A wide variety of tumour markers has been described in bronchial carcinoma. Clinical studies have been most frequently conducted with the substances CEA,
NSE
, CK-BB and for the peptid hormones ACTH, calcitonin and
ADH
. The serum levels for CEA,
NSE
and CK-BB correlate to a certain extent with the stage of the tumour disease, the prognosis and the survival time. On the other hand, the peptid hormones have no clinical significance on account of their low sensitivity and specificity. Outside of clinical studies the determination of tumour markers in bronchial carcinoma is clinically irrelevant.

Bronchial carcinomas are frequently associated with ectopic secretion of hormones which may be responsible for paraneoplastic syndromes. In non small cells carcinomas, serum calcitonin levels may be raised. Hypercalcaemia can be found in squamous carcinomas and secretion of hCG (responsible for gynaecomastia) in large cells carcinomas. In small carcinomas, many hormones (ACTH, MSH,
ADH
, calcitonin) can be produced; however, their serial measurements as well as that of the carcino-embryonic antigen add nothing to the information available with standard staging investigations. More recent studies have identified three protein products: the BB-isoenzyme of creatine kinase, bombesin and
neurone-specific enolase

, E.C. 1.1.1.2). Their enzyme types were related to those of T. cruzi. The comparison of enzyme patterns of the six enzymes has shown that each species was characterized by species-specific enzyme profiles. Among the stocks of the European species, T. dionisii, and T. vespertilionis, variations of the enzyme patterns of PGM, G-6-PD and GPI suggesting that the final status of this subspecies is probably not yet established. In relation to T. cruzi it has been found that T. dionisii showed identical enzyme profiles with group II of T. cruzi. For T. vespertilionis no enzyme types identical with T. cruzi were detectable. T.c. marinkellei showed only identical enzyme patterns to T. cruzi-group I by the enzymes

is strictly dependent on the abilities of different mutant strains to increase the level of glucose-6-phosphate after the addition of sugars. In contrast, the induction of
alcohol dehydrogenase
I is dependent on increasing concentrations of metabolites in the late stages of glycolysis.

The patient was a 63-year-old male admitted for further evaluation of the bleeding esophageal tumor. Endoscopic biopsy revealed small cell carcinoma. CT scan of the abdomen demonstrated nodular enlargement at the celiac axis. Under diagnosis of small cell carcinoma of the esophagus at Stage IVa, neoadjuvant chemotherapy with FP (5-FU+CDDP) was given. Immediately after fluid load, levels of serum sodium decreased to 117 mEq/L and persisted during chemotherapy treatment despite aggressive corrections. Response and shrinkage of the distant nodal metastases were confirmed, and an esophagectomy was conducted. Pathological examination with IHC demonstrated positive staining for CD56,
NSE
and synaptophysin but negative for
ADH
. Lymph node and liver metastases recurred. Progression of the disease again triggered hyponatremia.
