We studied whether dynamic contrast-enhanced
MRI
(DCE-MRI) could identify histopathological characteristics of breast cancer. Seventy-five patients with breast cancer underwent DCE-
MRI
followed by core biopsy. DCE-
MRI
findings were evaluated following the scoring system published by Fischer in 1999. In this scoring system, five DCE-
MRI
features, three morphological (shape, margins, enhancement kinetic) and two functional (initial peak of signal intensity (SI) increase and behavior of signal intensity curve), are defined by 14 parameters. Each parameter is assigned points ranging from 0 to 1 or 0 to 2, with higher points for those that are more likely to be associated with malignancy. The sum of all the points defines the degree of suspicion of malignancy, with a score 0 representing the lowest and 8 the highest degree of suspicion. Associations between DCE-
MRI
features and tumor histopathological characteristics assessed on core biopsies (histological type, grading, estrogen and progesterone receptor status, Ki67 and
HER2
status) were studied by contingency tables and logistic regression analysis. We found a significant inverse association between the Fischer's score and
HER2
-overexpression (odds ratio-OR 0.608, p = 0.02). Based on our results, we suggest that lesions with intermediate-low suspicious DCE-
MRI
parameters may represent a subset of tumor with poor histopathological characteristics.

Early external detection of cancer gene activity might enable early treatment of cancer and might reduce cancer mortality. We hypothesized that oncogene mRNA overexpressed at thousands of copies per malignant cell in a zone of transformed cells could be imaged externally by scintigraphic imaging, PET (positron emission tomography) or
MRI
(magnetic resonance imaging) with PNA (peptide nucleic acid) hybridization probes that include chelators for metal cations and a cyclized peptide analogue of IGF-1 (insulin-like growth factor 1), D(Cys-Ser-Lys-Cys), to mediate internalization by
IGF1R
(IGF-1 receptor) overexpressed on cancer cells. We observed that human MCF7 breast cancer cells that overexpress
IGF1R
efficiently internalized fluorescein-chelator-PNA-D(Cys-Ser-Lys-Cys) to the cytoplasm, but not with D(Cys-Ala-Ala-Cys). Scintigraphic imaging of MCF7 xenografts in immunocompromised mice revealed that CCND1 and MYC [(99m)Tc]chelator-PNA-D(Cys-Ser-Lys-Cys) probes yielded xenograft. PET imaging with [(64)Cu]chelator-PNA-D(Cys-Ser-Lys-Cys) yielded stronger signals. Scintigraphic imaging of human AsPC1 pancreas cancer xenografts with [(99m)Tc]chelator-KRAS PNA-D(Cys-Ser-Lys-Cys) yielded strong xenograft signals. Stronger xenograft image intensities were obtained by PET imaging of [(64)Cu]chelator-KRAS PNA-D(Cys-Ser-Lys-Cys).
MRI
required extension of chelator-polydiamidopropanoate dendrimers from the N-termini of the PNA probes to increase the number of contrast paramagnetic gadolinium (III) cations per probe. These results provide a basis for detection of oncogene activity in tissues from outside the body by hybridization with metal-chelator-PNA-peptides that are selectively internalized by cancer cells.

Bisphosphonate-related osteonecrosis of the jaws (BON), first described in 2003, is gaining importance due to the increasing indication spectrum of bisphosphonate therapy [S. Takeyama, M. Ito, H. Shinoda, A novel bisphosphonate,

Marx, Pamidronate (Aredia) and zoledronate (Zometa) induced avascular necrosis of the jaws: a growing epidemic, J. Oral Maxillofac. Surg. 61 (2003) 1115-1118]. BON patients suffering from varying bony defects and symptoms are extremely restricted in their quality of life. Due to a limited knowledge of the aetiology of BON efficient evidence-based treatment strategies are lacking. Until now 23 patients with bisphosphonate-related osteonecrosis have been admitted to the Department of Cranio-Maxillofacial Surgery of the University of Zurich. A complete history has been recorded. All patients underwent clinical and radiographic examination. CT scans and
MRI
have been performed in selected cases. All patients had in common that, before signs of BON were observed, a local traumatic incidence had occurred. All patients showed signs of infection which could be remarkably reduced by antibacterial treatment. Furthermore, the period of bisphosphonate treatment was found to be one of the significant factors causing bisphosphonate-related osteonecrosis of the jaws. The aetiology of BON appears to depend on multiple factors: period and type of bisphosphonate therapy and trauma paving the way for an invasion of pathogens. Because evidence based therapy protocols for complete remodelling of bone defect are still missing, prevention in bisphosphonate-treated patients seems to be of utmost importance. A close interdisciplinary collaboration is required.

Kallmann syndrome (KS) is a developmental disease characterized by the association of isolated hypogonadotropic hypogonadism and anosmia/hyposmia. We report an unusual presentation of two females with KS and empty sella. These females, aged at 20 and 29-year-old, presented primary amenorrhea with prepubertal estradiol and low gonadotropin levels. No other significant clinical signs were observed. Empty sella was observed on
MRI
in both cases. Sequencing of
FGFR1
gene, recently implicated in autosomal form of KS, was performed and one splicing mutation (IVS14 + 1G > A) was identified in one patient.

