We evaluated the proapolipoprotein A-I (proapo A-I)-converting activity to clarify the pathogenesis of the high proapo A-I/apo A-I ratio in the high-density lipoprotein (HDL) from patients with acute hepatitis and liver cirrhosis. The serum proapo A-I-converting activities were measured using 3H-labeled proapo A-I. 3H-labeled proapo A-I was purified from the media of cultured Hep G2 cells by immunoaffinity chromatography. The serum proapo A-I-converting activities were found to be significantly reduced in the patient with liver cirrhosis (140 +/- 53 dpm/ml per h) or acute hepatitis (140 +/- 48 dpm/ml per h) compared to normal subjects (315 +/- 32 dpm/ml per h). Serum proapo A-I-converting activity has a positive correlation with liver function tests such as serum albumin, choline esterase activity, ICG clearance and inverse correlation with proapo A-I/apo A-I ratio in HDL. These results suggest that the high proapo A-I/apo A-I ratio is due to the decreased proapo A-I-converting activity, and that the liver plays a significant role in the conversion of proapo A-I to apo A-I.

Arterial ketone body ratio (AKBR) were examined in 114 cases of hepato-biliary tract diseases. AKBR of the normal control was 1.47 +/- 0.38, while it remained less than 0.7 in liver cirrhosis, hepatocellular carcinoma (HCC), alcoholic liver diseases and malignant biliary tract obstruction. AKBR correlated well with serum albumin and cholinesterase. Thirty five cases of HCC were treated with transcatheter arterial embolization (TAE), 20 cases with gelatin sponge and 15 cases without gelatin sponge. In cases with gelatin sponge AKBR decreased significantly immediately after TAE and recovered gradually during 24 hours. Without gelatin sponge AKBR decreased slightly and remained unchanged until 24 hours later. Concerning the prognosis after TAE, AKBR recovered well in cases with good prognosis, while in poor prognosis AKBR progressively decreased to below 0.3. In experimental TAE with gelatin sponge using rabbit VX2-induced liver tumor, AKBR decreased significantly. In fatal rabbit group after TAE, AKBR decreased progressively. Plasma endotoxin was also measured in TAE with experimental rabbit, AKBR and endotoxin showed reverse correlation. From these results it was suggested that the measurement of AKBR is very useful for the evaluation of efficacy and prognosis of TAE in primary liver cancer.

Changes in the blood serum protein and mineral composition, trypsin inhibitor content, alanine amino-transferase, aspartate aminotransferase, alpha-glutamyl transpeptidase, creatine kinase, choline esterase activities, blood plasma trace element levels were examined in 112 patients with pyoinflammatory involvement of the soft tissues of the face and neck. The study was revealed reduced blood serum albumin concentration, elevated trypsin inhibitor levels and alpha-glutamyl transpeptidase and creatine kinase activities, decreased content of Mg and Zn and, in some patients, of choline esterase activity. Biochemical parameters gradually normalize, as the patients recover, their normalization depending on the therapeutic methods and detoxication treatment.

In order to identify non-invasive, biochemical indicators of di(2-ethylhexyl)phthalate (DEHP) exposure, we have compared the effects in blood serum with biochemical effects in liver in rats fed a diet containing 0, 0.25, 0.75 and 2% DEHP for 2 weeks. After 3 days of treatment serum arylesterase activity levels and serum triglycerides were decreased to 60% and 20% of control values, respectively. After a 2-week treatment with DEHP the effects were generally stronger. Compared to a control group, serum arylesterase activity levels, serum triglycerides and serum cholesterol were decreased to 40%, 20% and 50%, respectively. Serum cholinesterase activity levels and serum albumin concentrations were increased by the DEHP treatment to 290% and 135% of control values, respectively. In the livers a hepatomegaly, an induction of cytochrome P-450 IVA1 and induction of the activity of palmitoyl-CoA oxidase and carnitine acetyl-CoA transferase was found to be 180%, 1080%, 1300% and 1700% of control values, respectively. The liver is a more sensitive target for DEHP exposure compared to the biochemical effects in serum, but determination of the serum parameters can be used to determine early biological effects of exposure to DEHP.

