To detect early hepatic effects of chronic exposure to low-level trichloroethylene (TCE), serum total cholesterol (T-C), high-density-lipoprotein cholesterol (HDL-C) and activities of three serum enzymes [aspartate aminotransferase (AST),
alanine aminotransferase
(
ALT
), and gamma-glutamyl transpeptidase (GGT)] were determined in 148 workers (a cross-sectional study) and in 13 workers (a 2-year follow-up study) occupationally exposed to TCE in air. In the cross-sectional study, three exposure groups were defined by urinary total trichloro-compounds (U-TTC) levels of the workers [low-exposure group (n = 49): U-
TTC
< 10 mg/g creatinine; moderate-exposure group (n = 56): U-
TTC
10- < 100; high-exposure group (n = 43): U-
TTC
> or = 100]. With increasing exposure levels, T-C (P = 0.143 by ANOVA) and HDL-C (P = 0.080 by ANOVA) slightly increased. The exposure, however, had no effect on the activities of the three serum enzymes. In the follow-up study, the fluctuations in U-
TTC
were well reflected in subclinical changes in HDL-C, AST, and GGT, but not in T-C or
ALT
. These results suggest that exposure to low-level TCE influences hepatic functions, affecting cholesterol metabolism rather than causing hepatic cell damage, and that these influences are subclinical and reversible. The increases in HDL-C caused by exposure to low-level TCE may be an example of "chemical hormesis" in humans.

The inducible crassulacean acid metabolism (CAM) plant Mesembryanthemum crystallinum accumulates malic acid during the night and converts it to starch during the day via a pathway that, because it is located in different subcellular compartments, depends on specific metabolite transport across membranes. The chloroplast glucose transporter (pGlcT) and three members of the phosphate translocator (PT) family were isolated. After induction of CAM, transcript amounts of the phosphoenolpyruvate (PEP) phosphate translocator (PPT) and the glucose-6-phosphate (Glc6P) phosphate translocator (
GPT
) genes were increased drastically, while triose phosphate (TP) phosphate translocator (
TPT
) and the pGlcT transcripts remained unchanged. PPT- and
GPT
-specific transcripts and transporter activities exhibited a pronounced diurnal variation, displaying the highest amplitude in the light. pGlcT transcripts were elevated towards the end of the light period and at the beginning of the dark period. These findings, combined with diurnal variations of enzyme activities and metabolite contents, helped to elucidate the roles of the PPT,

TPT
and pGlcT in CAM. The main function of the PPT is the daytime export from the stroma of PEP generated by pyruvate orthophosphate:dikinase (PPDK). The increased transport activity of
GPT
in the light suggests a higher requirement for Glc6P import for starch synthesis rather than starch mobilization. Most likely, Glc6P rather than 3-phosphoglycerate or triose phosphates is the main substrate for daytime starch biosynthesis in M. crystallinum plants in which CAM has been induced (CAM-induced), similar to non-green plastids. In the dark, starch is mobilized both phosphorylytically and amylolytically and the products are exported by the
GPT
,
TPT
and pGlcT. The transport activities of all three phosphate translocators and the transcript amounts of the pGlcT adapt to changing transport requirements in order to maintain high metabolic fluxes during the diurnal CAM cycle.

Acute Fatty Liver of Pregnancy. The acute fatty liver of pregnancy (AFLP) is an uncommon entity, potentially fatal, which affects women during the last quarter of pregnancy. It is characterized by a prodromic period of symptoms followed by jaundice, hepatic failure, clotting disorders and fatty infiltration of the liver, evident through hepatic biopsy. The incidence ranks from 1 to 20 thousand births, and it is more frequent among women with multiple pregnancies. We report the case of a 29-year-old patient, with multiple pregnancy 33 to 34 weeks of gestation, blood pressure values of 140/90 mmHg, 160,000/dL platelets, PT 25.6 seconds,

64.7 seconds, blood glucose 52 mL/dL, creatinine 2.1 mg/dL, uric acid 11.9 mg/dL, lactic dihydrogenase 1063 U/l,

220 U/l, AF 1172 U/l, total bilirubin 8.4 mg/dL, proteinuria 30 mg/dL. A cesarean section was practiced after correcting the coagulation disorders. The first twin was a male with birth weight of 2,070 g, APGAR 8-9; the second twin was a female fetal death weighting 2,050 g. Hepatic biopsy confirmed the diagnosis. The cause of AFLP is unknown. The frequency among multiple pregnancies is higher. Almost half of the cases have hypertension and proteinuria. There are also high levels of both transaminases, phosphatase and bilirubins and hypoglycemia. The prothrombin time is enlarged. The differential diagnostic between pre-eclampsia and AFLP is not crucial since the obstetric management is the same. The main treatment is promptly deliverance and general measures. The obstetrician must be aware of this hepatic disease.

The claim by Nigerian traditional herbal medicine practitioners that Ocimum gratissimum leaves has antidiabetic properties was investigated. Diabetes mellitus was induced with streptozotocin and graded doses of the aqueous leaf extract were administered orally to the experimentally diabetic rats for 28 days. Administration of the aqueous leaf extract caused a statistically significant reduction in plasma glucose level in streptozotocin induced diabetic rats. The extract appeared nontoxic as evidenced by normal serum levels of AST,
ALT
, ALP,
TPT
, ALB and bilirubin. These data appear to agree with claimed hypoglycaemic effects of Ocimum gratissimum.
