Mesenchymal stem cells (MSCs) have the capacity for self-renewal and can form bone, fat, and cartilage. Alginate forms a viscous solution when dissolved in 0.9% saline and gels on contact with divalent cations. The viability and phenotype maintenance of chondrocytes in alginate beads have been well documented. However, little is known about the effect of microencapsulation in alginate on chondrogenesis of MSCs. In this study, human MSCs encapsulated in alginate beads were cultured in serum-free medium with the addition of transforming growth factor (TGF)beta1 (10 ng/mL), dexamethasone (10(-7) M), and
ascorbate
2-phosphate (50 microg/mL). The MSCs in alginate assumed a rounded morphology with lacunae around them after 1 week in culture. Cell aggregates were observed at 2 weeks or longer in culture. Histological findings agreed with the clinical determination of hyaline cartilage, characterized by isolated cells with ground substance positive in Safranin-O staining and immunohistochemistry for collagen type II at the periphery of cells. transcriptase
-polymerase chain reaction (RT-PCR) revealed the expression of COL2A1 and COL10A1, marker of chondrocytes and hypertrophy chondrocytes, respectively. These results indicate MSCs in alginate can form cartilage and the MSCs-alginate system represents a relevant model for the study of the molecular mechanisms involved in the chondrogenesis and endochondral ossification.

The purpose of this investigation was to examine by reverse-
transcriptase
polymerase chain reaction analysis the osteogenic differentiation of twice-passaged Sprague-Dawley rat bone marrow stromal cells in type I collagen gel cultured for 3 weeks. Two culture media were used here, namely Dulbecco's modified Eagle (DME) medium supplemented with
vitamin C
[Dex (-)] and those with
vitamin C
, dexamethasone and beta-glycerophosphate [Dex (+)]. Culture with Dex (-) medium in collagen gel for 3 weeks brought about the well-developed cell network and middle-stage osteogenic phenotype expression characterized by mRNA for alkaline phosphatase, osteonectin and osteopontin while those for bone sialo protein and osteocalcin were not detected. On the contrary, culture with Dex (+) medium in collagen gel for 3 weeks lead to necrosis of the cells. These results indicate that culture in collagen gel with Dex (-) DME medium containing
vitamin C
was useful for three-dimensional culture and middle-stage osteogenic differentiation of twice-passaged bone marrow stromal cells. This study might contribute to tissue engineering therapy to fix bone and periodontal defects in the future.

Concurrent use of natural health products (NHPs) with antiretroviral drugs (ARVs) is widespread among human immunodeficiency virus-infected patients. This article reviews the clinical pharmacokinetic and pharmacodynamic interactions between NHPs and ARVs. Many NHPs are complex mixtures and are likely to contain organic compounds that may induce and/or inhibit drug metabolizing enzymes and drug transporters. Although the weight of evidence for the effects of certain NHPs varies and many studies of these products lack scientific rigor, it has been observed that St. John's wort clearly induces cytochrome P450 3A4 and P-glycoprotein and reduces protease inhibitor and nonnucleoside reverse-
transcriptase
inhibitor concentrations, thereby increasing the likelihood of therapeutic failure. Limited clinical research suggests that intake of garlic and
vitamin C
results in reductions in ARV concentrations. The intake of milk thistle, Echinacea species, and goldenseal inhibits cytochrome P450 enzymes in vitro and may increase ARV concentrations, but by clinically unimportant amounts. Intake of fish oil reduces ARV-induced hypertriglyceridemia without significantly affecting lopinavir concentrations. Before recommending the use of NHPs as adjuncts to ARV use, studies should first exclude significant pharmacokinetic interactions and ensure that ARV efficacy is maintained.