In rats poisoned with soman, an irreversible organophosphate anticholinesterase, acute changes in blood-brain barrier (BBB) permeability to proteins were investigated, using Evans Blue (EB)-labelled serum albumin and plasmatic gamma-immunoglobulin G (IgG) as indicators. Confirming previously published data, soman produced a conspicuous seizure-related and reversible BBB opening which was greatest after 30 to 60 min of paroxysmal electroencephalographic (EEG) discharges when signs of cerebral hyperactivity (epileptic EEG pattern, hyperoxia) were also at their height. Topographically, the protein leakage was bilateral and restricted to anatomically defined brain structures, some of which being thereafter sites of parenchymal edema and neuronal damage. In these areas (e.g., the thalamus), the edema is probably, at least in part, "vasogenic" in origin, and the possible contribution of the transient BBB opening to the neuronal lesions was questioned. On the other hand, the hippocampus, a region preferentially affected by the soman-induced acute neuropathology, was always free of any protein leakage, suggesting that the edema is unrelated to vascular damage and "cytotoxic" in nature. Finally, no topographic relationship was shown to exist between the increase in cerebrovascular permeability produced by soman and the histochemically-detected inhibition of the parenchymal total cholinesterases (ChE) or endothelial butyrylcholinesterase (BuChE).

During an ultra-long-distance race (1000 km in 20 days) the influence of running was examined on the enzymes aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (AP), gamma-glutamyl-transferase (GGT), and glutamate dehydrogenase (GLDH) with regard to their release from the liver cells or their induction. Furthermore the liver synthetic capacity was assayed by measuring the enzyme activity of cholinesterase and the concentration of serum albumin during the race. Of the 110 participants, 55 finished the race and only the results of these runners were used in our study. AP increased continuously from day 0 (mean = 102 U/L) to day 19 (mean = 120 U/L). A fivefold increase of AST and a twentyfold increase of CK up to day 3 was followed by a significant decrease towards the end of the race. ALT rose as well up to day 6 from a mean value of 8 U/L to 24 U/L but remained at this level. Surprising was the individual increase of the enzymes GLDH (up to twentyfold) and GGT (up to sixfold) in more than half of the finishers on various days indicating liver cell injuries. The activity of CHE and the concentration of serum albumin decreased during the race, both were significantly correlated.

Hepatic binding protein (HBP) is a hepatic cell surface receptor specific for asialoglycoprotein. In vivo estimates of HBP concentration ([HBP]) were compared to classical indicators for hepatic functional reserve to clarify the validity of [HBP] in estimating the hepatic functional reserve in 30 humans. Estimates of [HBP] were obtained based on kinetic analysis of liver and blood time-activity data resulting from the hepatic clearance of a single injection of technetium-99m galactosyl-neoglycoalbumin, which is a synthetic analog radioligand specific to HBP. Estimates of [HBP] ranged 0.054 to 0.720 microM. Estimates of [HBP] in normal volunteers were 0.668 +/- 0.050 microM, whereas that in liver cirrhosis were 0.188 +/- 0.112 microM. The difference between the mean values of [HBP] estimates was statistically significant (p = 0.0001). Good correlations were observed between [HBP] and prothrombin time (r = 0.625, p = 0.0002), serum albumin level (r = 0.687, p = 0.0001), serum cholinesterase level (r = 0.764, p = 0.0001), indocyanine green plasma disappearance rate (r = 0.602, p = 0.0024), and Child-Turcotte classification score (Pugh's modification) (r = -0.797, p = 0.0001). We concluded that excellent correlations of [HBP] with classical indicators for hepatic functional reserve suggest potential value of [HBP] as a sensitive measure of functioning hepatocyte mass.

