In the period 1970 to 1987, 171 patients with small-intestinal mucosal atrophy have been hospitalized in our department. Of these, 132 patients fulfilled the diagnostic criteria of coeliac disease on the basis of histologic findings and clinical improvement on a gluten-free diet.

),
alanine aminotransferase
(ALAT), and alkaline phosphatase (ALP) were chosen as markers of hepatic involvement. Elevation above the normal range in one or more of these tests was seen in 62 patients (47.0%, group I). In 70 patients (53.0%, group II) of similar age the levels of these variables were normal. In group I, 14 (10.6%) patients had an elevation of ALP only, leaving 48 (36.4%) patients with pathologic values for one or both transaminases. In group I, 32 patients had their
ASAT
, ALAT, and ALP reexamined after at least 6 months of gluten-free diet. Among the patients with increased values of one or both transaminases 18 patients were tested before and at least 6 months after start of gluten-free diet. The variables were significantly reduced in all patients. Liver biopsies were performed in 37 patients, and findings were normal in 5. In 25 patients the changes were classified as non-specific. Chronic active hepatitis was demonstrated in five patients. In one of these patients primary sclerosing cholangitis and ulcerative colitis were also diagnosed. Concomitant malignant disease was found in 22 patients, of whom 16 had malignant lymphoma. Malignant disease was seen more often in group I than group II (p less than 0.01). , gamma-glutamyltransferase, and alkaline phosphatase activities in the blood serum of women taking the oral contraceptive preparation Microgynon through extended periods were raised; the activity of cholinesterase was simultaneously reduced. In rats liver homogenates ethynylestradiol, one of the active components of Microgynon, acted as an inducer of gamma-glutamyltransferase and alkaline phosphatase while leaving aspartate aminotransferase and

unaffected, but reduced the level of cholinesterase. Norgestrel, the other active component of the preparation, suppressed the biosynthesis of gamma-glutamyltransferase and alkaline phosphatase while leaving aspartate aminotransferase,
alanine aminotransferase
and cholinesterase levels unaffected. A mixture of ethynylestradiol plus norgestrel in the mass proportion occurring in Microgynon produced the same effects upon gamma-glutamyltransferase and alkaline phosphatase as ethynylestradiol alone. Estradiol, the parent hormone of ethynylestradiol, lacked the inducing capability of the latter while ethynylpropargyl chloride induced gamma-glutamyltransferase and alkaline phosphatase so it was concluded the inducing effect of ethynylestradiol must be ascribed to the ethynyl radical. Progesterone, the parent of norgestrel, shared the latter's suppressive activity for gamma-glutamyltransferase and alkaline phosphatase biosynthesis, and behaved like its derivative towards the other enzymes.

A detailed biochemical analysis has shown that during larval development on artificial medium, the amounts of K+, Na+, and Ca2+ in the hemolymph of healthy Choristoneura fumiferana varied from 85 to 110 mg/100 mL, 29 to 33 mg/100 mL, and 4.8 to 7.3 mg/100 mL, respectively. Similar results were obtained with Malacosoma disstria. Intoxication by Bacillus thuringiensis "H3a,3b" (B. t.) considerably modified the amounts of the cations. Thus, after 4 h, the quantity of K+ in M. disstria increased from 99 to 229 mg/100 mL and Na+ from 26.5 to 50.3 mg/100 mL while that of Ca2+ decreased from 5.8 to 1.2 mg/100 mL. Similar results were observed with C. fumiferana, but these variations occurred after 2 to 4 days of B. t. intoxication. The variations detected during the bacillosis, with respect to the cationic composition of the insect hemolymph, are rapidly detectable, well before light microscope observation can confirm the presence of this intoxication.

These results suggest that it is possible to diagnose biochemically the presence of a B. t. intoxication in lepidopteran forest pests following treatments by this biological insecticide for their control.

, alkaline phosphatase, and bilirubin levels were elevated in 44 (53%), 39 (47%), 45 (54%), and 5 (6%) of the patients on the first determination (2.0 +/- 0.1 days after onset of fever) and in 11 (13%), 17 (20%), 26 (31%), and 1 (1%) on the second determination (5.4 +/- 0.2 days after onset of fever), respectively. The elevation rarely exceeded three times the upper limit of normal. One patient had severe jaundice. An abnormality of at least one of these values was found in 55 patients (65%). There were no differences in site of infection, bacteria isolated, and outcome between patients with and without biochemical abnormalities. We conclude that in adult patients with bacteremia, elevation of liver enzymes and bilirubin is common, usually mild, of short duration, and of no prognostic significance.