Until now it remains less clear to what extent the different types of endoprostheses can simulate the physiological motion pattern of the knee joint. The aim of this study was to present fluoroscopy and functional
MRI
as well as the results of these in vivo imaging techniques for TKA kinematics. Videofluoroscopy is a dynamic investigation, analyzing the subjects under fluoroscopic surveillance during different activities. Three-dimensional (3D) kinematics were recovered from the two-dimensional fluoroscopic images using a model-fitting technique. Kinematic analysis with functional
MRI
was performed in an open MR system at different flexion angles with external loads being applied during imaging. Femoropatellar and femorotibial 3D kinematics were analyzed by image postprocessing. The findings in healthy knees obtained with functional
MRI
under static conditions are in good agreement with the fluoroscopic outcome under dynamic conditions. In all investigated TKA in the mean an increased external rotated position of the femur relative to the tibia was observed at full extension, while the amount of external rotation during knee flexion was decreased. Although there was great variability among the individuals, differences were observed between the TKA-groups (e.g. posterior stabilized vs
PCL
retaining). Significant changes of femorotibial and femoropatellar kinematics were found in TKA compared to healthy knees, which may lead to early aseptic loosening or increased polyethylene wear The presented techniques and results allow for advanced in vivo diagnostics and may help to improve the design of TKA and to enhance the long-term performance.

We propose a finite-element method (FEM) deformable breast model that does not require elastic breast data for nonrigid PET/
MRI
breast image registration. The model is applicable only if the stress conditions in the imaged breast are virtually the same in PET and
MRI
. Under these conditions, the observed intermodality displacements are solely due the imaging/reconstruction process. Similar stress conditions are assured by use of an
MRI
breast-antenna replica for breast support during PET, and use of the same positioning. The tetrahedral volume and triangular surface elements are used to construct the FEM mesh from the
MRI
image. Our model requires a number of fiducial skin markers (FSM) visible in PET and
MRI
. The displacement vectors of FSMs are measured followed by the dense displacement field estimation by first distributing the displacement, vectors linearly over the breast surface and then distributing them throughout the volume. Finally, the floating
MRI
image is warped to a fixed PET image, by using an appropriate shape function in the interpolation from mesh nodes to voxels. We tested our model on an elastic breast phantom with simulated internal lesions and on a small number of patients imaged, with
FMS
using PET and
MRI
. Using simulated lesions (in phantom) and real lesions (in patients) visible in both PET and
MRI
, we established that the target registration error (TRE) is below two pet voxels.

Ganglia of the knee joint are rare and are mostly an incidental finding during arthroscopy or
MRI
examinations. Usually their origin is intraarticular, arising from the menisci or ACL or
PCL
. Ganglia arising from the infrapatellar fat pad are rare and only few are mentioned in the literature. We report a case of infrapattellar ganglion in a 41-year-old female, which developed from the infrapattellar fad pad and with minimal intraarticular extension.

MRI
of flow remains a challenging problem despite significant improvements in imaging speeds. For periodic flow the acquisition can be gated, synchronizing data acquisition with the flow. However, this method fails to work if the flow is sufficiently fast that turbulence occurs, or when it is sufficiently fast that blurring occurs during the excitation of the spins or the acquisition of the signal. This paper describes recent progress in employing a very fast MR imaging technique, Single Echo Acquisition Imaging (
SEA
-
MRI
) and spin-tagging to visualize very rapid and turbulent flow patterns. Demonstrations are done on a separating channel phantom with input flow rates ranging from zero to over 100 cm/sec. Spin-tagging enables a "texture" to be placed on the spins, enabling clear visualization of the complex flow patterns, and in some cases measurement of the flow velocity.

receptor tyrosine kinase is a critical factor in the pathogenesis of gastrointestinal stromal tumors. Imatinib mesylate (IM, Glivec), a selective tyrosine kinase inhibitor, has been shown in clinical studies to work against such tumors. But there is little information on whether combination of IM and surgical treatment can prolong survival in cases with unresectable multiple liver metastases. We report a case of successful treatment of huge peritoneal metastasis from duodenal gastrointestinal stromal tumor resistant for IM. Therefore, we discuss some important implications. This 41-year-old Japanese man underwent a pancreaticoduodenectomy for GIST of the duodenum in January 2003. The postoperative course was good at first, but 3 months after the initial operation,
MRI
showed multiple liver metastases. The patient was treated with 400 mg of IM once daily with only weak liver dysfunction as a side effect. The initial response to treatment of CR continued for 20 months. Then huge mass of rt. abdomen appeared and gradually increased in size. Examination revealed that this mass is recurrent of peritoneal metastasis of the GIST. Extirpation was performed and this huge mass was recurrent GIST from omentum. Currently, IM is the first-line therapy for non-resectable GISTs, but a single agent therapy often leads to tumor resistance. IM-resistant GIST are treated with combination of novel molecular targeted-drug, RF, TAE, however, the effect is not enough. Surgical treatment is one of the successful treatments of huge peritoneal metastasis from duodenal gastrointestinal stromal tumor resistant for IM. Further examination in more cases of recurrent GIST is also necessary to estimate the effectiveness of treatment with IM.