The nutritional status of 41 male patients with carcinoma of the stomach was recorded, right after admission to hospital, and was related to tumour-specific operability. The nutritional condition of most of the patients was moderately impaired. Significant differences between operable and non-operable patients were recorded from the following parameters: upper-arm circumference, haemoglobin concentration, serum albumin, pseudocholinesterase, intradermal testing, E-rosette count. The findings thus obtained provided some limited information on operability.

The paper critically analyzes available data on the nutritional and metabolic effects of total parenteral nutrition (TPN) and enteral nutrition (EN) in cachectic cancer patients. Only papers dealing with adult cancer patients and providing data regarding type of tumor, duration of the nutritional support, and administration rate of calories and amino acids, validated by statistical analysis of the results, are included. The main conclusions are the following: (1) No nutritional variable worsened in cancer patients receiving TPN or EN, in conditions in which progressive deterioration of the nutritional status is the rule. (2) The nutritional variables improved by TPN and EN were body weight, fat mass, and some indicators of lean body mass (nitrogen balance and whole body potassium). Thyroxin-binding prealbumin and retinol-binding protein increased only with TPN, whereas some immunologic indexes (complement factors and lymphocytes) improved only with EN. (3) The daily regimens which improved lean body mass and visceral proteins ranged from 35 to 55 kcal/kg and from 1.2 to 2.0 g of amino acids/kg for TPN; for EN it was 35 kcal/kg and 1.3 g of amino acids/kg. However, the enteral regimen capable of improving some immune responses included at least 42 kcal/kg and 2.3 g of amino acids/kg. (4) Only three randomized studies were performed to compare TPN and EN, and conflicting results were obtained. Only TPN showed some significant advantages with regard to weight gain, nitrogen balance, maintenance of serum albumin levels and some mineral balances. However, the advantage of TPN was not clear enough to recommend its indiscriminate use. The choice between TPN and EN should always consider the functionality of the GI tract, the need for hospitalization to start a TPN regimen, and the higher cost of intravenous feeding. (5) When comparing TPN to a standard oral diet, the following variables improved with the nutritional support: body weight, nitrogen balance, 3-methylhistidine, urinary excretion, and serum levels of transferrin, cholinesterase, thyroxin-binding prealbumin, and retinol-binding protein. (6) When comparing TPN with glucose vs TPN with glucose-lipids, no major difference was found with regard to most nutritional variables. In conclusion, nutritional support alone probably has a small role in managing a limited number of advanced cancer patients dying primarily because of malnutrition or mainly suffering from nutritional deterioration. It can also have a "permissive" role in those patients potentially candidate to an oncologic treatment which cannot be delivered because of a poor nutritional status.(ABSTRACT TRUNCATED AT 400 WORDS)

Octadecyl-bonded silica, commonly used for reverse-phase high-pressure liquid chromatography, was modified using surfactants bearing ionizable groups and the modified packing used in ion-exchange chromatography of proteins. The surfactants 2-(n-hexadecylheptaethoxy)acetic acid, 1-(n-hexadecyloctaethoxy)ethylene-diamine, and N-(n-hexadecyloctaethoxy)pyridinium were adsorbed onto test columns packed with octadecyl-bonded silica particles. The proteins lysozyme, bovine serum albumin, trypsin, horse serum cholinesterase, and bovine liver carboxylesterase were used to study the ion-exchange characteristics of the modified packings. The retention order of the proteins on the surfactant-modified stationary phases were as predicted by the isoelectric point of each protein. In addition, the interaction of enzymes with the packings did not result in significant loss of enzymatic activity. Surfactant removal was possible with the use of organic solvents and this allowed the octadecyl-bonded surface to be used again in the reverse-phase mode. During the course of the experiments, no degradation in the packing's performance was observed due to loss of adsorbed surfactant, even after over 85,000 column volumes of sodium chloride and Tris-HCl buffers were circulated through the column.
