic disease underwent hormonal manipulation and 43 underwent transurethral resection of the prostate. External beam radiotherapy, percutaneous nephrostomy and ureteric reimplantation were performed in 4, 5 and 1 patient respectively. Actuarial survival of all 51 patients was 57 and 25% at 2 and 5 years. Presentation with bilateral or non-function did not predict a worse prognosis in comparison with patients with unilateral hydroureteronephrosis. Raised
alkaline phosphatase
and prostatic acid phosphatase were of no prognostic value, while creatinine reached marginal significance. A positive bone scan and raised urea were strongly predictive of a poor outlook. It was concluded that prostate cancer and obstructive
uropathy
should not uniformly imply a terminal event, and interventional therapy is justified with a 25% 5-year survival rate.

The authors compare the two prostate specific antigen assay methods most widely used in France. The first method (RIA Baxter) uses an isotope marker (Iodine 125), the other (EIA Biotrol) uses an enzymatic marker (
alkaline phosphatase
). Prostate specific antigen was assayed by means of these two techniques in two groups of patients: one group of 49 men considered to be free of any prostatic disease, recruited from blood donors; another group of 89 male patients in whom a prostate specific antigen assay was performed prospectively at the first urology outpatients visit. The two prostate specific antigen assay techniques gave different results, but the values obtained by these two methods were not discordant. It is therefore possible to define a coefficient of proportionality of 1.45 regardless of the prostate specific antigen concentration or the
urological disease

The case records of 327 patients who underwent bone biopsy in late or terminal renal failure, before any form of dialysis or transplantation, were examined for clues to the aetiology of renal osteomalacia and its manifestations. Fifty four per cent of the biopsies showed pure osteitis fibrosa, 34 per cent osteomalacia with osteitis fibrosa and 12 per cent showed neither abnormality. Osteomalacia was strongly associated with chronic pyelonephritis and obstructive
uropathy
as primary renal disease. In two matched groups of 100 each, and within the major primary diseases, it was associated with acidosis, hypocalcaemia and normophosphataemia (as opposed to hyperphosphataemia). There was no association with known length or uraemia and only a weak and inconsistent relationship with severity of uraemia. In the few patients studied, there was no relationship between osteomalacia and serum 25-hydroxycholecalciferol level. In contrast to the state of patients treated by haemodialysis, osteomalacia in this undialysed group was manifested by a higher level of serum
alkaline phosphatase
than pure osteitis fibrosa, serum iPTH did not differ between the groups, there was no predominance of symptoms in one group, other than proximal myopathy which had a weak association with osteomalacia, and Looser zones were more common than complete fractures. Our study shows that osteomalacia has different manifestations, and probably different causes, before and after the start of haemodialysis. These two stages of renal failure should be clearly distinguished in reports of renal bone disease.

In a hospital-based analytical case-control study of prostate cancer conducted during 1996-1997 at the Urological clinics of FNKV Prague 10,181 cases of prostate cancer and 277 controls of males with non cancerous
urological disease
were analysed. Mean age of newly diagnosed cases of prostate cancer was 70.8 years, mean age of death (83) from this cancer was 72.8 years. At the time of diagnosis 44.2% cases had already remote metastases in bones. Five-year cumulative survival rate of the cases was in this study 53.4%. Statistically significant differences were observed in examination of prostatic specific antigene (PSA) and
alkaline phosphatase
. Results of other biochemical examinations, digital rectal examination or sonography of the prostate did not show statistically significant differences between cases and controls. Implementation of both digital rectal examination and PSA examination into the scheme of preventive medical examination of males over 50 years of age represents the only possibility how to increase the proportion of the prostatic cancer diagnosed in early latent stage and thus therapeutically and prognostically more favourable.

Ketamine-associated diseases have been increasing with the rise in ketamine abuse. Ketamine-associated
uropathy
is one of the most common complications. We investigated the effects of ketamine-associated
uropathy
on renal health and determined predictors of renal function decline in chronic ketamine abusers. This retrospective cohort study analyzed 51 patients (22 with ketamine-associated hydronephrosis and 29 with ketamine cystitis) from Kaohsiung Veterans General Hospital in Taiwan. Primary renal outcome was end-stage renal disease or estimated glomerular filtration rate decline >30% from baseline. Compared with the ketamine cystitis group, the hydronephrosis group had lower initial and final estimated glomerular filtration rates and higher
alkaline phosphatase
and gamma-glutamyl transferase levels (
p
< 0.05). Elevated cholestatic liver enzyme levels correlated with renal dysfunction in ketamine-associated
uropathy
. The hydronephrosis group had a higher proportion of patients reaching endpoints than the ketamine cystitis group (50% and 7%, respectively,

< 0.001). After adjusting for age, sex, and initial serum creatinine level, hydronephrosis remained an independent risk factor for renal function deterioration.
