Nine inbred strains of mice, which are progenitors of recombinant inbred sets, were evaluated for aortic lesion formation and plasma and liver lipid levels. This survey was done to determine if a semi-synthetic high-fat diet could elicit the same extent of diet-induced atherosclerosis as that observed in mice fed a natural ingredient high-fat diet and to discover strain-specific plasma and liver lipid variants for future genetic characterization. Evaluation of aortic lesions after 18 wk of diet consumption showed that strains C57BL/6J, C57L/J, SWR/J and SM/J were susceptible to atherosclerosis and that A/J, AKR/J, C3H/HeJ, DBA/2J and SJL/J were relatively resistant. High-density lipoprotein cholesterol (HDL-C) levels were negatively correlated to lesion formation. Susceptible strains had decreased HDL-C levels when switched from chow to the semi-synthetic high-fat, high cholesterol diet, whereas resistant strains either showed no change or a slight increase in HDL-C levels. The exception to this pattern was found in SM mice, which were susceptible to aortic lesion formation but maintained the same HDL-C level on both chow and high-fat diets. HDL size differed among the strains, and levels of plasma apolipoprotein A-I and A-II correlated with HDL-C levels. Liver damage was not correlated to HDL-C levels or to susceptibility to atherosclerosis. Mice from strain A, which are resistant to atherosclerosis, had evidence of liver damage as observed by elevated levels of plasma alanine aminotransferase activity, by liver histology, by increased liver weight and by exceptionally high hepatic cholesterol content.(ABSTRACT TRUNCATED AT 250 WORDS)

The efficacy and safety of simvastatin were evaluated in an open multicenter study over a 24-week period. One hundred seventy-two patients (91 men, 81 women) with primary hypercholesterolemia (mostly polygenic) were enrolled in 14 Centers in Northern Italy. The mean age was 55.8 +/- 9.7 years and the mean baseline total cholesterol level was 305 +/- 59 mg/dL. After 4 weeks on an AHA step 1 diet, patients who met the inclusion criterion (total cholesterol > or = 250 mg/dL) were given simvastatin 10 or 20 mg in the evening. The dose could be titrated up to a maximum of 40 mg o.d. at week 6 and 12. No dose titration was allowed after week 12. One hundred forty-nine patients (86.6%) completed the study according to the protocol, 2 (1.2%) were withdrawn from the study because of adverse events not related to the drug, 21 (12.2%) were unavailable for follow-up. Simvastatin treatment was associated with a sustained dose-related reduction in total and LDL cholesterol (-28% and -39% respectively at the end of the study). Triglycerides showed a significant descending trend (-16% at week 24) and HDL-C increased by 9%. Apolipoproteins were measured in only 25 patients: apo B was reduced by 30% and apo A1 increased by 9%. Clinical side effects were not relevant. Mean levels of GOT, GPT and CPK significantly increased after 6 weeks on simvastatin, but remained stable and at any rate ioitlin the normal range thereafter. PMID:[Effectiveness and tolerability of simvastatin in subjects with primary hypercholesterolemia. Multicenter study]. 848 62

The purpose of this study was to investigate the relation between body weight reduction and some complications associated with obesity. The subjects were 248 middle-aged obese persons (male 37, female 209) who participated in a weight reduction program at the Aichi Prefectural Center of Health Care from 1983 to 1990. The subjects had no severe complication, and the obesity indexes of all of them were over 120%. The program was held monthly for five months and consisted of diet (1500 kcal/day) and exercise (10000 walk steps/day). After the program, mean obesity index decreased by about 9%, and mean body weight reduction was about 5kg. Blood pressure, total cholesterol, triglyceride and GPT were significantly decreased, and HDL cholesterol was slightly increased. For each item the percentage of persons with abnormal values reduced by about 5 to 20%. From the relationship between degree of decrease of obesity and changes of the values of each item, it was estimated that some complications of obesity require greater than 5 to 10% decrease in obesity index to be affected. The value of each examination item before the program and the degree of decrease of obesity index had an effect on changes of the values, but obesity index before the program and age had little effect.

