Two projective techniques were employed to explore the psychological characteristics of teenage mothers who were found, in a previous study, to have experienced more traumatic childhoods than the nonmother control group. Part I, presented here, investigated ego development using the Loevinger Sentence Completion Test. (Part II, responses to a set of TAT cards, will appear in a subsequent issue of Adolescence.) In comparison with the nonmothers, teenage mothers demonstrated more mature ego development. This is in distinct contrast with most reports in the literature, and the ramifications of these results are discussed. In addition, content analyses of the responses to the Sentence Completion Test confirm the highly involved mother-daughter and negative father-daughter relationships found in the previous study with this sample.

Human immunodeficiency virus type 1 (HIV-1) spends a significant part of its life cycle as latent provirus in nonactivated cells. It induction requires mitogen stimulation. TPA treatment induces HIV-1 transcription by protein kinase C (PKC)-mediated activation of the cellular transcription factor NF-kB. PKC activation induces the dissociation of NF-kB from its inhibitor protein (IkB). The liberated NF-kB then binds to its proviral recognition sequence in the HIV-1 long terminal repeat (LTR) sequence. This step, however, is not sufficient to augment transcription. We demonstrate that NF-kB-mediated HIV-1 LTR activation is regulated by an additional event that is not dependent on IkB. A further phosphorylation event is proposed, since this step could be blocked by an inhibitor of a phospholipase C (PLC) type reaction. This inhibitor precludes the formation of diacylglycerols, which are required for activation of PKC isoenzymes. As an alternative pathway that is not dependent on PLC reactions, high-level transcription from the HIV-1 LTR is shown to require binding of both NF-kB and TAT.

Anticoagulant activity of ionic and nonionic contrast media (CM) was investigated in vitro and in vivo. Based on the time course of FPA and TAT generations and gross examinations of the blood clots on the catheters placed in CM-blood mixtures [an 2 to 8 ratio (20% v/v)]. It was demonstrated that blood coagulation was activated during the period of 20 to 30 minutes when nonionic CM (iopamidol, iohexol) was employed, but no activation of blood coagulation was noted with ionic CM (diatrizoate, ioxaglate). Scanning electron microscopic examinations of the clots on the catheters revealed that fine fibrin meshwork fibers, in which many red blood cells were trapped in bound, were observed with nonionic CM. In contrast, no fibrin mesh was formed with ionic CM after 30 minutes. In vivo, antithrombin III and fibrinogen significantly decreased in the patients who underwent infusion of nonionic CM. Our studies confirmed that nonionic CM show weaker anticoagulant activity than do ionic CM. And these findings account for previous reported thromboembolic complications with the use of nonionic CM. Extreme caution should be therefore exercised when nonionic CM are employed during prolonged angiographic and interventional procedures.

We present a 65-year-old female with myelodysplastic syndrome (MDS) who has attended our O.P.D. since 1983. In early December, 1990, dyspnea on effort developed which then progressed to dyspnea at rest at the end of December. She was admitted on January 8 with orthopnea. Chest X-ray films revealed loss of vascular shadows of the right lung. Blood gas analysis showed hypoxemia and hypocapnemia. Abnormalities in the coagulation-fibrinolytic system (increased TAT (thrombin-anti-thrombin III complex) and alpha 2-PIC (plasmin inhibitor complex)), possibly due to MDS, were detected. The diagnosis of pulmonary thromboembolism was made by pulmonary perfusion scintigram and pulmonary arteriography. After commencement of anticoagulation therapy on January 15, the subjective symptoms, blood gas analysis, pulmonary scintigram, and disorders of the coagulation-fibrinolytic system improved. The patient was discharged on March 5, 1991. The present case of myelodysplastic syndrome was associated with abnormalities of the coagulation-fibrinolytic system and was complicated by pulmonary thromboembolism.

A validation study was performed on the MESAM 4, a digital recording device developed to monitor oxygen saturation, heart rate (HR), snoring, and body position in order to screen subjects for obstructive sleep apnea syndrome (OSAS). MESAM 4 recordings were scored with the computer-based automatic scoring system provided with the equipment. Nocturnal polysomnography (PSG) and MESAM 4 recordings were run simultaneously on 56 subjects presenting with any type of sleep complaint, including those secondary to OSAS. Patients were assigned to one room by hospital administration and were monitored consecutively. The polygraphic equipment and MESAM 4 equipment were placed on the subjects by separate teams. Records of PSG and MESAM 4 were analyzed in double-blind fashion. With the MESAM 4 computerized analysis, three indices based on SaO2 (ODI), on heart rate (HVI), and on snoring (ISI) were obtained, and the number of abnormal respiratory events occurring during the time selected for analysis (TAT) were determined. Polysomnographic records were scored by 30-s epochs following the American Sleep Disorders Association standards for sleep states and stages and for sleep-related events, including sleep apneas, hypopneas, and periodic leg movements. Following independent scoring, 26 subjects were identified with OSAS by PSG, while MESAM 4 identified 25 subjects with OSAS using oxygen algorithm; all had a respiratory disturbance index greater than or equal to 10 with PSG. Results of each polysomnogram and each MESAM 4 analysis were compared. With the polysomnogram used as a standard, the degree of error for each variable with the MESAM 4 was calculated. Specificity and sensitivity of the most accurate index of the MESAM 4, the ODI, were 97 percent and 92 percent, respectively. The other two indices, HVI and ISI, were less accurate: specificity and sensitivity were 32 percent and 58 percent for HVI and 27 percent and 96 percent for ISI. Nevertheless, a combination of all three indices (ODI, HVI, ISI) would have prevented the two false-positive cases we observed. The results of this validation study show that MESAM 4 can be helpful to general practitioners, clinicians, and epidemiologists as a low-cost screening device for subjects with OSAS and habitual snoring.

