Mebendazol (Vermox) was given for six months at an oral daily dose of 2 g to three patients with inoperable infestation of the liver by Echinococcus alveolaris. In two patients the general state improved greatly, with reduction in the size of the liver infiltrate, regression in serum bilirubin and alkaline phosphatase levels. Treatment continues in the third case.

Fifty surgically confirmed cases of hydatid disease in Eastern Libya were investigated. Of these, 30 had cysts of the liver, five had extra-hepatic abdominal cysts and 15 had cysts in the chest, including the lungs. Ten cases of malignancy of the liver and five cases of malignancy of the lung were used as controls. Differential diagnosis of cancer was made by measuring the ESR and glycoprotein levels (elevated x 4 and x 3 respectively, in cancer cases). Serum bilirubin, SGOT, SGPT and serum alkaline phosphatase levels were also significantly raised in malignancy, while the total protein levels and eosinophilia were the same in both diseases. The intradermal test was positive in 88%, the haemagglutination test in 86% and the complement fixation test in 70% of cases. In the post-operative follow-up, complement-fixing antibodies disappeared somewhat faster than did the haemagglutinins. The positive skin test cases, however, remained positive throughout the 12-month follow-up period.

Mice infected with protoscoleces of Echinococcus granulosus for 12-14 months were treated ig with mebendazole (Meb) 25-50 mg.kg-1 x d-1 for 7-14 d, albendazole (Alb) 200 mg.kg-1 x d-1, cr praziquantel (Pra) 500 mg.kg-1 x d-1 for 14 d. The mice were killed 24 h after the last medication, and acid phosphatase (ACP), alkaline phosphatase (AKP), and adenosine triphosphatase (ATPase) including (Na, K, Mg)-ATPase, (Na, K)-ATPase, and (Mg)-ATPase were determined and compared with those of untreated control group. The results showed that ACP activities of cyst wall in treated groups were lower than the control group. Whereas AKP activity of cyst wall in Pra group increased markedly, this is not the case in Meb and Alb groups. Three ATPase activities of cyst wall were inhibited in both Meb and Alb groups, Meb being more potent. No apparent changes in the ATPase activities were seen in Pra group.

1. Kinetic and physical parameters of purified alkaline phosphatase from Echinococcus multilocularis metacestodes, livers of infected gerbils and control animals were determined. 2. Km value for p-nitrophenyl phosphate was about 0.05 +/- 0.02 mM for the three enzymes. 3. Vmax values were 357 +/- 67 nmol/min/mg proteins for metacestode enzyme, and 6.7 +/- 1.1 and 6.7 +/- 0.8 nmol/min/mg proteins for liver enzyme of infected and control animals, respectively. 4. Mr and pI were different for the parasite and hepatic enzyme. 5. The parasite enzyme was less sensitive to the elevation of temperature than hepatic enzyme. 6. The isatin inhibition was a competitive inhibition type for parasite and uncompetitive type for host liver enzyme.

A sandwich-ELISA was developed for the detection of soluble Taenia hydatigena antigens in fecal samples of dogs. Affinity-purified polyclonal catching antibodies and alkaline phosphatase-conjugated detecting antibodies were employed, which had been obtained from rabbits hyperimmunized with excretory/secretory antigens derived from in vitro maintained adult Taenia hydatigena. The assay allowed the detection of 800 ng T. hydatigena antigen g-1 of feces as a lower limit. Six helminth-free dogs were each infected with 10 T. hydatigena cysticerci isolated from Swiss sheep. After prepatent periods ranging from 57 to 71 days, the dogs started to excrete Taenia eggs and/or proglottids. The ELISA detected Taenia antigens in all six dogs during the prepatent period starting individually between Day 18 and 45 post-infection (p.i.). Anthelmintic treatment of three dogs at Day 95 p.i. resulted in elimination of the cestodes and within the 5 following days in the disappearance of Taenia antigens from feces. The specificity of the assay was evaluated by testing crude antigens derived from helminths or bacteria. Four Taenia species showed cross-reactivity at concentrations of 5 micrograms protein ml-1. Conversely, no cross-reactions occurred with various antigen batches derived from Echinococcus granulosus, E. multilocularis, Dipylidium caninum, Mesocestoides corti, Diphyllobothrium sp., Toxocara canis and bacterial antigens (Salmonella and Escherichia). Moreover, fecal samples from dogs naturally infected with T. canis (n: 13), hookworms (n: 2), Trichuris vulpis (n: 13) and of 10 dogs with mixed infections with these three nematode groups were tested, and results confirmed the high degree of specificity. The Taenia antigens detectable by this ELISA remained immunologically stable in native feces stored at +25 degrees, +4 degrees or at -20 degrees C for at least 5 days.

