When 54MnCl2 was incubated with fresh bovine or caprine serum for 20 h and the serum subjected to electrophoresis at pH 9.5, the 54Mn bound to transferrin and alpha2-macroglobulin in proportions which varied with the temperature of incubation and the temperature of electrophoresis. Between 0 and 37 degrees C, the higher the temperature of incubation the larger the proportion bound to transferrin and the lower the proportion bound to alpha2-macroglobulin. The temperature at which electrophoresis was performed had little effect on the proportion of 54Mn bound to transferrin, but increasing temperature reduced the proportion of 54Mn bound to alpha2-macroglobulin. Mn2+ did not bind to purified transferrin in vitro in the absence of an oxidising agent. In the presence of permanganate, Mn3+ was formed and chelated by transferrin at physiological pH. In fresh serum this oxidation step may be performed by ceruloplasmin or molecular oxygen. Mn2+ was bound reversibly to alpha2-macroglobulin but this protein played no part in the oxidation of divalent manganese and had no effect on the protein binding of trivalent manganese. Manganese in the divalent state, either free as Mn2+ or bound to alpha2-macroglobulin, is removed from blood plasma very efficiently by the liver. However, the manganic-transferrin complex normally found in circulation is not rapidly removed from plasma. The liver can remove large amounts of excess manganous manganese which it presumably excretes; the small essential fraction of the manganese absorbed is oxidised to the trivalent state and bound to transferrin.

Five glycoproteins have been measured in the blood of 145 diabetic patients with and without clinical evidence of complications. Patients with diabetic complications have higher glycoproteins levels particularly when expressed as a ratio to serum albumin levels. In 32 pairs of patients matched for age, sex, body weight, duration and treatment of diabetes, significantly higher haptoglobin, fibrinogen and caeruloplasmin levels were associated with the presence of diabetic complications, but blood glucose levels were not significantly different. Beta-lipoprotein levels were positively correlated with age and alpha2-macroglobulin levels with the duration of clinical disease, but the type of antidiabetic therapy administered did not significantly alter glycoprotein levels. It is suggested that rising levels of certain glycoproteins in the blood of diabetic patients may indicate the development of diabetic vascular complications, but a prospective study is required before it can be decided whether this change predates the clinical appearance of the complications.

A new case of analbuminemia is described for a six month old child of Algerian origin. The serum albumin concentration was 64 mg/l and its immunochemical action was identical to that of normal albumin. The system reacted by an increase of the synthesis of globulins. For the subject, the alpha1-antitrypsin, ceruloplasmin, haptoglobin, alpha2-macroglobulin, transferrin and immunoglobulins M contents were three times higher than the standard figures. However, it was possible to show that the presence of free bilirubin independent from proteins could be detected at a concentration of 17 mumol/l.

A linear correlation was found between the serum/cerebrospinal fluid (CSF) concentration ratios of albumin, caeruloplasmin and alpha2-macroglobulin and their hydrodynamic radii in a semilogarithmic plot. This protein gradient is used as a parameter to evaluate the blood-CSF barrier under normal and pathological conditions. Irrespective of vastly different transfer rates, the ratio/size permeation curves of proteins at the blood-CSF barrier and the blood-lymph barrier have comparable characteristics. Therefore the protein gradients found in various disease states are interpreted by means of Renkin's general law of lymph formation. Declined gradients are caused either by an increased permeability of the barrier sites or by a decreased turnover rate of the CSF within the compartment punctured. The concentration ratios of immunoglobulins are related to the gradient that is constructed with the ratios of the barrier-indicative marker proteins. As judged by comparative disc electrophoresis of serum and CSF, those disease states that are dominated by barrier impairment are used to establish the range of concentration ratios, compatible with a passive immunoglobulin transfer in any condition in which the barrier is disordered. A mathematical approach is described, which allows the quantitative evaluation of the minimal immunoglobulin portion that is synthesized within the central nervous system.

The authors have elaborated a biochemical technique for the assessment of the selectivity of proteinuria by means of the clearances of ceruloplasmin, haptoglobin and albumin. The results obtained in 69 patients were compared to those established by the method of Cameron and Blanford, as well as by the clearance of alpha2-macroglobulin. No significant differences were found in the indices of proteinuria, as determined by the proposed biochemical technique and by the method of Cameron and Blanford. Thus the diagnostic value of both methods to assess the selectivity of proteinuria could be considered as equal. The additional measurement of the selectivity of proteinuria by means of proteins with considerably high molecular weight (haptoglobin, alpha2-macroglobulin) gave the possibility of recording cases with severely impaired glomerular permeability. In this respect, the use of the clearance of haptoglobulin, particularly the phenotype Hp 2--2, provided valuable diagnostic information in cases in which the routine methods gave borderline values for the index of proteinuria.

