Two hundred and fifty attendees at two London genitourinary medicine clinics were asked to complete an anonymous self-administered questionnaire, enquiring about sexual behaviour whilst abroad. Two hundred and forty-three questionnaires were evaluable. In the study group there were 116 women, and 127 men (62 heterosexuals and 65 homosexuals). Ninety women, 53 heterosexual men and 53 homosexual men had travelled abroad over the preceding 6 months. Of these 18 (20%) of women, 26 (51%) of heterosexual men and 19 (36%) of homosexual men had sex with a local foreign contact on holiday. Although both heterosexual and homosexual men were statistically more likely to have sex abroad with a local inhabitant, women were more likely to have unprotected sexual intercourse with a local partner. This has important implications for the spread of sexually transmitted disease including hepatitis B and HIV.

PMID:Sexual behaviour amongst travellers: a study of genitourinary medicine clinic attenders. 128 21

To identify the importance of heterosexual activity as a possible route for the transmission of the hepatitis C virus (HCV), a screening of antibodies against HCV (anti-HCV) was performed in 200 sexually transmitted disease patients with different risks for incurring genital infections as well as in 100 registered prostitutes. Out of all 300 persons tested, 14 cases of HCV infection were detected. Anti-HCV was present in 3 of the prostitutes and in 11 of the STD patients. Evaluating known risk factors, such as intravenous drug use or blood transfusion, 6 out of the 11 STD patients and all of the prostitutes in whom anti-HCV was present were intravenous drug users and exhibited highly promiscuous behavior. Intravenous drug use was the probable means of acquisition in 9 of the 14 subjects in whom anti-HCV was present, and homosexual promiscuous behavior was assumed to be the means of acquisition in another 2 subjects. In heterosexual patients engaging in high-risk behavior (high number of sexual partners and genital infections), the exclusion of intravenous drug use decreased the prevalence of anti-HCV from 12.1% to 4.1%, demonstrating no significant increase from the prevalence among low-risk persons. Most of the patients were screened for STDs, such as syphilis, Neisseria gonorrhoeae, Chlamydia trachomatis, human immunodeficiency virus (HIV), hepatitis B virus (HBV), trichomoniasis, and yeast infections. The seroprevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections were prospectively assessed in 356 heterosexuals with STDs (sexually transmitted diseases) and compared to a control group of 381 healthy first-time blood donors. Eighty-one of 356 STD patients were anti-HBC positive (22.8%) compared to 14/381 blood donors (3.8%; p less than 0.001). In addition, 18 of the 81 anti-HBC positive STD patients, but none of the controls, were positive for HBSAg (p = 0.06). The prevalence for anti-HCV was also significantly higher in the STD group than in the controls (5.3% vs. 0.5%; p less than 0.001). Among the various STDs syphilis (anti-HBC: 67.5%; anti-HCV: 12.5%) and Chlamydia trachomatis infections (anti-HBc: 20.2%, anti-HCV: 8.1%) had the highest prevalence for both infections. This study provides strong evidence of heterosexual transmission of hepatitis B and C virus infections. Thus, heterosexuals with STDs or multiple partners should be actively vaccinated against hepatitis B.

In order to obtain more information on sexual transmission of hepatitis C (HCV) we compared different high-risk groups for HIV and hepatitis B to see if they were seropositive for HCV. A high seroprevalence (38/81) of hepatitis C (HCV) was found among intravenous drug users. Nursing staff (n = 35) and patients of a dialysis unit (n = 57) had a low prevalence of anti-HCV antibodies (0% and 5%, respectively). Serology laboratory technicians also had a very low prevalence (0% out of 29). Among prostitutes (n = 114), healthy homosexual men (n = 132) and HIV-infected homosexual men (n = 31), we found a remarkably low seroprevalence of HCV (3.5%, 0.8% and 0.0% respectively). These data support the view that parenteral exposure to the virus is the most important way of acquiring the infection and that neither heterosexual nor homosexual promiscuity are associated with a high risk of transmission of hepatitis C.

PMID:Hepatitis C among risk groups for HIV and hepatitis B. 165 May 87

In response to the risk of occupationally acquired infection with hepatitis B and human immunodeficiency virus, the State of Oklahoma enacted legislation which provided for a system of notification of emergency medical personnel who sustain risk exposures to blood or other potentially infectious body fluids. The system is based on the immediate report of the exposure to the STD/HIV Division of the Oklahoma State Department of Health. Between January 1, 1989, and December 31, 1990, emergency response facilities reported 115 exposures to blood or other body fluids. There was a mean delay of 12 days between exposure and report to the STD/HIV Division. Only 10 (9%) of the exposed workers had been previously vaccinated against hepatitis B, and universal precautions were in use only 40% of the time. Forty-eight reports (41.7%) indicated exposures that did not pose a risk of disease transmission. These data indicate that emergency response facilities are in need of further education directed at the risk and prevention of transmission of bloodborne pathogens.

