The porphyrias are uncommon diseases caused by enzymatic deficiencies in the heme pathway. In the 25 year period 1969-1994, the Department of Dermatology of the Hospital Clinic of Barcelona has been able to study 793 cases of porphyria (724 cases of PCT, 27 of EPP, 26 of PV, 5 of
CEP
, 5 of
HEP
, 5 of AIP, 1 of HCP). Homozygous expression of an enzymatic deficiency in the heme pathway produces severe disease. Commonly, clinical manifestations appear in the homozygous state (the autosomal recessive porphyrias). However, homozygous forms of autosomal dominant porphyrias may occur exceptionally. Moreover, there are cutaneous porphyrias whose clinical manifestations do not permit dermatologists to classify them clearly into one of the well-defined syndromes. These uncommon and atypical forms are difficult to recognize without biochemical and enzyme studies. The porphyrias have a wide clinico-biochemical spectrum, including a large proportion of well defined diseases. Nevertheless, atypical forms occur and may be difficult to evaluate. It is important to note the genetic heterogeneicity of porphyrias, which accounts for the varying phenotypic expression.

Constitutively activating internal tandem duplication (ITD) and point mutations of the receptor tyrosine kinase
FLT3
are present in up to 41% of patients with acute myeloid leukemia (AML). These
FLT3
/ITD mutations are likely to be important because their presence is associated with a poor prognosis. Both types of mutations appear to activate the tyrosine kinase activity of
FLT3
. We describe here the identification and characterization of the indolocarbazole derivative
CEP
-701 as a
FLT3
inhibitor. This drug potently and selectively inhibits autophosphorylation of wild-type and constitutively activated mutant
FLT3
in vitro in
FLT3
/ITD-transfected cells and in human
FLT3
-expressing myeloid leukemia-derived cell lines. We demonstrate that
CEP
-701 induces a cytotoxic effect on cells in a dose-responsive fashion that parallels the inhibition of
FLT3
. STAT5 and ERK1/2, downstream targets of
FLT3
in the signaling pathway, are inhibited in response to
FLT3
inhibition. In primary leukemia blasts from AML patients harboring
FLT3
/ITD mutations,
FLT3
is also inhibited, with an associated cytotoxic response. Finally, using a mouse model of
FLT3
/ITD leukemia, we demonstrate that the drug inhibits
FLT3
phosphorylation in vivo and prolongs survival. These findings form the basis for a planned clinical trial of
CEP
-701 in patients with AML harboring
FLT3
- activating mutations.

gene are found in up to 34% of patients with acute myeloid leukemia (AML) and are associated with a poor prognosis.

/ITDs result in constitutive activation of the tyrosine kinase domain and transform growth factor-dependent cell lines.
FLT3
activation leads to antiapoptotic and proliferative signals, but little is known about the impact of
FLT3
/ITDs on differentiation. This study was designed to investigate the effect of
FLT3
/ITD expression on the differentiation of the 32Dcl3 (32D) myeloblastic cell line to neutrophils in response to granulocyte colony-stimulating factor (G-CSF). Expression of
FLT3
/ITD completely blocked morphologic differentiation and induction of myeloperoxidase (MPO), lysozyme, and CCAAT/enhancer-binding protein epsilon (C/EBPepsilon) in response to G-CSF. Wild-type
FLT3
and vector-transfected 32D cells were able to differentiate, although the maturation of
FLT3
-transfected cells was delayed by
FLT3
ligand (FL) stimulation.

tyrosine kinase inhibitor, overcame the morphologic block in differentiation caused by
FLT3
/ITD expression and allowed G-CSF induction of myeloid maturation markers. These findings suggest that blocking differentiation may be one of the mechanisms by which

/ITDs contribute to leukemogenesis.

All of the cases of medullary thyroid carcinoma (MTC) express the
RET
receptor tyrosine kinase. In essentially all of the hereditary cases and approximately 40% of the sporadic cases of MTC, the
RET
kinase is constitutively activated by mutation. This suggests that
RET
may be an effective therapeutic target for treatment of MTC. We show that the indolocarbazole derivatives,

-751, inhibit
RET
in MTC cells. These compounds effectively inhibit
RET
phosphorylation in a dose-dependent manner at concentrations <100 nM in 0.5% serum and at somewhat higher concentrations in the presence of 16% serum. They also blocked the growth of these MTC cells in culture.

-751 and its prodrug,

-2563, also inhibited tumor growth in MTC cell xenografts. These results show that inhibiting
RET
can block the growth of MTC cells and may have a therapeutic benefit in MTC.