In thirty children hospitalized with acute benign, short-duration gastroenterocolitis, no obligate pathogens were isolated from stools. Five bleedings were established from each patient in order to obtain the protein profiles of albumin, orosomucoid, haptoglobin, alpha2-macroglobulin, ceruloplasmin, transferrin, C3-component, C-reactive protein, immunglobulins IgG, IgA, IgM and IgD. The proteins were quantitated by the single radial immunodiffusion method. The initial drop in some of the proteins followed may be related to general protein loss, negative nitrogen balance or hemodilution. The absence of a significant increase in all the investigated immunoglobulin classes contrasted with remarkable increase in haptoglobin and orosomucoid, both reaching normal levels in late convalescence. C-reactive protein could be demonstrated in half of the children showing early normalization with disappearance of clinical symptoms. In contrast to ceruloplasmin and C3- component, alpha2-macroglobulin was not involved in the acute phase protein reaction.

Using single radial immunodiffusion, ten glycoproteins from non purulent pleural fluids have been estimated in different diseases. For five proteins (prealbumin, ceruloplasmin, alpha2HS-glycoprotein, transferrin, beta2-glycoprotein 1) the results have been found not to correlate with the causal disease. However for orosomucoid, alpha1-antitrypsin, haptoglobin, alpha2-macroglobulin and hemopexin, there was good correlation between proteins levels and aetiology. The glycoprotein concentration was low in mechanical effusions from cirrhosis and chronic cardiac failure. It was high in inflammatory, post-embolism and particularly neoplastic effusions. A raised orosomucoid level occurred as the most characteristic of cancer states especially when associated with a parallel increase of the four other glycoproteins. A simultaneously elevated level of these five pleural glycoproteins seems to be a good and significant biological sign for neoplastic effusion diagnosis.

Proteins of vaginal fluid from normal and hysterectomized women were identified and compared with those of cervical mucus. Albumin, alpha1-antitrypsin, alpha2-haptoglobin, alpha2-macroglobulin, beta-lipoprotein, orosomucoid, ceruloplasmin, gamma chains, gamma G. K. (Bence Jones), and immunoglobulins G, A, and M were present in both vaginal fluid and cervical mucus. Fibrin and C-reactive protein were not found in the vaginal fluid but were identified in cervical mucus. alpha2-Haptoglobin, alpha2-macroglobulin, beta-lipoprotein, orosomucoid, and immunoglobulin M were absent in the vaginal fluid of hysterectomized women, indicating that their presence in the vaginal secretion of normal women may be due to contamination from cervical mucus.

Clofibrate, a widely used hypolipidaemic agent was given for twelve weeks to ten subjects with hypertriglyceridaemia. Its effect on lipoprotein-lipids and caeruloplasmin, IgA, IgM, alpha2-microglobulin and transferrin was assessed by comparing analyses at 4, 8 and 12 weeks on therapy with the means of values at two weeks before and at the start of treatment. The normal variation in plasma proteins was assessed in six healthy volunteers during the same period of time. On clofibrate, very low density lipoprotein (VLDL) cholesterol and triglyceride concentrations fell, but the concentrations of cholesterol in low density (LDL) and high density (HDL) lipoproteins showed no consistent change. Caeruloplasmin and IgM concentrations decreased significantly, IgA showed a limited falls (significant only at 8 weeks) and alpha2-macroglobulin did not change. The concentration of transferrin increased on therapy. No relationships were found between the falls in VLDL-lipid concentrations and the alterations in other plasma proteins. No significant variation occurred in the concentrations of lipids or proteins in the normal subjects during the period of study. The results indicate that clofibrate exerts general effects on protein metabolism.

For detection of plasma proteins produced by human malignant tumors, a survey of blood plasma obtained from nude mice bearing serially transplanted human tumors was performed by immunoelectrophoresis and the double immunodiffusion technique. Among 34 lines including 18 types of human tumors, human specific plasma proteins were demonstrated in the plasma of nude mice transplanted with two lines of renal cell carcinoma, one adenocarcinoma of the colon, and one squamous cell carcinoma of the maxillary sinus. These tumors can be designated as "ectopic" plasma protein-producing tumors since the organs or tissues from which they originated are not considered to be usual sites of plasma protein synthesis. Plasma protein production, as well as that of alpha1-fetoprotein, was also found in one line of hepatoblasotma and three lines of yolk sac tumors. The above tumors were shown to produce one or more of the following 10 of 20 plasma proteins examined: albumin, prealbumin, alpha1-antitrypsin, ceruloplasmin, alpha2-macroglobulin, hemopexin, haptoglobin, C3 and C4 component of complement, and transferrin. An immunochemical demonstration of human specific cancer products observed in human tumors xenotransplanted into nude mice may provide a new approach for investigating the metabolism of neoplastic cells.