23 boys and 13 girls, aged 13 to 17 yr., from two homes for delinquents were given the Thematic Apperception Test. Girls were also given the Rorschach. The 1960 Hafner and Kaplan TAT and Rorschach hostility indices were related to the 1977 Luukkonen indices (aggression-inward, aggression-outward, fantasy-aggression, oral, anal and phallic content), to the Pruitt and Spilka empathy index, and to the Neiger reality index. The TAT hostility index for boys was significantly higher than that for girls; boys expressed more physical and girls more verbal hostility in TAT stories. On the Rorschach, aggression outward and inward correlated significantly with Rorschach hostility, phallic and oral themes.

It has been suggested that unstable angina at rest, like acute myocardial infarction, might be associated with a thrombotic process. In order to study the hypothesis that myocardial ischemia during exercise could also be associated with an activation of blood coagulation and/or fibrinolysis, we investigated the presence of plasma markers of a prethrombotic or thrombotic state (thrombin-antithrombin III complexes TAT, prothrombin fragment F1 + 2, and D-dimers DD) in 100 consecutive patients with confirmed or suspected coronary artery disease during ergometric test with myocardial thallium-201 scintigraphy. Symptoms and scintigrams allowed to define three groups of patients: those showing no ischemia (n = 79) and those with symptomatic (n = 8) or silent myocardial ischemia (n = 13). Before exercise, DD and TAT levels were not significantly different among the three groups. On the other hand, the F1 + 2 levels were slightly albeit significantly higher in the patients without ischemia than in the patients with symptomatic or silent ischemia. After exercise, no significant difference was found between the three groups. Exercise induced a significant and parallel increase in both the TAT and the F1 + 2 levels (but not of the DD levels) in the three groups. Thus, our study does not support the hypothesis that myocardial ischemia, silent or symptomatic, is associated with an activation of plasma coagulation and fibrinolysis that can be distinguished from the exercise-induced thrombin generation.

A generalized deficiency of the mitochondrial matrix enzyme ornithine aminotransferase (OAT) is the inborn error in gyrate atrophy (GA), an autosomal recessive degenerative disease of the retina and choroid of the eye. Mutations in the OAT gene show a high degree of molecular heterogeneity in GA, reflecting the genetic heterogeneity in this disease. Using the combined techniques of PCR, denaturing gradient gel electrophoresis, and direct sequencing, we have identified three nonsense-codon mutations and one nonsense codon-generating mutation of the OAT gene in GA pedigrees. Three of them are single-base substitutions, and one is a 2-bp deletion resulting in a reading frameshift. A nonsense codon created at position 79 (TGA) by a frameshift and nonsense mutations at codons 209 (TAT----TAA) and 299 (TAC----TAG) result in abnormally low levels of OAT mRNA in the patient's skin fibroblasts. A nonsense mutation at codon 426 (CGA----TGA) in the last exon, however, has little effect on the mRNA level. Thus, the mRNA level can be reduced by nonsense-codon mutations, but the position of the mutation may be important, with earlier premature-translation termination having a greater effect than a later mutation.

Studies on the binding specificity of actinomycin D (ACTD) to tetranucleotide sequences of the form -XGCY- have been extended to include the non-self-complementary sequences. ACTD binding characteristics are investigated by equilibrium, kinetic, and thermal denaturation for decameric duplexes d(ATA-XGCY-ATA)-d(TAT-Y'GCX'-TAT), where X and Y are complementary to X' and Y', respectively, but not to each other. The results indicate that when X = G or Y = C, the oligomers exhibit significantly weaker ACTD binding affinities, smaller melting temperature increases upon drug binding, and faster SDS-induced ACTD dissociation rates than the other sequences. Estimated binding constants at 18.5 degrees C for decameric duplexes containing -AGCA-/-TGCT-, -AGCG-/-CGCT-, or -CGCA-/-TGCG- are in the range of 4-9 microM-1, whereas for the ones containing -GGCT-/-AGCC-, -GGCA-/-TGCC-, or -GGCG-/-CGCC- they range from 0.6 to 2 microM-1. In contrast to the characteristic SDS-induced ACTD dissociation times of 600-1000 s for the stronger binding sites, the sequences containing X = G or Y = C exhibit at least an order of magnitude faster dissociation kinetics. These observations are further supported by the induced CD results and fluorescence measurements with 7-amino-ACTD. The findings from these non-self-complementary -XGCY- tetranucleotide sequences are consistent with those found earlier for the self-complementary counterparts, and they together clearly demonstrate that a base sequence alteration adjacent to the GC site can have a profound effect on the ACTD binding as well as dissociation characteristics, likely a consequence of subtle conformational alterations near the binding site.(ABSTRACT TRUNCATED AT 250 WORDS)

In 86 neurotic patients, judgements about TAT pictures were defined before a session of hypnosis. Later other judgments about the same pictures were suggested after the patients entered the hypnotic state. 34 patients who were in a deep enough hypnotic state manifested the lack of suggestion realization, which can be accounted for by image and intellectual activity that occurred at the moment of suggestion. Suggestibility increased whatever the depth of the hypnotic state, on condition that at the moment of suggestion the image and intellectual perception of TAT pictures could be rearranged. The given study made it possible to explain the occasionally occurring difference in suggestibility and hypnability which is of practical value.
