We present a comparative evaluation of the electrode systems of three modern blood gas analysers: IL-413,
ABL
-1 and AVL-937C. The response curves, accuracy and precision of the pH-, pCO2- and pO2-electrodes were established with tonometered blood and buffer solutions. pH values (range 6.8-7.8) measured on the AVL deviate (-0.03 pH for blood and +0.03 pH for buffer) from those of BMS2 Mk2; whereas on the IL and
ABL
analysers the pH values deviate by not more than 0.01 pH. The standard deviation was better than 0.005 pH. pCO2 values of blood and buffer (range 14-106 mm Hg) deviate from the calculated tonometer values by quantities ranging from 3 to 10 mm Hg. The average precision (CV)1) of the pCO2 measurement on each analyser was better than 1.8%. pO2 values of blood (range 0-130 mm Hg) did not differ by more than 3 mm Hg from the calculated values. Above 130 mm Hg a linear negative increasing difference was seen. For buffer solutions a linear relationship between pO2 difference and pO2 value was found over the whole range from zero up to 642 mm Hg: a positive difference below and a negative difference above the pO2 of the previous calibration; if the calibration pO2 is higher, the sample pO2 is shifted to a higher value. The average precision of the pO2 measurements was better than 3%. In the (patho)-physiological range the three instruments may provide suitable results for the clinician. Suggestions are made for standardization and improvement of the electrode systems.

Painful unanesthetized arterial puncture may produce transient hyperventilation, and this hyperventilation might alter resting values of arterial pH and PCO2. We investigated this possibility by comparing pH and PCO2 values of blood samples obtained by arterial puncture with values of arterialized venous blood obtained by a painless method. In 19 consecutive subjects, virtually no difference in pH or PCO2 resulted from an arterial puncture that could not be attributed to the inherent precision of the measuring instrument. Mean +/- SEM pH was identical (7.45 +/- 0.05) both before and during an arterial puncture, as was PCO2 (34.4 +/- 1.2 mm Hg). The variation (SD) in PCO2 within an individual subject was +/- 1.7 mm Hg, which was almost identical to the inherent precision of the Radiometer

In the Harleco Micro CO2 System a volumetric procedure is used to measure total CO2 content in a small volume (0.10 ml) of serum or plasma. CO2 gas is released by mixing specimen and acid, either manually or with a "Micro CO2 Shaker." Within-batch CV's by manual and machine shakings were, respectively, 7.7 and 5.6% for a total CO2 value of 8 mmol/liter, decreasing to 2.0 and 1.4% for a value of 45 mmol/liter. Between-batch CV's were 8.0 and 7.1% at low total CO2, decreasing to 2.4 and 2.0% at high total CO2. Measurements were linear for values from 5 to 60 mmol/liter. Analytical recoveries at low and high concentrations were 97.7 to 98.9%. Correlation coefficients for results obtained with the Technicon "SMA Plus" autoAnalyzer and the Radiometer

were 5% of all acute leukaemias. Four
ABL
types can be distinguished : (a) the basophilic terminal phase (basophilic blastic crisis) of cronic myeloid leukemia; (b) the mixed basophilic-oesinophilic types; (c) the promyelocytic basophilic type; and (d) histio basoblastic type. The first two are quite rare. The promyelocytic basophilic type can be easily differentiated from PNL; the frequency of the latter is three times higher. Fundamental to diagnosis are cytochemical stains specific for acid mucopolysacchaes. Myelobiopsy is always essential since in almost 50% of the cases no typical cells appears in the peripheral blood. In the bone marrow Ab are very numerous and and pleiomorphic.

is marked from its onset by a severe symptomatology. In contrast to our experience in PNL, only do some patients with

achieve complete remission.

Lung volumes and arterial blood gases have been studied in six severely obese patients (mean weight 143 kg), admitted for jejunoileal shunt-operation. The lung volumes were recorded on a Siemens constant volume body plethysmograph, and the arterial blood gases were measured by means of a Radiometer

-1 blood gas analyzer. The patients were in the seated body position. The Functional Residual Capacity (FRC) was found to be 40% (mean value) of Total Lung Capacity (TLC). Predicted normal value of FRC/TLC% in non-obese subjects of the same height, sex and age is 54%. During tidal breathing the arterial oxygen tension (PaO2) was 84 mm Hg (mean value) compared to 95 mm Hg in non-obese subjects. A positive correlation between FRC/TLC% and the PaO2 was found. A series of 5 deep breaths normalized the PaO2 which rose by 18 mm Hg to 102 mm Hg (mean value). These results confirm the generally held opinion that the main abnormality of lung function in obesity is a reduction of lung volume to such a low value that airway closure occurs during tidal breathing, causing arterial hypoxemia.

Three new pH-blood-gas analyzer systems have been evaluated. These are the
ABL
-1, the Croning 165, the Il 513. Because of automation of analysis, accuracy, and speed of analysis, this generation of instruments represents great progress over the previous one. The
ABL
-1 is the most automated system: all analyses are computer controlled. The pH, P02 and PCO2 values are accurate. Calibration can not be adjusted. The Corning 165 is the least automated system. Its pH determinations are the most accurate and it is the one which uses the smallest volume of blood (125 mul). However it is the least stable and takes the longest time for completion of the analysis. The IL 513 is probably the best balanced system and its calibration can be modified. Its operation is well automated and its results are accurate.

Lymphocytes that could bind 125I-labelled thyroid microsomal membranes (

) were present in the peripheral blood of patients with various types of thyroid disease as well as in normal healthy subjects. In patients with anti-thyroid cytoplasmic antibodies the number of
ABL
was about three times normal (11-21 +/- 0-60 compared with 3-26 +/- 0-18 per 10(4) lymphocytes). In contrast, in patients with thyroid disease without anti-thyroid cytoplasmic antibodies, the number of antigen-binding lymphocytes was not significantly greater than in the normal controls (3-94 +/- 0.14 per 10(4) lymphocytes). The binding of thyroid microsomes by antigen-binding lymphocytes could be blocked by thyroid microsomes but not by thyroid mitochondrial membranes, thyroglobulin or liver microsomes.

The performance of the Radiometer
ABL
1 blood-gas analyser has been evaluated using tonometered blood samples of known PO2 and PCO2. The pH electrode was compared with two other pH electrode systems and the haemoglobin estimations were compared with measurements made by the standard cyanmethaemoglobin method. The accuracy of the machine compared well with standard blood-gas equipment, but the haemoglobin values had a small error because of a zero offset in the machine. The analyzer is simple to operate and has low running costs.

The examination of the presence of Ph chromosome and of the fused gene BCR-

in patients with chronic myeloid leukemia (CML) is significant for the precise diagnosis and in some cases for the prognosis of the disease. We examined peripheral blood for the presence of BCR-
ABL
fused gene by polymerase chain reaction (PCR) in eight patients with CML consecutively cytogenetically studied before and after the bone marrow transplantation and in two patients treated with interferon. Southern blot analysis was performed before BMT in two patients and the molecular rearrangement of Ph chromosome was found. In all cases our results have proved that cytogenetic and recombinant DNA evaluations confirm each other. Due to the high sensitivity of PCR technique the minimal residual leukemia can be detected.
