All seven pure yolk sac tumors of gonadal and extragonadal origin tested showed a bright positive fluorescence for alpha-fetoprotein in the tumor tissue. A positive reaction was seen in both the tumor cells and the hyaline globules. In all cases, however, the positive fluorescence was distributed in some focal areas of the tumor tissue. Certain tumor cells showed a strong granular intracytoplasmic fluorescence, whereas others showed a weak or a negative fluorescence. The fluorescence-positive tumor cells were located mainly in the areas rich in fluorescence-positive hyaline globules. Besides alpha-fetoprotein, certain plasma proteins--albumin, alpha-1 antitrypsin, and transferrin--were also demonstrated in all five yolk sac tumors tested. The pattern of the distribution of positive fluorescence was basically similar to that of alpha-fetroprotein. Other plasma proteins--orosomucoid, haptoglobin, Gc-globulin, alpha-2 macroglobulin, hemopexin, and ceruloplasmin--were present in certain tumors, and were distributed mainly in a limited number of hyaline globules. Both IgG and IgA were present in two tumors of ovarian origin. The immunoglobulins were for the most part present in extracellular hyaline globules, suggesting that these are taken up from the circulation. Test for fibrinogen, beta-lipoprotein, IgM, IgE, beta-1C/beta-1A and beta-1E globulins were negative or questionable. In a hepatoblastoma, tests for alpha-fetoprotein were positive, but those for other plasma proteins were negative. Fine granular fluorescence was seen in each hepatocellular tumor cell. Mesenchymal elements were virtually unstained.

Human liver-specific gene products are expressed by hybrid cells resulting from the fusion of human amniocytes with mouse hepatoma cells. Amniocytes grown from human amniotic fluid have no detectable levels of secreted human albumin, transferrin, alpha-1 antitrypsin, or ceruloplasmin, while the mouse hepatoma line, HH--, secretes several mouse liver-specific gene products including transferrin and albumin. Fifty-five hybrids were isolated and analyzed for the expression of serum proteins by Ouchterlony double diffusion and Laurell immunoelectrophoresis. All hybrids continued to express mouse albumin and transferrin, and 29 hybrids from this series were found to express one or more human serum proteins. Activation of the human amniocyte genome provides a model for prenatal diagnosis of serum protein abnormalities.

Serial determinations of serum C-reactive protein are helpful in the detection and monitoring of postoperative complications associated with inflammation and/or tissue necrosis. The serum C-reactive protein level begins to increase within sex hours after operation, peaks on the second day, and by the third postoperative day begins to decrease toward the preoperative level. In cases with surgical complications involving inflammation, serum C-reactive protein levels remain elevated and do not show a decline on the third postoperative day. Serum levels of other "acute-phase proteins," such as alpha-1 acid glycoprotein, ceruloplasmin, alpha-1 antitrypsin, and haptoglobin, were found to increase in response to surgical procedures, but subsequent to the increase in C-reactive protein. An objective approach for monitoring the treatment of acute pulmonary exacerbation in cystic fibrosis was evaluated. Eleven biochemical markers of inflammation (erythrocyte sedimentation rate, neutrophil count, C-reactive protein, alpha-1 antitrypsin, haptoglobin, ceruloplasmin, fibronectin, alpha-1 glycoprotein, alpha-2 macroglobulin, C3, granulocyte elastase and anti-Pseudomonas IgG) were measured in blood serum and plasma from 46 cystic fibrosis patients with chronic Pseudomonas aeruginosa colonization before and after treatment. The overall outcome in each patient was evaluated by means of a pondered sum of clinical, chest X-ray and lung function scores. Biochemical markers were related to the overall clinical improvement: haptoglobin, ceruloplasmin, fibronectin and alpha-1 glycoprotein showed a good sensitivity (64-70%), specificity (60-70%) and positive predictive value (86-89%). Granulocyte elastase showed a similar sensitivity (67%) and positive predictive value (85%) but a lower specificity (33%). The negative predictive value was generally poor (32-39%). Our data suggest that the combined measurement of some markers of inflammation and of conventional clinical parameters, may help in evaluating the efficacy of anti-infective treatment in cystic fibrosis.

Sixty follicular fluids obtained from 26 women with either clomiphene citrate and human menopausal gonadotropins (hMG) or hMG-induced ovulation were analyzed for the contents of total proteins, fibrinogen, plasminogen, antithrombin III, ceruloplasmin, alpha-2 macroglobulin, alpha-1 antitrypsin and immunoglobulins (IgG, IgA, IgM). Concentrations of these proteins was correlated to the type of ovarian follicle growth induction. Follicular fluids from patients stimulated with clomiphene citrate-hMG contained significantly higher concentrations of ceruloplasmin than those treated with hMG alone. No significant differences in the concentrations of other proteins were noted between the two types of ovarian induction. A multivariate data analysis resulted in three Varimax factors (VRX I) suggesting that proteins with antiprotease activity in the follicular fluid may play a role in human follicle maturation. Follicular fluid Ig may reflect the degree of follicular wall permeability under hMG treatment. Accordingly, it may be assumed that a combination of different proteins described by VRX factors could be used for evaluation of ovarian stimulation.