Significant antitumor efficacy was observed similarly against orthotopically implanted LNCaP human prostate carcinomas in male nude mice and orthotopically implanted renal carcinoma (RENCA) tumors in BALB/c mice, in terms of a significant reduction in the metastatic score and the extent of pulmonary metastases. Constitutively activating mutations of
FMS
-like tyrosine kinase 3 (FLT3) occur in approximately one third of patients with acute myeloid leukemia (AML) and are associated with poor prognosis. Altered FLT3 signaling leads to antiapoptotic and proliferative signaling pathways. We recently showed that these mutations can also contribute to the differentiation arrest that characterizes leukemia. In this report we investigated the mechanism by which internal tandem duplication (ITD) mutation of FLT3 signaling blocks differentiation. Normally, myeloid differentiation requires the induction of CCAAT/enhancer-binding protein alpha (C/EBPalpha) and PU.1 expression. Expression of both genes was repressed by FLT3/ITD signaling in 32Dcl3 (32D) cells and this repression was overcome by treatment with a FLT3 inhibitor, allowing differentiation to proceed. We also observed increased expression of C/EBPalpha and PU.1 accompanied by signs of differentiation in 2 of 3 primary AML samples from patients with FLT3/ITD mutations receiving a FLT3 inhibitor,

A series of potent vascular endothelial growth factor R2 (VEGF-R2) tyrosine kinase inhibitors from a new indenopyrrolocarbazole template is reported. The structure-activity relationships for a series of 9-alkoxymethyl-12-(3-hydroxypropyl)indeno[2,1-a]pyrrolo[3,4-c]carbazole-5-ones revealed an optimal R9 substitution with ethoxymethyl 19 (VEGF-R2 IC(50) = 4 nM) and isopropoxymethyl 21 (VEGF-R2 IC(50) = 8 nM) being the most potent inhibitors in the series. The VEGF-R2 activity was reduced appreciably by increasing the size of the R9 alkoxy group or by alpha-methyl branching adjacent to the ring. The combined R9 alkoxymethyl and N12 hydroxypropyl substitutions were required for potent VEGF-R2 activity, and the corresponding thioether analogues were weaker than their ether counterparts. Activating mutations of
FMS
-like tyrosine kinase 3 (FLT3) are present in approximately 30% of patients with de novo acute myeloid leukemia (AML) and are associated with lower cure rates from standard chemotherapy-based treatment. Targeting the mutation by inhibiting the tyrosine kinase activity of FLT3 is cytotoxic to cell lines and primary AML cells harboring FLT3 mutations. Successful FLT3 inhibition can also improve survival in mouse models of FLT3-activated leukemia.

-701 as salvage treatment for patients with refractory, relapsed, or poor-risk AML expressing FLT3-activating mutations. Fourteen heavily pretreated AML patients were treated with

-701 at an initial dose of 60 mg orally twice daily.
CEP
-701-related toxicities were minimal. Five patients had clinical evidence of biologic activity and measurable clinical response, including significant reductions in bone marrow and peripheral blood blasts. Laboratory data confirmed that clinical responses correlated with sustained FLT3 inhibition to
CEP
-701. Our results show that FLT3 inhibition is associated with clinical activity in AML patients harboring FLT3-activating mutations and indicate that
CEP
-701 holds promise as a novel, molecularly targeted therapy for this disease.

inhibitors into existing chemotherapeutic regimens has the potential to improve clinical outcomes in this high-risk group of patients.

FLT3
inhibitor, potently induces apoptosis in
FLT3
/ITD-expressing cell lines and primary leukemic blasts. We conducted a series of in vitro cytotoxicity experiments combining
CEP
-701 with chemotherapy using the
FLT3
/ITD-expressing cell lines MV4-11 and BaF3/ITD as well as a primary blast sample from a patient with AML harboring a
FLT3
/ITD mutation.
CEP
-701 induced cytotoxicity in a synergistic fashion with cytarabine, daunorubicin, mitoxantrone, or etoposide if used simultaneously or immediately following exposure to the chemotherapeutic agent. In contrast, the combination of pretreatment with
CEP
-701 followed by chemotherapy was generally antagonistic, particularly with the more cell cycle-dependent agents such as cytarabine. This effect appears to be due to
CEP
-701 causing cell cycle arrest. We conclude that in
FLT3
/ITD-expressing leukemia cells,
CEP
-701 is synergistic with standard AML chemotherapeutic agents, but only if used simultaneously with or immediately following the chemotherapy. These results should be considered when designing trials combining chemotherapy with each of the
FLT3
inhibitors currently in clinical development.