Forty six children suffering from Protein Energy Malnutrition (PEM) were classified according to the Wellcome classification. Their aspartate aminotransferase,

and gamma-glutamyl transferase were measured.

was raised in 20 patients (43.5%) and
alanine aminotransferase
was raised in 12 patients (26%). Y-glutamyl transferase was raised in only one patient suffering from marasmic kwashiorkor, who, in contrast to the rest of the patients had a marked rise in aminotransferases. The aminotransferase elevation correlated positively with a Severity Index calculated from height and weight retardation and serum albumin levels. It is suggested that the moderate rise in aminotransferases found in PEM is not due to damage to the liver. However, marked enzyme elevations can occur in a small minority of patients, suggestive of liver injury, probably caused by hepatotoxins.

Sequential serum levels of carbohydrate-deficient transferrin (CDT) were determined in 72 alcoholics at various intervals during detoxification. Before treatment, 57 patients (79%) had increased CDT values (Group A), whereas in 15 individuals (21%) (Group B), CDT levels were within the normal range. In 51 Group A patients, CDT decreased progressively after cessation of alcohol intake (half-life, 16 +/- 5 days), but fluctuated and remained abnormal in the remaining six. Nine Group B patients maintained normal CDT values throughout the follow-up period, but slightly or moderately increased levels were recorded on one occasion in the other six Group B subjects. Patients whose CDT levels had reached normal values after treatment, showed a recurrent increase in CDT after a relapse. gamma-Glutamyl transferase activities, which were elevated in 56% of Group A and in 80% of Group B alcoholics, showed a decrease after cessation of alcohol consumption in most patients with initially elevated values (Group A, 30 of 32; Group B, 10 of 12).

) activities, as well as mean corpuscular volumes (MCV) were normal in the majority of patients. CDT/total transferrin ratios correlated positively with CDT levels. CDT proved to be the most sensitive marker for chronic alcoholism (79%), whereas GGT activity levels were more useful only in patients with normal CDT levels before alcohol withdrawal. In the assessment of treatment outcome, the combination of CDT and GGT as markers yielded a sensitivity of 95%.

Data on 15 laboratory analytes obtained in 145 prospectively investigated cholestatic patients with viral hepatitis, chronic intrahepatic cholestasis and extrahepatic biliary obstruction were submitted to a computer-based graphical evaluation using probabilistic test analysis. This revealed a marginal utility for alkaline phosphatase, gamma-glutamyltransferase and the direct/total bilirubin ratio at specific cut-off points for the exclusion of extrahepatic cholestasis (PVneg 90%-100%).

ratio above 1.5 and serum gamma-globulin concentrations above 20 g/l strongly suggested cholestasis due to chronic parenchymal liver disease (PVpos 92% and 90%, respectively). This graphical approach to laboratory data analysis enhances the understanding of the interrelations between cut-off points and sensitivity, specificity and predictive values and also of the influence of disease prevalence on disease prediction. It also adds to present knowledge by demonstrating the clinical relevance of several readily available, albeit rarely utilized diagnostic analytes.

(1) The acute effects of ethanol on protein synthesis by liver and skeletal muscle were investigated in young (95-100 g) rats. Rats were injected intraperitoneally with ethanol, 75 mmol/kg body wt; controls were injected with isovolumetric 0.15 M NaCl. After 140 min rates of protein synthesis were measured by injection of a large dose of L[4(3)H]phenylalanine and at 150 min rats were killed. (2) Fractional rates of protein synthesis in control animals were approximately four to five times greater in liver than muscle. Absolute rates were, however, comparable in liver and skeletal muscle. Ethanol reduced the fractional rate of liver protein synthesis by 5-20%; the response for muscle was relatively greater (25-30%). The decrease in the amount of protein synthesized by muscle was also greater than that by liver. (3) After 150 min, plasma gamma-glutamyl transferase,
alanine aminotransferase
, alkaline phosphatase, lactate dehydrogenase and creatine kinase activities were all decreased by 25-60%.

activity was increased by 42%, though this was not statistically significant. (4) Increased plasma glucose and triglycerides in ethanol-dosed rats indicated that limitations in substrate supply were not mediating factors in reducing protein synthesis. Ethanol was also able to exert its effects in the presence of elevated insulin levels. A direct effect of ethanol, or its metabolites, on protein synthesis, is therefore implied.

activity, which may be detected on routine blood chemistry analysis and erroneously considered to indicate the presence of liver disease. Clinicians should be aware of this phenomenon so patients are not subjected to unnecessary procedures. In patients with unexplained
AST
elevation, liver and muscle disease can be biochemically excluded by the finding of normal serum levels of
alanine aminotransferase
and creatine kinase. The presence of macro-

can be determined by exclusion chromatography, electrophoresis, and activation assays with pyridoxal 5-phosphate. The elevated
