Salivary function,
dental caries
and caries-associated salivary microorganisms were investigated in children undergoing
BMT
during a 4-year longitudinal study. Fourteen children were conditioned with CY and TBI and 12 with CY with or without BU. Four years after
BMT
the mean salivary secretion rate was 1.3 +/- 0.7 ml/min in the chemotherapy group, compared to 0.7 +/- 0.5 in the TBI/CY group (P < 0.05). The mean salivary secretion rate fell from 0.9 +/- 0.5 ml/min before TBI to 0.2 +/- 0.1 after 3 months (P < 0.01), 0.3 +/- 0.3 ml/min after 6 months (P < 0.01) and 0.5 +/- 0.6, 1 year after TBI (P < 0.05). Mean reduction in stimulated salivary flow 3 months after TBI was 78% in the TBI/CY group compared to 36% in the chemotherapy group (P < 0.05). Children conditioned with chemotherapy showed an increased salivary flow compared to baseline; this was not found in TBI-treated children, suggesting that damage to the salivary glands may be permanent. Four years after
BMT
, children conditioned with TBI had significantly higher counts of mutans streptococci (P < 0.05) and lactobacilli (P < 0.01) compared to age-matched controls. However, the prevalence of
dental caries
did not differ between children conditioned with TBI, chemotherapy and healthy controls.

Minimal data about oral and dental health in long-term survivors after
BMT
are available. We studied the dental status of 27 children (19 males, eight females) with leukaemia, followed up with a routine oral examination, panoramic tomogram and, when necessary, an endoral radiograph at a median of 2 years (range 1-10) after
BMT
. Community periodontal index treatment necessity (CPITN),
dental caries
, missing or filled permanent teeth (DMFT) and dento-facial alterations according to WHO criteria were registered and evaluated. Median age of the patients at
BMT
was 9 years (range 1.1-17.9). The mean DMFT score ranged from 1.6 to 12.4 according to age at examination and was slightly higher than that which we previously reported in children who received chemotherapy alone. CPITN showed the presence of soft deposits in 77.7%, serious gingivitis in 59.2% and parodontal involvement in 3.7% of cases. Dento-facial abnormalities were found in 55.5% of patients, while 62.9% of the patients had tooth abnormalities or agenesis. Nine out of 27 patients (33%) had root hypoplasia. factors was found. Age is the crucial factor in determining a developmental defect of enamel and root. The follow-up of long-term survivors after
BMT
should include regular dental examination.

Fanconi's anemia (FA) is an autosomal-recessive disorder characterized by a progressive pancytopenia, diverse congenital abnormalities and increased predisposition to malignancy. Sixteen children with FA, aged between 4 to 16 were divided into two groups according to treatments. Nine children had bone marrow transplantation and seven children were treated with steroid and/or anapolan. The changes in
dental caries
, caries-associated microflora, salivary status and periodontal health were investigated in children with FA. Data were analyzed by one-way ANOVA. A statistically significant difference was found in hematological findings between children who have received bone marrow transplantation (BMT+) and the others, who have not received (
BMT
-). There was no significant difference in
dental caries
experience, salivary flow rate, buffering capacity, mutans streptococci and Lactobacilli levels between the study groups. A statistically significant difference was found in gingival index, plaque index, bleeding on probing, probing depth scores between the patients with FA in
BMT
(+) and
BMT
(-) groups (p<.05). In conclusion, besides systemic control, additional preventive measures during their whole life to maintain oral health is necessary in these children.
