Fever after bone marrow transplantation may indicate the onset of bacterial or opportunistic infection, or acute graft-versus-host disease (GVHD). In an attempt to differentiate between infection and GVHD, we prospectively studied 41 bone marrow transplants in 38 patients (24 allogeneic, 17 autologous). Elevation of
C-reactive protein
(
CRP
) proved to be a good indicator of disseminated infections. In 40 episodes of documented (11) or presumed (29) sepsis,
CRP
rose above 5 mg/dl in 38 episodes (95%), and above 10 mg/dl in 32 episodes (80%). The
CRP
concentration paralleled the clinical course of the infectious episodes. Elevated
CRP
values were not observed in the 15 episodes of acute GVHD without concurrent infection. High peak values of serum total IgE, ranging from 4-fold to over 4000-fold baseline, were observed posttransplant in 18/22 allogeneic
BMT
recipients, temporally associated with activation of acute GVHD. IgE was elevated neither in episodes of sepsis without concurrent GVHD, nor in viral or focal bacterial infections. In general, septic infections were characterized by high
CRP
but low IgE levels. Acute GVHD without concurrent infection was characterized by high IgE but low
CRP
. We conclude that
CRP
and serum total IgE utilized together in serial fashion are helpful in distinguishing sepsis from acute GVHD.

Daily serum samples collected during 6 autologous and 13 allogeneic
BMT
were assayed retrospectively for tumour necrosis factor-alpha (TNF), interleukin-6 (IL-6) and
C-reactive protein
(
CRP
). In addition, for 6 allogeneic transplant patients soluble TNF receptor (sTNFR) levels were determined. IL-6 levels were regularly raised during febrile episodes and closely mirrored changes in serum
CRP
but were not predictive for non-infectious major transplant-related complications (TRC). Levels of TNF showed no such close association with infection and in contrast to previously reported data for allogeneic transplants having TRC, TNF levels were consistently detectable in only 3 of 9 patients. Pre-transplant levels were not predictive for the development of TRC and no profile was recognized to be specific for a particular complication. In transplants with only minor complications TNF levels remained consistently undetectable. sTNFR levels increased in a more stable manner in association with TRC, suggesting that they may be a more suitable marker to monitor major TRC.

IL-10 plays an important role in the control of immune reactions during systemic infection. Here, IL-10 serum levels were investigated in patients after
BMT
. The IL-10 levels correlated with the clinical course of the patients and with serum levels of
C-reactive protein
(
CRP
) and neopterin (NP). A total of 26 patients with AML (7), ALL (12), CML (2), NHL (3) and multifocal Ewing's sarcoma (2) had received autologous (10) or allogeneic (16)
BMT
from related (9) or unrelated donors (7). Routine serum samples were obtained prior to
BMT
and at days 46 and 100 after
BMT
. However, in patients with severe complications additional samples were drawn at individual points in time. Prior to
BMT
, IL-10 serum levels were not detectable in 24/24 patients. Post-
BMT
, 11 patients developed elevated IL-10 levels, of these eight died of complications (DOC), whereas only one of 15 patients with undetectable IL-10 died of complications, indicating that high IL-10 levels were significantly correlated with severe life-threatening complications (chi2, P < 0.01). To determine the pathomechanism and role of the increased IL-10 levels, they were correlated to the respective NP and
CRP
serum concentrations.
CRP
and NP concentrations were found significantly elevated in patients with detectable IL-10, indicating a severe acute phase reaction associated with macrophage activation. In conclusion, high IL-10 serum levels in patients after
BMT
were significantly associated with fatal outcome. Since IL-10 is a strong suppressor of T cell immunity, high IL-10 production in patients with severe complications such as septic shock or GVHD > grade II after
BMT
might lead to functional immunodeficiency contributing to the poor prognosis of these patients.

