Seven healthy men volunteers received 6.6 +/- 1.3 (SD) percent-hours of halothane oxygen
anesthesia
without surgery. Serum bilirubin,
alanine aminotransferase
, and aspartate aminotransferase significantly increased after
anesthesia
, which may indicate subclinical liver-cell damage. Creatine kinase of skeletal muscle origin increased above 90 U/liter in six subjects, indicating subclinical muscle-cell damage. Cortisol, triiodothyronine uptake, thyroxine, and free thyroxine index increased significantly immediately after
anesthesia
. Serum bromide concentrations had increased by fivefold on the second day after
anesthesia
, and on the ninth day was still elevated fourfold. Oral temperatures increased 0.7 degrees C 6 h post-
anesthesia
, possibly because of increased thyroxine activity. Lactate dehydrogenase, hydroxybutyrate dehydrogenase and gamma-glutamyltransferase activities did not change significantly. No drugs administered during the course of this study chemically interfered with any of the test methods used.

Liver tests were performed postoperatively on 25 women with "healthy" livers. Patients were divided into 3 groups according to the type of
anesthesia
used: 1. halothane; 2. neuroleptic; 3. peridural. In all 3 groups the variations in serum proteins and liver enzymes were similar up to the 15th postoperative day. Variations of LDH, AP, LAP, and CHE were within normal range; slight to moderate increases above normal were observed for
GPT
, GOT, GLDH, and gamma-GT. All 3 groups barely differ with respect to enzyme levels during the postoperative course. We therefore interpret these changes to indicate a reaction of the liver to the stress of operation rather than to the mode of
anesthesia
.

The influence of four different types of
anaesthesia
-- halothane and enflurane inhalation anaestheis, neurolept analgesia and epidural analgesia with single shot technique -- on the serum levels of GOT,
GPT
, GLDH, LDH, AP, LAP, gamma-GT, total protein and bilirubin was examined in 104 comparable cases undergoing gynaecological surgery. A significant increase in GOT,
GPT
, GLDH, AP, and gamma-GT levels was observed between the 6th and 9th postoperative day with all types of
anaesthesia
. The serum levels remained elevated until the 15th postoperative day. There was no significant difference between the four types of
anaesthesia
as regarded their effects on the serum constituents investigated. The same applied to the serum CHE activity which reached a minimum on the 3rd postoperative day followed by a steady rise. The results indicate that the postoperative changes of the liver enzyme pattern are not related to the type of
anaesthesia
used. Bilirubin was elevated on the first postoperative day, then dropped rapidly and stayed below the pre-operative values until the 12th postoperative day. There was no difference between the four types of
anaesthesia
. Glucuronisation of bilirubin may be inhibited initially by the anaesthetic agents. The subsequent rapid fall of the bilirubin levels may be the result of enzyme induction.

The influence of general
anaesthesia
for operations devoid of substantial tissue traumas on the postoperative pattern of hepatic enzymes was studied in 40 patients undergoing ophthalmologic surgery. 20 had neurolept analgesia, 20 had halothane
anaesthesia
. The duration of
anaesthesia
, age of the patients and their previous history corresponded fairly closely to those of a group of patients who had gynaecological operations and were the subject of a previous study. In contrast to the latter group total protein, cholinesterase, GOT,
GPT
, LDH, GLDH, AP, LAP and gamma-GT remained normal up to 12 days after the operation. The exception were changes in the total bilirubin levels which were similar to those observed in the gynaecological cases. Possible causes are discussed.

The methodology of a large prospective study on the influence of repeated anaesthetics on liver function is reported and the problems involved are discussed. The most suitable patients were those presenting for endoscopic examination of the bladder and urethra, for urethral dilatation and for cervical implantation of radium. Blood samples were taken immediately before induction of
anaesthesia
and on days 3-4 and 13-15 after operation, when a clinical assessment of the patient was also carried out. The concentrations of six enzymes (lactate dehydrogenase, alkaline phosphatase, aspartate aminotransferase,
alanine aminotransferase
, serum cholinesterase and gamma glutamyl transpeptidase) werechosen specifically as indices of liver function. The eosinophil count was measured to reflect any hypersensitivity reaction. The non-Gaussian distribution of these necessitated using appropriate non-parametric tests together with parametric tests on logarithmic transformed data. In addition a quantal method was used to measure the frequency of patients showing an "abnormal" increase in enzyme concentrations.

Blood sampling by catheterisation of the abdominal aorta to test plasma haemoglobin and various plasma enzyme activities was compared with two other blood sampling techniques and their application to farm-kept minks under general
anaesthesia
, cardiac puncture and tail-tip amputation. The plasma haemoglobin values in plasma obtained from the abdominal aorta were significantly lower than those recorded from the two other blood sampling techniques, which applied to plasma enzyme activities, as well. Those findings were equal for males and females. The plasma enzyme activities tested were glutamate oxalo-acetate transaminase, glutamate-
pyruvate transaminase
, lactate dehydrogenase, alpha-hydroxybutyrate dehydrogenase, and malic dehydrogenase. Some plasma enzyme actvities were tested from a random sample of 164 animals for sex-dependence (88 males and 76 non-pregnant unserved females), with no consideration being given to blood sampling techniques. The standards or physiological indices derived from those tests are reported.

