Colchicine treatment was used in this randomized placebo-controlled trial in patients with severe acute alcoholic hepatitis [serum bilirubin greater than or equal to 5 mg/dL (85.5 mumol/L) mean, 17.5 +/- 7.5 mg/dL (299.25 +/- 128.25 mumol/L)]. Hospitalization mortality and morbidity and the effect on biochemical test results were the end points of the treatment. Patients in the two groups were evenly matched by demographics and laboratory test results. Mean time to study entry was less than 7 days from admission. The duration of the trial was 30 days. Thirty-six patients (24 men, 12 women) received colchicine (1 mg orally every morning) and 36 (25 men, 11 women) received an identical placebo. Seven (19%) colchicine-treated and six (17%) control patients died during the index hospitalization after a mean of 17.4 +/- 10.8 and 17.8 +/- 5.3 days, respectively (NS). During a 4-month follow-up period from entry into the trial, there were two additional deaths in each group. No differences between placebo- and colchicine-treated patients were observed in any of the laboratory parameters (serum bilirubin, aspartate transaminase,

, and creatinine) that were followed up over the 30-day treatment period. The frequency of complications did not differ statistically between the two groups. The distributions of erythrocyte values (red cell count,

concentration, etc.) of healthy male workers working in a certain factory were studied in order to evaluate the usefulness of the variables as health indicators. In addition to the estimation of erythrocyte values, anamneses concerning chronic bleeding, smoking habit and alcohol consumption were questioned and serum Fe, Cu, TIBC, ferritin, plasma erythropoietin, and serum enzymes (GOT,
GPT
, gamma-GTP) were measured. Two-dimensional frequency distributions with axes for the red cell count and
hemoglobin
concentration, and frequency distributions of the score calculated from principal component analysis, showed bimodal patterns. Using nonlinear curve fitting methods, the distributions of principal component scores were fitted to a mixture of two different Gaussian distributions. The workers were then divided into two groups corresponding to the Gaussian distribution he belongs. Then the frequencies of the items and mean values of the variates were compared between them. There were no differences in the incidences of diseases that caused iron deficiencies, but the mean serum ferritin level was significantly lower in one group than in the other, in other words the workers belonging to the group with low serum ferritin level had smaller iron stores than the other. The mean serum erythropoietin level and the mean serum copper level were higher in that group than in the other. As a result, this analysis gives a new evaluation of the health status of a man who belongs to the so-called healthy male group.

Theophylline administration to seven healthy male volunteers resulted in a rapid and significant decline in both plasma and erythrocyte pyridoxal-5'-phosphate levels. Total erythrocyte pyridoxal kinase levels increased during 15 wk of theophylline treatment from a mean initial activity of 19.23 +/- 5.03 (mean +/- SD) to 62.64 +/- 11.59 nmol pyridoxal-5'-phosphate formed/(g

Although plasma pyridoxal levels remained normal, the threefold increase in total erythrocyte pyridoxal kinase activity levels did not normalize plasma and erythrocyte pyridoxal-5'-phosphate levels. We employed cross circulation (CC) as an assist method for canine orthotopic cardiac transplantation in 8 dogs to obtain long term survivals. Hematological and biochemical examinations were performed intra- and postoperatively, to estimate usefulness of CC. The support dog weighted about 3 times as much as the recipient dog. Using different anesthesia, the systolic pressure of the support dog and the pressure gradient between the support and recipient were kept above 150 mmHg and 50 mmHg, respectively. Cyclosporin and prednisolone were administered for immunosuppressive therapy. All could be extubated on the 1st to 3rd postoperative day, and 6 survived more than 2 weeks. During the operation under CC, changes of LDH and lactic acid were much less, no free
hemoglobin
was detected, and total protein and hematocrit remained well compared with operation under heart-lung machines. CRK, CPKMB, GOT and
GPT
increased shortly after operation, but returned to the preoperative values within 2 weeks. In CC, owing to avoidance from mechanical destruction of red blood cells, coagulation disorders and hemodilution, hemolysis and hemorrhage could be decreased, and oncotic pressure remained well, and no severe damage occurred in the recipient organs. Thus, CC is useful as an assist method for canine orthotopic cardiac transplantation.

The primary cause of death following i.v. injection of the basic phospholipase A2 (PLA2) from Naja nigricollis venom has been attributed to its direct cardiotoxicity. In view of our recent findings that cardiac failure caused by the basic PLA2 from Naja m. mossambica is primarily due to hyperkalemia resulting from cellular damage and possibly also from hemolysis, the cause of death due to the basic PLA2 from Naja nigricollis was re-investigated. In the anesthetized mice and rats, the PLA2 (0.3 micrograms/g, i.v.) produced a transient hypotension followed by recovery and subsequently by cardiac failure with ECG changes suggestive of hyperkalemia, such as P-R prolongation, tall T-wave, biphasic QRS-T complex, low voltage of QRS, A-V block, etc. Analysis of blood chemistry revealed marked increases in the plasma levels of K+, CPK, LDH, GOT,
GPT
, inorganic phosphate and
hemoglobin
(probably a mixture of
hemoglobin
and myoglobin). In the atrial preparation, however, no marked cardiotoxicity was observed except for a slight negative inotropic effect at 30 micrograms/ml. When 200 micrograms of the enzyme was injected into the coronary circulation in the Langendorff preparation, also no marked cardiotoxic effect was observed except for a decrease (about 40%) of coronary flow. From these results, it is concluded that the primary cause of death following i.v. injection of the basic PLA2 from Naja nigricollis is apparently cardiac failure due to hyperkalemia, resulting from cellular damage and possibly also from hemolysis, rather than direct cardiotoxicity.

Serum osmolalities and osmolar gap were determined in 43 normal healthy beagles (control group) and 40 dogs with dirofilarial hemoglobinuria (hemoglobinuria group). In the control group, the measured and calculated serum osmolality levels were in the means of 296 +/- 5 (SD) mOsm/kg and 293 +/- 6 mOsm/kg respectively, showing an osmolar gap less than 10 mOsm/kg. In the hemoglobinuria group, the measured serum osmolality ranged from 272 to 370 mOsm/kg. A considerable number of dogs had normal serum osmolalities in spite of severe intravascular hemolysis, suggesting that the changes in serum osmolality would not be the direct cause of intravascular hemolysis. The measured serum osmolality (331 +/- 28 mOsm/kg) was significantly higher in 11 dogs which died after a surgical removal heartworms than in 29 dogs which recovered after the removal (302 +/- 17 mOsm/kg). The calculated serum osmolality level was 296 +/- 16 mOsm/kg in 24 recovered cases, and 304 +/- 22 mOsm/kg in 10 fatal cases. The osmolar gap stayed in the normal ranges of 5.4 +/- 5.9 mOsm/kg in recovered cases, but it attained a higher level of 22.7 +/- 8.9 mOsm/kg in fatal cases, suggesting poor prognosis in cases with large osmolar gaps.