Prognostic scores, such as the PRISM and APACHE II, have been established, predicting with reasonable accuracy the outcome of patients admitted to intensive care units (ICU). In keeping with previous reports, we found, however, that these scores failed to perform in a series of 28 recipients of hematopoietic auto- or allografts (
BMT
) who required ICU admission for reasons including respiratory (82%) and multi-organ (36%) failure. We therefore retrospectively analyzed the charts of these patients, evaluating predisposing factors and prognostic variables which might confound the validity of these ICU tools which in other clinical scenarios have proven so valuable. Of all the parameters tested, logistic analysis established the following as predictors for poor outcome: increased
C-reactive protein
(
CRP
) to > 10 mg/dl (P = 0.04), macroscopic hemorrhage (P = 0.04), hypotension (mean arterial pressure < normal) (P = 0.04) and GVHD > or = III (P = 0.002). Most of these factors are not accounted for by the standard prognostic questionnaires. The development of an 'oncological' or 'post-
BMT
' risk of mortality score, taking into account these patients' specific clinical problems, might improve the risk assessment for this patient group, and might thus facilitate the timely recognition of those patients most in need of more intensive therapeutic measures.

We monitored levels of

) in 96 consecutive adult allogeneic
BMT
patients (age 15-50 years) transplanted in our unit. Major transplant-related complications (MTC) occurred in 32% of cases and included: hepatic veno-occlusive disease, pneumonitis, severe endothelial leakage syndrome and >II acute GVHD. Transplant-related mortality (TRM) before day 100 post-
BMT
was 13.5%. Variables included in a stepwise logistic regression model were: gender, age, disease category, donor type, T cell depletion, TBI, use of growth factors, bacteremia, mean
CRP
-levels >50 mg/l between days 0 and 5 (
CRP
day 0-5) and >100 mg/l between days 6 and 10 (
CRP
day 6-10) post-
BMT
. Only high
CRP
-levels (for MTC and TRM) (P < 0.001) and donor-type (for TRM) (P= 0.02) were independent risk factors. The estimated probability for MTC was 73% (
CRP
day 6-10 >100 mg/l) vs 17% (
CRP
day 6-10 <100 mg/l). Using the same cut-off levels, the probabilities for TRM were 36.5% vs 1% in the identical sibling donor situation and 88% vs 12.5% in other donor-type transplants. We conclude that the degree of systemic inflammation, as reflected by
CRP
-levels, during the first 5-10 days after
BMT
identifies patients at risk of MTC and TRM. Our data may be useful in selecting patients for clinical trials involving pre-emptive anti-inflammatory treatment.

We prospectively evaluated the capacity of serum procalcitonin (PCT), compared with serum levels of
C-reactive protein
(
CRP
) and endotoxin, to identify children at high risk for mortality from sepsis after
BMT
. Of 47 pediatric bone marrow transplantation patients studied, 22 had an uneventful course post-transplant (Group 1), 17 survived at least one septic event (Group 2), and eight died from multiorgan failure (MOF) following septic shock (Group 3). Median concentrations of PCT over the course of the study were 1.3, 15.2, and 102.8 ng/ml, respectively, in each of the three groups (P<0.002 for each comparison). Median concentrations of
CRP
were 91, 213, and 260 mg/l, respectively (P<0.001 for Group 1 vs Group 2 and Group 3; P=NS for Group 2 vs Group 3). Median concentrations of endotoxin were 0.21, 0.30, and 0.93 U/l, respectively (P=NS for each comparison). Median concentrations of PCT, in contrast to serum
CRP
and endotoxin, correlated with the severity of sepsis (8.2 ng/ml in 'sepsis' and 22.3 ng/ml in 'severe sepsis', P=0.028) and provided useful prognostic information during septic episodes.

Gram-negative infection is an important cause of morbidity and mortality after unrelated donor-bone marrow transplantation (UD-BMT). We performed a retrospective case-control study to examine the risk factors, prophylaxis, therapy and outcome of Gram-negative bacteraemia (GNB) in 428 patients undergoing UD-
BMT
. The incidence of GNB was 3.6% in children and 19% in adults. Of the adults, 11% developed GNB >60 days post UD-
BMT
. Predisposing risk factors for GNB included 'high-risk' disease status, chronic graft-versus-host disease and use of systemic steroids. Fever, a raised
C-reactive protein
(
CRP
) and hypotension were common findings at presentation. Patients were routinely given prophylactic ciprofloxacin: resistance to this antibiotic was seen in 33% of isolates. We identified an age-matched control group undergoing UD-
BMT
over the same time period as the study group. Gram-positive bacteraemia was significantly more common in cases than controls. Mortality from GNB was 17% in children and 24% in adults. We conclude that GNB is a common complication of UD-
BMT
with a high associated mortality. Patients should be educated further to present rapidly with symptoms suggestive of infection.
