Galactosyltransferase (EC 2.4.1.22) requires bivalent metal ions for its activity. However, preparations of this enzyme solubilized from Golgi membranes of lactating rat mammary gland were shown to be activated not only by Mn2+, Ca2+ and Mg2+, but also by spermine, spermidine, lysyl-lysine, ethylenediamine and other diaminoalkanes, and by a range of basic proteins and peptides, including clupeine, histone, polylysine, ribonuclease, pancreatic trypsin inhibitor, cytochrome c, melittin, avidin and myelin basic protein. Both N-acetyl-lactosamine synthetase and lactose synthetase activities were enhanced. A basic protein fraction was isolated from bovine milk and shown to activate galactosyltransferase at low concentrations. The polyanions ATP, casein, chondroitin sulphate and heparin reversed the activation of galactosyltransferase by several of the above substances. Galactosyltransferase, assayed as a lactose synthetase, showed a 10-fold greater affinity for glucose when Mn2+ ions were replaced by clupeine or by ribonuclease as cationic activator. Evidence was obtained for the presence of an endogenous cationic activator in solubilized Golgi membrane preparations which evoked a similar low apparent Km,glucose. The findings are discussed in the light of cationic activations of glycosyltransferases generally, of the porous nature of the Golgi membrane, and of the unlikelihood of bivalent metal ions being the physiological activators of galactosyltransferase. It is suggested that the natural cationic activator of lactose synthetase may be a secretory protein acting in a manner analogous to the enzyme's activation by alpha-lactalbumin. A scheme is proposed for the two-stage synthesis of lactose and phosphorylation of casein within the cell, to accommodate the apparent incompatibility of these two processes.

We assessed the value of several serologic markers in detecting pancreatic carcinoma in a prospective study of 270 patients. The sensitivity and specificity of galactosyltransferase isoenzyme II (GT-II), carcinoembryonic antigen (CEA), alpha-fetoprotein, ferritin, C1q binding, and ribonuclease were determined. GT-II was the most sensitive (67.2 per cent) and specific (98.2 per cent) for discriminating between benign and malignant disease and was more sensitive and specific than CEA, the next most useful marker. Sensitivity was 64 per cent for ultrasound, 79.4 per cent for computerized body tomography (CBT), and 92.8 per cent for endoscopic retrograde cholangiopancreatography (ERCP). As a single test, only ERCP was more sensitive than GT-II, but more sensitive diagnoses resulted when GT-II was combined with ultrasound (92 per cent), CBT (88 per cent), or ERCP (100 per cent). Serum GT-II may be useful both by itself and in combination with imaging techniques in distinguishing benign from malignant pancreatic disease; however, this test does not discriminate between pancreatic carcinoma and other gastrointestinal neoplasms.

Oncofetal markers for colon carcinomas are CSAp, a nonsulfated mucin, a second trimester fetal antigen, an altered thymidine kinase, a monosialoganglioside, and glycolipid antigens. For gastric carcinoma, they are basic fetoprotein, a sulfoglycoprotein, and for pancreatic carcinomas--POA, an oncofetal pancreatic antigen, and designated as CAPI, an oncofetal antigen. Tumor-associated markers for colon carcinomas are: UDP-galactosyltransferase and zinc glycinate marker; for gastric carcinomas, sulfated glycoprotein and for pancreatic carcinomas, pancreas carcinoma-associated antigen, a polycytidylic acid-specific ribonuclease, and galactosyltransferase. Suggested as tumor-specific markers for colon carcinomas are an altered mucoprotein, basic antigen, beta 2-microglobulin-associated antigen, and a specific adenosine deaminase; for gastric carcinomas, a specific protein, an antigen with 3-oxyanthranilic acid, and an antigen of unknown origin in gastric secretions; for pancreatic carcinomas, an antigen with molecular weight of 380,000 daltons and an antigen suggested by tumor immunity.

Laboratory tests are the object of continuous interest in acute as well as chronic pancreatic disease. Enzymic assays play an important role, particularly in screening for pancreatic disease. The diagnostic contribution of amylase, isoamylases, immunoreactive trypsin and lactoferrin, ribonuclease and galactosyltransferase, as well as the problem of chronic nonpancreatic hyperamylasemia is reviewed. Functional methods detect a normal or abnormal function and in this sense the results should be interpreted. Present evaluation of the pancreozymin-secretin test, the Lundh test, fecal chymotrypsin, determination of stimulated chymotrypsin secretion by peroral synthetic substrates marked with 4-aminobenzoic acid, duodenal excretion of 75Se-methionine and plasma pancreatic polypeptide is given. Up to now, immunologic methods have not fulfilled the expectations in spite of considerable attention paid to them in recent years.

The present study gives an evaluation of carcinoembryonic antigen (CEA), macrophage electrophoretic mobility test (MEM), sialyltransferase, galactosyltransferase isoenzyme (SGT), ribonuclease and reverse transcriptase as diagnostic aids in malignant diseases. CEA and sialyltransferase are of certain value in the monitoring of cancer, as their values in the serum may rise before progression of disease or relapse. Both tests are not reliable parameters in the early diagnosis of malignancy. Our results with regard to the MEM test have not proved in any way useful in the diagnosis of cancer. Our preliminary results appear to indicate that, provided further simplification of the method can be achieved, SGT isoenzyme determination seems to be a better means of diagnosing cancer. In view of inherent-methodological difficulties reverse transcriptase has, at present, no clinical application in the diagnosis of cancer.