A cholestatic syndrome has been reported as one of the main side effects of CyA therapy. The aim of the present study was to evaluate frequency and degree of severity of the cholestatic syndrome in a group of patients with renal transplant treated with CyA. In 55 patients we evaluated both clinical: jaundice, pruritus, presence of biliary lithiasis and biochemical parameters: total serum biliary salts (TBS), total bilirubin (TB), alkaline phosphatase (AP), gammaglutamyl transpeptidase (GGT), transaminase (AST, ALT), cholesterol (CT), triglycerides (TG), HDL-cholesterol (HDL-C) and compared them with a control group matched for sex and age. In the transplant patients significantly higher values of TBS, TB, AP (p < 0.05) were found; 55% of the patients had above mean values of at least one of the classical parameters of liver function and an higher frequency of biliary lithiasis was also found, in the absence of the classical risk factors. However, none of the patients presented severe signs of hepatic disease and to date it has never been necessary to stop treatment. In conclusion, our study shows that the dosage of CyA used at present is quite safe; however, it is necessary to monitor in these patients some parameters of liver function to prevent the minor side effects we observed from progressing into more serious damage.

In this study, for determining the influence of different life style on the serum alpha hydroxybutyrate dehydrogenase activity (alpha HBD), we carried out surveys and laboratory studies on the relationships of the nutritional intake, physical activity, cigarette smoking and alcohol consumption of young females with their serum alpha HBD activity. There are significant positive correlation between alpha HBD and GOT, GPT, LDH, CPK, phospholipids, total cholesterol, and HDL-cholesterol, and significant negative correlation between alpha HBD/LDH and GOT, GPT, and gamma GTP. There are significant positive or negative correlation between alpha HBD and systolic pressure, diastolic pressure, cardiac index, stoke volume and total periperal resistance, and significant negative correlation between alpha HBD/LDH and vital capacity in young females. The serum alpha HBD activity levels of nonsmokers was significant higher than those of smokers (p < 0.05). There are significant positive correlations between serum alpha HBD activity and beverage consumption, and between alpha HBD/LDH and protein, calcium, vitamin A, B1, B2, C, pulses, vegetables and milk in young females. The finding shows that the group of serum alpha HBD activity level of 240 or more units exhibited higher level of rate of those who exercised sometimes+usually than the other group in young females. However, the serum alpha HBD activity level did not show significant correlation with energy expenditures per day. Exercise at 50 and 70% VO2 max may produce a significant increase in their serum alpha HBD activity. There is a possibility that serum alpha HBD activity may serve as an indicator for ones health condition. It is undeniable that observation of variation of serum alpha HBD activity is highly significant in health maintenance.

The nature of changes in the lipid profile caused by an acute infection is controversial. The aims of the present study were to study the changes in plasma lipids and lipoproteins in community-acquired pneumonia, to determine whether these changes differ according to the aetiologica/agents, and finally to observe the behaviour of these lipoproteins six months later. Sixty patients, aged between 18 and 87 years, admitted during the period September 1992 and April 1993 with suspected community-acquired pneumonia, were included in the study. Fifty-three of the patients completed the 15-day follow-up investigation, and 37 remained available for study for up to 6 months. On admission and at 15 and 180 days, analyses were carried out for total cholesterol, HDL cholesterol, apolipoproteins A1 and B, triacylglycerols and transaminases. Student's t test for parametric variables was used for statistical analysis, and the Mann-Whitney test for non-parametric variables. The concentrations of total cholesterol (4.2 +/- 1.0 vs 5.5 +/- 1.3 mmol/1), HDL cholesterol (0.9 +/- 0.4 vs 1.2 +/- 0.3 mmol/l), apolipoprotein A1 (0.80 +/- 0.25 vs 1.15 +/- 0.28 g/l) and apolipoprotein B (0.77 +/- 0.28 vs 0.95 +/- 0.28 g/l) showed significantly lower values during the acute infectious process. These analyte concentrations became stable after 15 days with the exception of HDL cholesterol which continued to increase until 6 months (1.2 +/- 0.3 vs 1.3 +/- 0.3 mmol/l, p < 0.01). Patients with non-viral atypical pneumonia showed, on admission, higher triacylglycerol values (1.8 +/- 0.8 vs 1.3 +/- 0.9 mmol/l, p < 0.01) and lower HDL cholesterol values (0.6 +/- 0.3 vs 1.0 +/- 0.4 mmol/l, p < 0.03). Values of aspartate aminotransferase (112 +/- 117 vs 23 +/- 11 U/l, p < 0.001), alanine aminotransferase (127 +/- 141 vs 24 +/- 16 U/l, p < 0.02) and gamma-glutamyl transferase (113 +/- 158 vs 33 +/- 25 U/l, p < 0.03) were higher in the subgroup of non-viral atypical pneumonia. In conclusion, patients with community-acquired pneumonia present a significant decline in total cholesterol, HDL cholesterol and apolipoprotein A1 and B concentrations. Lower concentrations of HDL cholesterol are maintained up 15 days. Patients with non-viral atypical pneumonia present on admission significantly higher triacylglycerol and lower HDL cholesterol values. Those with non-viral atypical pneumonia also present higher transaminase values.