Macrophages activated at sites of tissue injury produce interleukin-1, which induces hepatocytes to synthesize acute phase proteins (APP). Daily serum levels of C-reactive protein (CRP), haptoglobin (HPT), transferrin (TRF), alpha-1 antitrypsin, and ceruloplasmin (CER) were measured in 60 patients, 30 having inguinal herniorrhaphy (H), 18 cholecystectomy (C), and 12 major abdominal trauma (MAT). APP response was proportional to the level of tissue injury. CRP rose in all groups, MAT greater than C, which was greater than H. HPT levels were depressed in MAT, presumably due to removal of hemoglobin-HPT complexes from the serum. TRF was severely depressed in MAT and may be implicated in the higher infection susceptibility in this group. CER was elevated in C, suggesting a stimulating mechanism in this group as opposed to H and MAT. Explanation for this is unknown. APP changes, especially CRP, may be useful as markers of the amount of tissue damage.

Serum viscosity's increase in diabetes has been linked to the presence of microvascular sequelae and to changes in serum protein composition. The major change is a decline in albumin and an increase in the levels of acute-phase proteins. In this study, albumin and five acute phase proteins--alpha-1 acid glycoprotein, alpha-1 antitrypsin, haptoglobin, ceruloplasmin, and C-reactive protein--were measured. Levels in adult diabetes (principally type II) were compared with those in both subjects with glucose intolerance and control subjects (healthy subjects and nondiabetic ambulatory patients). Haptoglobin, alpha-1 acid glycoprotein, and C-reactive protein increased markedly in both diabetes and glucose intolerance; ceruloplasmin and alpha-1 antitrypsin increased more marginally. Serum albumin level decreased more strikingly as hyperglycemia advanced. Acute-phase proteins also increased in advanced glucose intolerance as in established diabetes. The acute-phase protein elevation did not differ with degree of control or duration of diabetes. When diabetics were divided into those with and without clinically detectable evidence of microvascular sequelae, elevation of haptoglobin, C-reactive protein and alpha-1 acid glycoprotein, and depression of albumin were found to progress with number of sequelae. The levels of these proteins, particularly haptoglobin, were also highly correlated with serum viscosity expressed as viscosity number. Mild serum albumin depression and a more striking acute-phase protein elevation are greater in diabetes with microangiopathy, develop in glucose intolerance, and contribute substantially to elevated plasma viscosity in diabetes.

Inflammation and injury influence plasma proteins and immunoglobulins. They respond to noxious agents by a rise of c-reactive protein (CRP), orosomucoid, fibrinogen, haptaglobulin, alpha-1 antitrypsin, ceruloplasmin, C 3 and C 4 components of complement. The rate of rise and return to normal values varies. CRP reacts most'promptly. The following substances react by a decline of the levels: albumin, prealbumin, transferrin, alpha-1 lipoprotein and immunoglobulins. Investigation of the patient's immunological state is of prognostic value. CRP, orosomucoid, albumin and prealbumin are considered the most valuable indicators; from their values the so-called PINI index can be calculated and the patients can be divided into five prognostic zones. To threatened patients all-round care must be devoted incl. aimed provision of ATB and immunoprotective substances.

A heterogeneous group of surgery patients at septic risk was studied through monitoring of acute phase proteins (APP). Plasma levels of 8 acute phase proteins (C-reactive, alpha-1 antitrypsin, fibrinogen, ceruloplasmin, transferrin, albumin, prealbumin, alpha-2 macroglobulin) were measured in the pre- and postoperative period of septic surgery patients suffering from disease processes in various sites and with different aetiopathogenesis. The experiment is related to some interesting research proposed by authors in the Shock Physiopathology Study Centre of the Catholic University of Rome and their results. The constant finding of increased values of PCR, fibrin, cerul, alpha-1 Tryp in the septic risk surgery patient and their normalisation following treatment is particularly significant. In these same cases, a depression or, depending on circumstances, normal findings of Transf, alpha-2 Macro, Albu and Prealbu were observed. The results point to the practical utility an high reliability of APP changes as markers of septic risk.

A biochemical and immunological survey of a selected military group has been performed, before and after a jungle raid. Biochemical (total proteins, albumin, proteins electrophoresis, transferrin, haptoglobin, ceruloplasmin, orosomucoid, alpha-1 antitrypsin and alpha-2 macroglobin) and immunological (Ig G., Ig A., C 3c, C 4, specific antibodies against Leishmania) analysis of sera have been studied as well as the level of cellular immunity (Multitest scoring, macrophage cytotoxicity, interleukin-1 and prostaglandin E-2, macrophage production). The results show some modifications as a consequence of the work in tropical forest, particularly the tendency to some cellular immunity deficiency. This can be linked to several factors: nutritional balance, cutaneous infections, sustained physical activity and excessive sweating.