A study on the biochemical and ultrastructural effects of isatin on Echinococcus multilocularis metacestodes in Meriones unguiculatus is reported. In the metacestode, after an 18 day treatment period, the alkaline phosphatase and lactate dehydrogenase activities decreased and the acid phosphatase activity increased. Glucose and glycogen stores declined significantly. Examinations of the ultrastructure revealed an increase of lysosomes and phagocytic reactions. The protoscolesces were damaged and an abnormal striated process appeared. The tegumental cytoplasmic syncytium remained unaltered. These observations suggest that isatin derivatives may have a role in the chemotherapy of infections caused by Echinococcus spp.

We present a case of polycystic hydatid disease produced by Echinococcus vogeli in a tapper of rubber trees from the State of Acre, Brazil. The most relevant clinical data were pain, increased abdominal volume caused by palpable painful masses, fever and poor clinical condition. Laboratory tests showed anemia, eosinophilia, hypoalbuminemia, hypergammaglobulinemia and increased plasma levels of alkaline phosphatase. Computerized tomography revealed diffuse cysts throughout the peritoneal cavity up to the pelvis, and inside the liver, pancreas and spleen. Anatomopathological examinations of cysts obtained by laparotomy confirmed the etiological diagnosis. Treatment with 10 mg/kg Albendazole for 6 months caused complete regression of the disease.

Several approaches were adopted for the disruption and removal of the tegumental surface from protoscoleces of the horse strain of the hydatid organism, Echinococcus granulosus. The effectiveness of each method and the purity of subsequent microthrix-enriched fractions obtained by differential centrifugation were evaluated by electron microscopy, by the amount of protein released and by the degree of enrichment of surface plasma membrane marker enzymes. Incubation in saponin for 10 min produced the purest microtriche preparation, but in low yield; freeze/thawing, incubation in Triton X-100 for 10 min or in saponin for 20 min produced fractions containing significant amounts of relatively pure microtriches, but mild homogenization was a poor method for surface disruption and subsequent isolation of microtriches. Phosphodiesterase, adenosine triphosphatase (total and ouabain-inhibited), leucine aminopeptidase and glutamyltransferase were active in the protoscoleces but none were enriched in any of the microthrix fractions. In contrast, alkaline phosphatase, acid phosphatase, 5' nucleotidase and maltase were enriched significantly in all of the isolated microtriche preparations, which suggests that these enzymes are predominantly surface membrane bound. The protein profiles of the microthrix-enriched fractions, following SDS-PAGE, were basically similar, although there were some qualitative and quantitative differences in the proteins released by each isolation procedure. Three major PAS-staining components were present in all the preparations and these probably originated from the glycocalyx. One of these PAS-positive components, with an approximate molecular weight of 110 kDa, may be a glycoprotein specific to the horse strain of E. granulosus.

An immunoassay (double-antibody-sandwich-ELISA) was developed to detect circulating antigens (CAg) in patients with cystic (Echinococcus granulosus) echinococcosis. Echinococcus antigens derived from heterologous intermediate hosts were used to immunize rabbits and to purify the rabbit-IgG-fraction obtained by affinity-chromatography, thus avoiding major interference with host components. The purified rabbit anti-hydatid IgG was immunosorbed with bovine and human sera. One part of the resulting IgG served as coating agent in a double antibody sandwich-ELISA; the other part, coupled to alkaline phosphatase, as detecting conjugate. The specificity of the antibody reaction was demonstrated by immunoelectrophoresis. Sera of 21 patients with cystic echinococcosis were examined with this test system. In seven of the patients' sera CAg were detected in concentrations ranging between 310 ng and 680 ng protein per ml serum. Comparing pre- and postoperative serum samples obtained from nine patients operated on for cystic echinococcosis, four sera were found to be CAg-positive before and three after operation.

Specific antibodies against Giardia lamblia in 92 sera from asymptomatic carriers were investigated by ELISA. Using cyst antigen, antibodies were detected in 91 of the sera, by means of alkaline phosphatase-labelled antispecies immunoglobulin. The E/405nm values of the positive sera ranged between 0.350 and 1.700, whereas in the 78 control sera they were 0.270 +/- 0.220. Cross-reactivity between the anti-G. lamblia antibodies and 18 heterologous parasitic antigens and fractions was also studied. Cross-reactions were observed with antigens of Toxoplasma gondii, Sarcocystis spp., Leishmania donovani, Fasciola hepatica Y, F. hepatica I, Echinococcus granulosus AM, Cysticercus tenuicollis, Taenia saginata, Dipylidium caninum Y, D. caninum I, Trichuris suis Y, T. suis I, Trichinella spiralis and Litomosoides carinii. No cross-reactions were observed with the antigens and fractions of Entamoeba histolytica, Dicrocoelium dendriticum, Schistosoma mansoni and Echinococcus granulosus Y. The results show that the ELISA could be easily and very successfully applied for routine diagnosis of giardiasis.