The relationships of change in body mass index (BMI) on blood pressure, blood and serum tests, and of life style, working posture and job category on change in BMI after age twenty were investigated in a cross sectional study. A total of 771 male employees aged 21 or older (mean age 29.9 +/- 9.1 years) of a semiconductor factory were examined in December 1993. BMI change of each subject was expressed as percent increase after age twenty. BMI value at age twenty was calculated from self-reported body weight at twenty and height measured at the 1993 health examination. Prevalences of high blood pressure (systolic blood pressure > or = 160 mmHg or diastolic blood pressure > or = 95 mmHg), high blood pressure including borderline hypertension (systolic blood pressure > or = 140 mmHg or diastolic blood pressure > or = 90 mmHg) and abnormal values of serum GOT, GPT, gamma-GPT, total cholesterol, triglyceride, HDL-cholesterol, uric acid and fasting blood glucose were greater in the high BMI change group than in the low BMI change group (Chi-square test, P < 0.1). With the exception of hypertension, these associations remained virtually unchanged after adjusting for age, alcoholic consumption, smoking habits and BMI at 20 years of age by multiple logistic regression analysis. After adjustment for age by the Mantel-Henszael method, increases in BMI after 20 years of age were positively associated with the following eight items (p < 0.05): shorter standing time, shorter walking time, lower meal skipping frequency, shorter meal time, longer sitting time during work, larger meal size, greater frequency of salad consumption, and sedentary work such as clerk and engineer. Greater rice consumption and higher frequency of instant foods consumption were weakly associated with the increase in BMI (0.05 < p < 0.1). Job categories such as clerk and engineer were significantly associated with longer sitting time and shorter meal time, resulting in greater BMI increase after 20 years of age. Multiple logistic regression analysis revealed that shorter standing time, shorter meal time, larger meal size, greater frequency of instant foods consumption and greater frequency of salad consumption were independently correlated with the increase in BMI (p < 0.05). Larger rice consumption was slightly correlated with increasing BMI (0.05 < p < 0.1). This study showed that job category, working posture and food intake patterns were strongly associated with BMI change after age 20 in young male workers. This study provides useful information for health care and health promotion programs in young male workers.

In a prospective study 40 perimenopausal women with climacteric complaints between 45 to 52 years of age were examined. Twenty three (100%) were assigned to treatment with hormonal replacement therapy (Climen fy Schering-estradiol valerate cyproteron acetate) for 6 months. The therapy had to be stopped in 5 women (22%) for side effects. The drug was contraindicated in 17 patients because of non-treated hypertension, varices, hepatopathy and uterine fibroids with repeated metrorrhagia. In 18 patients (78%) Climax score, ALT, AST, bilirubin, total cholesterol, HDL-Chol, LDL-Chol, triglycerides, haemogram, fibrinogen, APTT, Quick test, serum Ca2+ and P2+ and urine C2+ and P2+/24 hours, weight and arterial blood pressure before and after the treatment were examined. The results confirm the benefical effect of Climen on the Climax score (p < 0.0001), total cholesterol (p < 0.01), HDL-Chol (p < 0.05) and LDL-Chol (p < 0.0001). After administration of Climen a significant decrease of P2+ serum concentration was demonstrated. Climen is suitable for treatment of acute climacteric complaints and has benefical effects on lipid metabolism.

