The present study was performed in order to obtain the
thiopurine methyltransferase
(
TPMT
) activity frequency distribution histogram in a Spanish population. A total of 3640 Spanish clinical laboratory samples were evaluated, which included 1249 patients with Crohn's disease, 589 with ulcerative colitis, 348 with
multiple sclerosis
(MS), 487 with several autoimmune diseases different from the above-mentioned diseases and 967 a donor group. We have measured the
TPMT
activity in red blood cells (RBCs) by a radiochemical method, using S-adenosyl-L-[methyl-3H]methionine as methyl donor. The different groups present in their entirety a normal distribution histogram and a wide range of
TPMT
activity from 0 to 41 U/ml RBCs. The differences found between the Spanish population
TPMT
activity frequency distribution histogram and the pattern previously described in a North American population were not due to azathioprine treatment or gender. The effect of autoimmune diseases on
TPMT
activity was evaluated: the enzymatic activity was similar in the donor group (19.9 +/- 6.3 U/ml RBCs) and in the patients with Crohn's disease (20.0 +/- 5.8 U/ml RBCs) and ulcerative colitis (19.7 +/- 6.1 U/ml RBCs); however, it decreased significantly (p<0.0001) in MS patients (17.1 +/- 6.1 U/ml RBCs) with respect to the donor group. In conclusion, our results show that the Spanish population
TPMT
distribution is closer to that of the Jewish population of Israel than to North American populations, and that in MS the enzymatic activity of
TPMT
decreases significantly. This observation may take into account the usage of azathioprine as therapeutic agent in Spanish MS patients.

We describe two
multiple sclerosis
patients who developed pancytopenia following treatment with azathioprine. They were found to have the homozygous polymorphism for
thiopurine methyltransferase
deficiency and recovered after cessation of drug therapy. We review the literature concerning this molecular derangement and underscore the importance of performing surveillance testing for allelic characterization prior to treatment intervention with this agent for immune-mediated disorders.

We sought to assess the activity of
thiopurine methyltransferase
(
TPMT
) in 14,545 Spanish patients with different diseases amenable to treatment with azathioprine/6-mercaptopurine (6-MP), and to evaluate the proportion of patients with low
TPMT
activity and therefore a higher risk of myelotoxicity with these drugs.
TPMT
activity in red blood cells (RBCs) was measured by a radiochemical method. The association between several clinical variables and
TPMT
activity was assessed by multiple linear regression. We included 14,545 patients: autoimmune hepatitis (n=359 patients), inflammatory bowel disease (n=7,046),
multiple sclerosis
(n = 814), myasthenia gravis (n=344), pemphigus (n=133), and other diseases (n=5,849). Mean
TPMT
activity was 20.1 +/- 6 U/mL, but differed depending on the disease (P<.001).
TPMT
distribution was low (<5) in 0.5%; intermediate (5.0-13.7) in 11.9%; or high (>or=13.8) in 87.6%. Only when
TPMT
activity was considered separately in each disease did it reveal a normal distribution. In the multivariate analysis, gender, hematocrit, and treatment with 5-aminosalicylates/steroids/azathioprine/6-MP statistically influenced
TPMT
activity, although, probably, in a clinically irrelevant manner. This study shows, in a large sample of 14,545 patients, that 0.5% had low
TPMT
activity, indicating a higher risk of myelotoxicity with azathioprine/6-MP, a figure similar or slightly higher than that reported in other areas. Nevertheless, the trimodal distribution of
TPMT
activity varied depending on disease, and the proportion of patients with low activity values ranged from 0-0.8%. The drugs prescribed for the treatment of autoimmune diseases, including azathioprine/6-MP, modified
TPMT
activity, but the magnitude of this effect was very small and the differences found are probably irrelevant from the clinical point of view.
