Using immunoblotting, the antigenic structure of 6 Leishmania strains has been studied: 1) MHOM/IN/80/
DDS
--Leishmania (Leishmania) donovani; 2) MHOM/SU/63/VL--L. sp. ZMA; 3) MHOM/SU/73/K-27--L. tropica; 4) MHOM/SU/73/5Ash--L. major; 5) MHOM/GE/84/H-132--L. mexicana amazonensis; 6) REPT/SU/83/3960-GC--L. (Sauroleishmania) gymnodactyli. Antigens of Leishmania surface membranes and rabbit antisera against them have been used. Among the agents of leishmaniasis in the Old World (1-4), the most intensive antigenic lines were found in the medium and high-molecular mass area (43-200 kD). In L. (L) mexican a amazonensis (5) species-specific lines have been identified in 10-22 kD area, which is indicative of considerable antigenic differences in Leishmania of the Old and New Worlds. The most marked antigenic differences from other types were noted in L. (S) gymnodactyli (6). It was characterized by the absence of antigenic bands in the medium and high molecular mass areas, the lines associated with species-specific determinants located in the low molecular mass area (18 and 25 kD). The results of cross-reacting c-ELISA using the same antigens and antisera correlated well with the above data of immunoblotting. Immunoblotting may be used for identification of species-specific antigenic structures which may be helpful in serological tests for more accurate leishmania identification and leishmaniasis diagnosis.

From a series of thirty six 2,3, N-substituted 4,4'-diaminodiphenyl sulphones studied for their suppressive activity in mice against blood induced erythrocytic stage of Plasmodium berghei infection, six sulphones (1-6) showed 100% suppressive and curative activity at an intraperitoneal dose of 1 mg/kg x 4 days. These sulphones have been studied for their suppressive activity in still lower doses ranging from 1.0-0.25 mg/kg i.p. x 4 days and for their curative activity at 1 mg/kg i.p. x 4 days in comparison to

as standard drug. The maximum tolerated dose of these compounds and

has also been determined. These sulphones have better therapeutic efficacy for their suppressive and curative action than

The feasibility and effects of a 3-year treatment using rifampicin (RFP), clofazimine (B663) and dapsone (
DDS
) in multibacillary leprosy patients in Yangzhou Prefecture and Dongtai County (1983-1986) are reported. Among 591 active multibacillary leprosy patients in the two areas, 569 (96.30%) were treated with this regimen. Of 303 cases available for analysis, 196 (64.7%) cases showed negative skin smears and clinical inactivity. The rest showed different degrees of improvement. The average reduction of BI was 0.78. The intensity and frequency of ENL and neuritis decreased markedly with treatment. The main side-effects were pigmentation and ichthyosiform changes of the skin, but these did not influence treatment.

Airway reactivity and disease severity were investigated in 24 subjects with stable chronic bronchial asthma. Disease severity was determined by assigning a disease severity score (DSS) representing six clinical and therapeutic parameters. Airway hyperresponsiveness was assessed in two ways: airway reactivity score (ARS) based on the number of positive responses to a question concerning exposure to 22 nonspecific inhaled irritants and methacholine challenge testing and determining the cumulative dose causing a 20% reduction in FEV2 (CMD20). A significant correlation between
DDS
and CMD20 (r = 0.57; p less than 0.003) and DSS and ARS (r = 0.67; p less than 0.0003) attested to the important influence of airway hyperresponsiveness on disease severity. Significant correlations for ARS with CMD20 (r = -0.60; p less than 0.002) suggested the consistency with which the ARS estimated methacholine hyperresponsiveness. We found no statistically significant correlations between DSS, ARS, or CMD20 and the age of subject, duration of asthma, or other host characteristics. There was not a significant correlation between the degree of airway obstruction and DSS or ARS noted. The results of this investigation demonstrate the value of the use of clinical information for assessing airway hyperresponsiveness and disease severity in patients with chronic stable asthma. Both ARS and DSS are useful clinical tools for estimating methacholine reactivity.

The administration of an Elemental Diet to 5 patients with dermatitis herpetiformis, requiring high doses of
Dapsone
(diaminodiphenylsulfone,
DDS
), showed a rapid and beneficial effect on the skin lesions within two weeks. This effect was not influenced by simultaneous gluten challenge in one patient. A possible explanation is a reduction in the amount of harmful immune complexes due to the elimination of proteins from the diet. Subsequent introduction of a more comprehensive diet led to an increase of the minimal effective dose of
Dapsone
. These results underline the importance of dietary influences on the skin activity in dermatitis herpetiformis, other than gluten alone.

Serotonin is N-acetylated to melatonin. The purpose of this study was to explore the possibility of N-acetylation of dapsone reflecting serotonergic activity. The ratio of monoacetyldapsone/dapsone (MAD/
DDS
) in plasma, 5-HIAA in CSF, and imipramine-binding to platelets were investigated in a group of psychiatric patients, diagnosed according to the DSM-III as affective disorders, schizophrenia, and personality disorders. There was no significant correlation between either of the serotonergic estimates and N-acetylation in the whole patient group or in diagnostic subgroups of patients. Sixty-four percent of the patients were slow N-acetylators (MAD/
DDS
less than 0.4), which is a ratio in line with several other studies of psychiatric patients. Among patients with affective disorders, all unipolar patients were slow N-acetylators, while five out of six bipolar patients were fast N-acetylators. The N-acetylation of patients with a history of suicide attempt did not differ from those without. The discrepancy in N-acetylation between uni- and bipolar patients might again address the issue of them representing two different biochemical and genetic disorders.

The inhibitory effects by the combined doses of bacteriostatic
DDS
and several immunostimulants on the growth of leprosy bacilli (LB) inoculated into the footpads of nude mice were examined, using a strain of hybrid nude mice named Jcl:AF-nu, which is resistible to infections more than strain BALB/c-nu. The results found were: 1. LB could proliferate up to 10(9) level in the footpads of this strain of hybrid nude mice. 2. The combined dose of muramyl dipeptide (MDP) or a water-soluble lipoidal amine, CP-46665 and
DDS
mixed with chow in the content ratio of 0.005% completely inhibited the growth of LB. Whereas, the growth inhibition by
DDS
alone was only partial. 3. A derivative of MDP, muroctasin could partially inhibit the growth of LB without combined dose of
DDS
. 4. The combined dose of an antitumor beta-1, 3-glucan named ATSO could not enhance the partial growth inhibition due to the dose of
DDS
through 0.005%-

chow. Based on these results, the possibility of Jcl:AF-nu mice as a favorable animal model for examining the synergic inhibitory effect on the growth of LB due to the combined dose of an antileprous chemotherapeutic and an immunostimulant was discussed.