It was reported previously that selection for high (HG) or low (LG) plasma total cholesterol (TC) at 8 wk of age in a composite four-breed swine population resulted after four generations in divergent mean concentrations in the selected lines. The data revealed a significant positive correlation between body weight (BW) and TC concentration at 8 wk of age and differential responses in litter size, backfat depth, and carcass length at 6 mo of age. We report here the relationship between plasma TC concentration and other plasma traits related to growth and metabolism in the seventh generation of selection in these two lines of pigs. We measured plasma concentrations of TC, HDL cholesterol (HDL-C), triglycerides (TG), alkaline phosphatase (ALP), total protein (TP), albumin (ALB), urea nitrogen (urea N), and three transaminases (alanine aminotransferase, ALT; aspartate aminotransferase, AST; gamma glutamyltransferase, GGT) in seventh-generation male and female pigs at 8 wk of age. Birth weight (1.48 vs 1.38 kg), 8 wk BW (14.85 vs 12.00 kg), TC (116.8 vs 63.6 mg/dL), HDL-C (43.9 vs 25.5 mg/dL), TG (50.5 vs 33.0 mg/dL), and ALP (78.3 vs 44.9 units/L) were higher (P < .01) in HG than in LG pigs, whereas ALB (3.2 vs 3.4 g/dL), ALT (43.0 vs 45.9 units/L), and AST (53.0 vs 62.2 units/L) were lower in HG than in LG pigs (P < .05). At 8 wk, overall plasma TC concentration was correlated with BW (r = .34, P < .01) and with ALP (r = .23, P < .05) but was not related to ALT, AST, or GGT. Plasma TP urea N, and GGT were unaffected by genetic line on sex. We conclude that the difference between HG and LG pigs in TC concentration in generation 4 at 8 wk of age has persisted but not broadened in pigs of generation 7, that changes in plasma ALP, ALT, and AST may have occurred in response to selection for high or low plasma TC, and that ALP is correlated with plasma TC concentration.

Hyperlipidaemia of 18 male and 20 female patients following successful renal transplantation was treated with daily 20 mg fluvastatin (Lescol) for 12 weeks. The patients were several months after transplantation, and their total cholesterol levels exceeded 6.5 mmol/l following an 8-week diet. The effect of fluvastatin on the levels of total cholesterol, HDL, LDL, triglyceride, Apo A1 and Apo B, as well as of lipoprotein(a) was examined. Furthermore, changes of the renal function (GFR-urea, creatinine, uric acid) and hepatic function (bilirubin, GOT, GPT, CPK, ALP) were followed up, together with the body weight and blood pressure. The results of the examinations are summarized as follows: Fluvastatin may be administered effectively and without side effects in a daily dose of 20 mg in appropriately selected renal transplant patients. The average total cholesterol values, which were 7.91 mmol/l in men and 7.78 mmol/l in women following the diet, were reduced by 22-25% (p < 0.001) after 6 and 12 weeks, respectively, of fluvastatin treatment. The levels of LDL also decreased significantly (p < 0.001): in response to a 20 mg evening dosage, reduction of more than 25% was observed in 78% of men and 65% of women. Reductions of the Apo B levels were more pronounced in the females (18.3% men vs. 21.2% women). The ratio C/HDL-C decreased both in men (from 5.49 to 4.19) and in women (from 4.83 to 4.02). The ratio Apo B/Apo A1 also decreased (men: from 0.86 to 0.73, women: from 0.73 to 0.66). The concentrations of HDL and Apo A1 did not increase significantly, the reductions in the levels of triglyceride and lipoprotein(a) were not considerable either. An increase in the levels of hepatic enzymes and CPK was not encountered during the administration of fluvastatin. In two patients the levels of serum bilirubin increased by 2-4 micromol/l. Three patients complained about temporary myalgias of the sacroiliac or lumbar region which, however, were not accompanied by elevated CPK levels. The monitored levels of cyclosporine, urea and creatinine did not increase significantly during the 12 weeks of treatment. Two patients had temporary gastric complaints.
