Parotid gland
carcinomas are rare. Among them, the epithelial-myoepithelial carcinoma (EMC) is extremely rarely diagnosed. This case report was based on a female with a history of 17 years of recurrent EMC of the parotid and evidence of distant metastasis over a period of 2 years. During debulking procedures of her extensive facial tumour, small tumour samples were transplanted to nude mice. The tumours grew well on the mice and were characterized morphologically and immunohistochemically after explantation. Cellularity per mm(2) ranged between 3,470 and 7,410. The tumours were characterized by the typical bipolar pattern of tumour cells and broad stromal septae. All but one of 7 transplanted tumours were positive for pan-cytokeratin marker KL-1. The proliferation index in terms of MIB-1-stained nuclei increased from 2% to 20% and was correlated positively to the expression of
EGFR
. IGF-1R-, VEGF- and
FLK1
-stained cells were found in all cases. The increase in
EGFR
- and MIB-1-positive cells correlated with the clinical course of the patient, who showed shorter periods of tumour recurrence prior to her death. These findings in EMC transplanted to the nude mouse demonstrate the feasibility of growing EMC in vivo.

BACKGROUND:
Parotid gland
metastasis in breast cancer is extremely rare, and only 14 cases have been reported between 1982 and 2010. CASE REPORT: A 67-year-old female patient was diagnosed with invasive lobular carcinoma of the left breast. Although clinical staging was T1N3M1 (stage IV), the tumor experienced a complete response to chemotherapy. We therefore performed a mastectomy followed by radiotherapy, and continued administration of trastuzumab. However, 11 months later, the patient complained of a swelling in the left parotid gland. Histology following a partial parotidectomy revealed a parotid gland metastasis from the breast. CONCLUSION: Treatment with capecitabine in addition to trastuzumab, which is one of the strategies applied in
HER2
-positive breast cancer, was effective in our patient. Analysis of the 14 cases of parotid gland metastasis from the breast reported between 1982 and 2010 revealed that the metastasis may occur not by direct lymphatic but by hematogenous spread.

Mammary gland analog secretary carcinoma (MASC) of salivary gland is typically a tumor of low histologic grade and behaves as a low-grade malignancy with relatively benign course. This tumor shares histologic features, immunohistochemical profile, and a highly specific genetic translocation, ETV6-

, with secretory carcinoma of breast. Histologically, it is often mistaken as acinic cell carcinoma, adenocarcinoma not otherwise specified, and other primary salivary gland tumors. Here we report a case of MASC with high-grade transformation and cervical lymph node metastases confirmed with ETV6-
NTRK3
translocation arising in the hard palate of a 41 year-old adult. Interestingly, the metastatic carcinoma has lower grade than the original tumor which strongly support malignant transformation of the original tumor. Most commonly, MASC arises from the parotid gland and less often in minor salivary glands. Metastasis is relatively uncommon and high-grade histology has only been reported in four cases with three of them arising from the parotid gland and the location of the fourth one has not been reported. This is the first case with high grade histology that arise from minor salivary gland and it emphasizes the importance of molecular screening of salivary gland tumor with high-grade histology for ETV6-
NTRK3
translocation. In our literature of 115 cases that includes the current case, MASC occurred predominantly in adult with only a few cases under 18 years of age and a male to female ratio of 1.2:1.
Parotid gland
is more commonly affected but there is also significant occurrence in minor salivary glands. Except for the cases with high grade histology, the overall prognosis is good.
