Gingival crevicular fluid (GCF) samples were collected from 204 teeth from 35 subjects, including 8 rapidly progressive periodontitis (RPP), 14 chronic adult periodontitis (CAP), 7 marginal
gingivitis
(MG) and 6 healthy subjects (H). The results of
alkaline phosphatase
(
ALP
) examination indicated that the
ALP
levels from CAP and RPP patients were significantly higher than those from H subjects and MG patients. The
ALP
levels were closely related to the amount of GCF flow and various clinical periodontal parameters, especially to probing depth and attachment loss. The
ALP
levels and the various clinical parameters decreased significantly after periodontal therapy. The present study suggests that the measurement of GCF--
ALP
could be an useful adjuctive criterion for detection of periodontal inflammatory status and extent of tissue destruction.

The purpose of this study was to investigate how levels of gingival crevicular fluid (GCF)
alkaline phosphatase
(
ALP
) change in relation to levels of plaque and gingival inflammation in 20 adults during a 21 day period of experimental
gingivitis
. The source of
ALP
within GCF was also investigated using a repeat sampling protocol; by determining enzyme levels derived from 30 putative periodontal pathogenic and non-pathogenic species; and by examining inhibition profiles from a variety of host and bacterial
ALP
isoenzymes. Total 30-s GCF
ALP
levels increased significantly (p < 0.002) during experimental
gingivitis
and preceded an increase in gingival index (GI) by approximately 7 days. Enzyme levels correlated with GCF volume (R = 0.7; p < 0.0001), but repeat sampling indicated that entry of
ALP
into the gingival crevice was independent of the rate of fluid flow. Only 5 of the bacterial species investigated produced clearly detectable levels of
ALP
in culture supernatants, these were P. gingivalis (381), P. intermedia (581), P. nigrescens (8944), Dentin P. gingivalis (TW 471: clinical isolate) and C. ochracea (25). Levamisole inhibition and studies on suspensions of washed plaque demonstrated that host-derived
ALP
contributed to > 80% of the enzyme in GCF. We conclude that elevated 30-s GCF
ALP
levels measured using the chemiluminescent assay reported, are detectable before increases in gingival indices and appear to be a better marker of gingival inflammation than
ALP
concentrations. The major source of
ALP
within GCF is host derived and in early inflammatory disease is likely to be of polymophonuclear leukocyte origin.

We report the first diagnosed case of Papillon-Lefevre syndrome in Thailand. The patient is the youngest child of consanguinous parents, and she has had symmetrical hyperkeratotic plaques on both plantar surfaces since birth with a history of chronic
gingivitis
, periodontitis, and premature loss of primary dentition. The histologic study revealed compact hyperkeratosis with epidermal acanthosis. Radiologic studies of the skull were normal. The radiographic panoramic view of the oral cavity revealed generalized severe vertical and horizontal alveolar bone loss. The immunologic analysis of polymorphonuclear leukocyte phagocytic function by nitrobluetetrazolium test (NBT test) showed decreasing response to latex stimulation. Serum parathyroid hormone, calcium, phosphate, and
alkaline phosphatase
levels were within normal limits. The skin lesions were temporary relieved with topical keratolytic agents. The oral lesions were improved by the extraction of hopeless teeth and conventional periodontal treatments.

Aim of study was to examine periodontal status among 20 44 year old patients and to study the secretory function of peripheral venous blood neutrophilic leukocytes (NL) exposed to various antigens and
alkaline phosphatase
(AP) activity in gingival crevicular fluid (GCF) in patients suffering from
gingivitis
and periodontitis. Clinically were determined Russell's periodontal index (PI). Secretory function of NL affected by opsonized zymosane, non-opsonized E. coli was examined in 77 patients with
gingivitis
and periodontitis, and in 35 donors, free of internal diseases, by means of beta-glucuronidase (beta-GD), lysozyme (LZ). NL secreted higher levels of beta-GD in incubation medium in patients with periodontitis (p < or = 0.001) subject to degree of periodontal lesion. NL affected by various antigens secreted higher levels of LZ into non-cellular matrix in patients with
gingivitis
and periodontitis comparing to control environment in analogous groups. Data obtained from this study suggest that in patients with periodontitis response of NL to bacterial stimuli is specific and subject to the degree of periodontal lesion. Our study showed a significant difference of AP activity in GCF subject to pocket depth and degree of periodontal lesion. Once NL are exposed to corpuscules prone to phagocytosis, an increase in secretion of beta-GD and LZ can be explained by overall increase in secretion of NL lysosomic enzymes, thus disclosing the mechanism of inflammatory periodontal tissue damage.