This study documents the prevalence of Hepatitis B serological markers among STD patients who have had multiple sexual partners in Kuala Lumpur, Malaysia, and compares the rates with those of a sample of the population with single or no sexual partners. A total of 336 Chinese STD patients (multiple partners group) and 234 Chinese control subjects (non-multiple partner group) were screened. Those with a history of blood transfusion or parenteral drug abuse had been excluded from the study, and all study subjects were heterosexuals. The overall carrier rate was 9.2% for the multiple partner group (MP group) and 6.8% for the non-multiple partner group (NMP group). Infection rates were 64.3% for the MP-group and 38.9% for the NMP group. After adjustments for age and sex, there was no significant difference in carrier rates between the two groups, but infection rates were significantly different with the MP group, being 3.2 times more likely to acquire infection than the NMP group. The study concludes that in heterosexuals, those with multiple sexual partners have increased chances of acquiring HBV infection.

The prevalence of viral and bacterial sexually transmitted diseases were studied in 101 men attending a dermatovenereal outpatient clinic in Mogadishu. A control group of 103 healthy adult men were included for the serological part of the study. Serological markers of hepatitis B virus (HBV), human immunodeficiency virus (HIV), cytomegalovirus (CMV) and herpes simplex virus (HSV) were studied. All sera were tested for syphilis markers. HBV serum markers were detected in 84% of the men in the study group and 66% of the healthy controls (P less than 0.005). Hepatitis B virus carriers were detected more frequently in the study group than among the controls. Also, 96% of the men in both groups had CMV antibodies and all of them had antibodies to HSV. No sera were found to contain HIV antibodies. The TPHA-positivity was 10% and 3% in the study and control groups respectively, and 5% of the patients had syphilis IgM antibodies. Sexual contact with prostitutes was recorded in 54% and 48% respectively of patients and controls, and such contact was correlated with TPHA-positivity in the study group. Chlamydia trachomatis antigen was detected in urogenital specimens of 14% of the men in the study group and gonococcal culture was positive in 53% of those with urethral discharge.

A total of 166 volunteers for an AIDS vaccine study (Vaxsyn, baculovirus produced recombinant GP160; MicroGeneSys Inc, West Haven, Connecticut, USA) were interviewed and examined. Blood was collected for routine laboratory testing as well as T-cell counts, HIV ELISA (EIA), Western blot (WB) and p24 Ag. Eighty-five men (mean age 22.2 years, range 18-42) and 81 women (mean age 23.9 years, range 17-50) volunteered; 130/166 (78%) were university students. Most had learned of the study from news media (55%), friends or workplace (37%). The most common causes for exclusion were the presence of indeterminate WB (26.5%) or a change of mind after the initial interview (24%). Other causes were abnormal cell count and differential (7.2%), elevated alanine aminotransferase (3.6%), positive hepatitis B antibody (3.6%), abnormal urinalysis (3.4%), recent venereal disease (3.0%), T4 cell count less than 400 (1.9%), abnormal chest X-ray (1.7%), recognized high-risk behaviour (1.7%), multiple sex partners (1.2%), positive rapid plasma reagin test (1.2%), failure to meet age criteria (1.2%), unable to be available for entire study (1.2%), abnormal physical examination (0.6%) and positive p24 Ag (0.6%). No volunteers had positive EIA, but 14.5% had more than one reason for exclusion. Even in a community with low prevalence for HIV, a large majority of healthy heterosexual volunteers can be expected to be ineligible for enrollment in HIV vaccine trials. An average of 4.8 volunteers were screened for each of 12 vaccinees chosen.

Three doses of hepatitis B vaccine were administered via an intradermal route to 316 health service staff. One month after the final dose, 89.9% of subjects had antibodies to hepatitis B surface antigen at levels of 10 IU/l or greater. A programme of hepatitis B immunization based upon the intradermal route is substantially less expensive than one using an intramuscular technique, and can enable health authorities to provide protection for increased numbers of staff without diverting resources from other programmes of health care.

The prevalence and risk factors of hepatitis B infection were studied in 354 non-drug abusing female prostitutes and 360 female controls in Tijuana, Mexico. Hepatitis B surface antigen (HBsAg) was found in the same percentage (0.8%) of prostitutes and controls. In contrast, antibody markers (anti-HBs or anti-HBc) were found in a significantly higher percentage of prostitutes than controls (8.2% vs. 2.2%, p = 0.0006). Prostitutes also had a higher prevalence of a positive RPR/FTA-ABS test for syphilis (p less than 0.0001). There was a significant association between the presence of hepatitis B markers and positive syphilis serology and a history of having had a STD. In this non-drug abusing population, prostitution was found to be a risk factor for total hepatitis B infection but not for surface antigenemia. Further studies are indicated to determine the incidence of chronic infection in adult women following sexual transmission of hepatitis B.