Piritramide with chlorpromazine in doses of 3 mg kg-1 of live weight causes total
anesthesia
the intensity of which differs according to the breed and condition of the individual.
Anesthesia
sets in in 30 to 45 minutes and lasts for 3/4 to 1 1/2 hour. During the
anesthesia
somatic temperature and frequency of pulse decrease, and even after 24 hours they do not reach the original values. In a majority of cases breathing frequency rises insignificantly, and after 2 to 3 hours it returns to the normal. In single cases there occurs an extreme increase of the number of respirations. The per minute breath volume in the various cases and measurings differs considerably. In the course of
anesthesia
there occurs an increase of glucose, a decrease of the total protein, an increase of the activity of
GPT
and GOT, a decrease of the numbers of erythrocytes, of haematocrit and haemoglobin, an increase of pCO2 of the blood, and a decrease of the actual pH of the blood. In the number of leucocytes no changes occurred. The changes in the BE were insignificant. All deviations, except the deviations of the pulse and of the temperature, return to the original values within 24 hours. In the ECG there occurred a fluctuation in the forming of emotions and changes of the amplitude values of the QRS group and of the T wave. In the EEG striking periodical complexes of slow waves were found or combinations of slow and rapid frequencies.

1. Adult rats were subjected to a brief period of diethyl ether
anaesthesia
and were given diets with 200 or 100 g casein/kg with or without arginine plus glycine supplementation in the post-
anaesthesia
period. Nitrogen retention was measured as well as liver protein content and liver and muscle transaminase activities (L-aspartate aminotransferase (GOT), (EC 2.6.1.1), and
L-alanine aminotransferase
(
GPT
)(
EC 2.6.1.2
). 2. Results demonstrated that
anaesthesia
-stressed rats consuming the high-protein diet with supplemental arginine and glycine retained twice as much N as did rats given the diet with 200 g casein/kg alone, for the first 5 d post-
anaesthesia
. 3.
Anaesthesia
-stressed animals consuming the diets with 100 g casein/kg with or without arginine plus glycine supplementation did not differ from each other in N retention. 4. Liver protein content increased after
anaesthesia
in rats given the high-protein diets; liver transaminase activity increased, whereas muscle transaminase activity decreased, in animals consuming the high protein diets. 5. Possible mechanisms to account for these results are discussed.

Effects of 3 hours of methoxyflurane
anesthesia
in 20 dogs were determined by blood urea nitrogen (BUN), serum creatinine (SC), serum
alanine aminotransferase
(
ALT
), serum alkaline phosphatase (ALP). sulfobromphthalein (BSP), phenosulfonphthalein (PSP) clearance test, 24-hour water intake and urine excretion, and serum inorganic fluoride (SIF) evaluation. Values for BUN, SC, serum
ALT
, BSP, and PSP after the anesthetic were not significantly different (P less than 0.05) from the base-line values. The serum ALP values were significantly increased (P less than 0.001). Water intake and urine excretion showed a peak increase 48 hours after
anesthesia
. Serum inorganic fluoride concentration increased significantly (P less than 0.001) compared with the base line. The SIF 20 minutes before
anesthesia
was 4.54 +/- 0.82 mumol/L, at 90 minutes of surgical
anesthesia
92.35 +/- 8.91 micronmol/L, at 20 minutes after
anesthesia
132 +/- 12.55 micronmol/L, and at 1, 3, and 6 days after
anesthesia
they were 105.60 +/- 8.93, 42.10 +/- 6.90, and 12.65 +/- 1.32 micronmol/L. Clinical signs of renal or hepatic failure were not detected in any of the treated dogs during 7 day post-anesthetic observation period.

A 63 year old man underwent MCA aneurysmal neck clipping under O2-N2O-enflurane
anesthesia
. On the 46th postoperative day after the first operation, he had cranioplasty under O2-N2O-sevoflurane
anesthesia
. Hepatic injury occurred after the operation, and GOT,
GPT
and bilirubin increased above 700 IU.l-1, 800 IU.l-1 and 15.0 mg.dl-1 respectively but consciousness disturbance, hyperammonemia and DIC did not appear. His hepatic injury improved on conservative therapy. It seems that his hepatic injury was not caused by hepatitis viruses or hepatotoxicity of any drugs, but caused by cross sensitization between halogenated inhalation anesthetics, especially enflurane and sevoflurane, judging from drug induced lymphocyte stimulating test (DLST). We have to select an anesthetic method considering potential hepatic injury by halogenated anesthetics in a case of repeated
anesthesia
and operations during a short-term.
